Unexpected decline of pulmonary function in a patient with thymoma mimicking obstructive bronchiolitis.

Obstructive bronchiolitis (OB) is a rare disorder characterized by progressive airflow limitation and is most commonly associated with autoimmune diseases, inhalation of toxins, lung transplantation, and hematopoietic stem cell transplantation. Thymoma is a rare mediastinal tumor frequently accompanied by autoimmune diseases, such as myasthenia gravis. We report a male patient with mild chronic obstructive pulmonary disease who developed thymoma and severe respiratory dysfunction mimicking OB. Postmortem autopsy revealed histopathological findings resembling systemic graft-versus-host disease

The probability of reducing hospitalization rates for bronchiolitis with epinephrine and dexamethasone: A Bayesian analysis.

BACKGROUND: Bronchiolitis exerts a high burden on children, their families and the healthcare system. The Canadian Bronchiolitis Epinephrine Steroid Trial (CanBEST) assessed whether administering epinephrine alone, dexamethasone alone, or in combination (EpiDex) could reduce bronchiolitis-related hospitalizations among children less than 12 months of age compared to placebo. CanBEST demonstrated a statistically significant reduction in 7-day hospitalization risk with EpiDex in an unadjusted analysis but not after adjustment. OBJECTIVE: To explore the probability that EpiDex results in a reduc

Regional Differences and Time Trends in the Treatment of Infants Hospitalised With Bronchiolitis in Norway.

AIMS: The Norwegian national recommendations for the evaluation and treatment of bronchiolitis were updated in 2018. Two Choosing Wisely recommendations were launched to reduce unwarranted chest X-ray, repeated blood tests and routine inhalations. The aim of this study was to evaluate adherence to the recommendations, temporal changes and differences between hospitals. METHODS: In this multicentre retrospective study, we compared routine data from 1186 infants < 12 months of age hospitalised for bronchiolitis at five Norwegian hospitals during the winter seasons 2017-2018, 2018-2019 and 2021-

Bronchiolitis in Early Infancy Is Associated With Irreversible Airway Obstruction in Young Adulthood.

BACKGROUND: Childhood risk factors, such as bronchiolitis, have been associated with airway obstruction later in life. However, assessing childhood risk factors for airway obstruction in adulthood requires long follow-ups with carefully gathered data. Our long-term follow-up represents lung function in early adulthood after infant bronchiolitis with evaluation of asthma and tobacco smoke exposure as childhood risk factors. METHODS: We prospectively followed 166 term children hospitalized for bronchiolitis under 6 months of age. Lung function was measured at the mean age of 18.3 years by spiro

Nirsevimab and Acute Bronchiolitis Admissions in Infants Under One Year of Age.

BACKGROUND AND OBJECTIVES: Spain was among the first countries to implement universal prophylaxis for respiratory syncytial virus (RSV) using nirsevimab, leading to a 60%-80% reduction in bronchiolitis hospital admissions. The main objective of this study was to describe the characteristics of infants admitted with bronchiolitis after RSV prophylaxis had been established. METHODS: A retrospective study was conducted in 15 Spanish pediatric emergency departments during the 2023-2024 RSV season. We reviewed medical records coded as "acute bronchiolitis" of infants aged less than 12 months who w

Clinical Profile and Outcome of Bronchiolitis in Children With 1-24 Months of Age.

Introduction Bronchiolitis poses a significant challenge in pediatric critical care. It is an acute illness affecting the lower respiratory tract in children under the age of two. The most common cause of bronchiolitis is the seasonal respiratory syncytial virus, with influenza and adenovirus also notable contributors. It is characterized by various clinical symptoms and indicators, such as an upper respiratory prodrome, increased respiratory effort, and wheezing in younger children under two years old. This study primarily examines the clinical profile, risk factors, severity, and outcomes of

Bronchiolitis guidelines: what about the Italian situation in a primary care setting?

Acute viral bronchiolitis is the most common cause of hospitalization in children under 12 months of age. The variable clinical presentation and the potential for sudden deterioration of the clinical conditions require a close monitoring by healthcare professionals. In Italy, first access care for children is provided by primary care physicians (PCPs) who often must face to a heterogeneous disease presentation that, in some cases, make the management of patient with bronchiolitis challenging. Consequently, Italian studies report poor adherence to national and international guidelines processed
