OBJECTIVES: This study describes a series of cases best explained by invoking the left septal fascicle (LSF) as a critical component of the arrhythmia circuit.
BACKGROUND: Numerous anatomic studies have shown evidence of the LSF, but its precise role in the onset of arrhythmia is unclear.
METHODS: This paper presents 5 cases that implicated the LSF as a critical component of arrhythmogenesis.
RESULTS: The first case had ventricular fibrillation repeatedly documented after a single premature atrial complex, produced left-sided conduction delay and simultaneous earliest activation of the left

Most clinicians currently rely on patient history and specific electrocardiographic criteria to establish the diagnosis of arrhythmias. However, as illustrated by this case, the physical examination-especially auscultation of the first heart sound-can also provide useful diagnostic clues.

INTRODUCTION: Right ventricular apex (RVA) pacing has been reported to induce pacing-induced cardiomyopathy (PICM), with biventricular pacing being the standard cardiac resynchronization therapy (CRT) for RVA-PICM. However, recent studies suggest that left bundle branch area pacing (LBBAP) may provide even better outcomes as a CRT. In this case, we observed a dynamic alteration in the left ventricular (LV) activation pattern when transitioning from RVA-PICM to LBBAP, including changes in the functional block line.
CASE: A female patient with dilated-phase hypertrophic cardiomyopathy (d-HCM),

AIMS: Left bundle branch area pacing (LBBAP) is a recent technique aiming at preservation of physiological ventricular electrical activation. Our goal was to assess mechanical synchrony parameters in relation to electrocardiographic features during LBBAP performed in routine practice.
METHODS AND RESULTS: From June 2020 to August 2021, all patients of our institution with permanent pacemaker implantation indication were eligible for LBBAP. A 'qR' pattern in V1 and a delay from pacing spike to the peak of the R-wave in V6 < 80 ms defined a successful LBBAP. Electrocardiogram and echocardiograp

BACKGROUND: It was recently shown that template beats and fixation beats of the premature ventricular contractions (PVCs) were generated during lead deployment. These could be exploited to guide the left bundle branch (LBB) pacing (LBBP) procedure. However, lack of a revolving connector that can continuously record and pace during lead rotations has been a limitation when using the traditional implant technique. Here, we report ten cases in which a revolving connector was used and showed that the premature beats of selective left bundle branch (SLBB-PBs) were generated as the lead was reached

A 37-year-old man was admitted to our hospital with paroxysmal palpitation for half year. A previous electrogram showed a narrow complex tachycardia. Electrophysiologic study (EPS) found a concealed left-sided free wall pathway accessory. In addition, a transseptal approach was used for radiofrequency ablation. After successful ablation, EPS induced a wide complex tachycardia and a narrow complex tachycardia. The wide complex tachycardia was diagnosed as a right-sided Mahaim fiber atriofascicular accessory pathway, and the narrow complex tachycardia was diagnosed as atypical atrioventricular n

OBJECTIVES: The aim of the study was to report the efficacy of left bundle branch pacing (LBBP) in the management of left bundle branch block (LBBB)-induced cardiomyopathy (LIC).
BACKGROUND: Chronic LBBB is known to cause mechanical dyssynchrony and cardiomyopathy. Hyperresponse to cardiac resynchronization therapy (CRT) with biventricular pacing (BVP) is a hallmark of LIC. LBBP has recently shown promise as an alternative to BVP.
METHODS: Patients undergoing CRT between 2018 and 2020 were retrospectively screened, and those who met the criteria for LIC were included in the study. Duration o

A 14-year-old male with a history of symptomatic tachycardia was referred for ablation. Sinus rhythm electrocardiogram was not showing any pre-excitation. Tachycardia episode was showing antidromic tachycardia with left bundle branch block morphology. Echocardiographic examination was showing Ebstein's anomaly with septal tricuspid leaflet displaced 24mm apically. The right bundle branch block (RBBB) was concealed during sinus rhythm. The RBBB was revealed with ablation of right posterior atriofascicular accessory pathway.

We report a case of a 7-year-old girl with atriofascicular Mahaim (AFM) pathway concomitant with Ebstein's anomaly. The QRS wave showed left bundle branch block pattern on electrocardiogram. Holter electrocardiogram showed prolongation of the PR interval and QRS morphological change during sinus tachycardia. An electrophysiological study demonstrated that the distal His potential appeared earlier than the proximal His potential, which suggested retrograde His conduction toward the atrioventricular node. Conduction from the Mahaim fiber to the His bundle was faster than that from the atrioventr
