Salvage therapy of complicated Candida albicans spondylodiscitis with Rezafungin.

Invasive fungal infections, particularly those caused by Candida species, represent an increasing cause of hospital-related morbidity and mortality. Most commonly Candida spp. is the causative agent of candidemia and invasive candidiasis. In this report we present a complex case of Candida albicans spondylodiscitis, successfully treated with rezafungin, a new echinocandin suitable for weekly outpatient administration. This case illustrates the first documented use of rezafungin in an outpatient setting for C. albicans spondylodiscitis, demonstrating its potential in managing complex fungal inf

Emergence and Genomic Profiling of Multidrug-Resistant Candida auris Isolated from Intensive Care Unit Patients in a Malaysian Teaching Hospital.

Candida auris, a significant pathogen causing invasive candidiasis with high mortality rates and frequent hospital outbreaks, is understudied in Malaysia. This study retrospectively examined invasive candidiasis cases at Universiti Malaya Medical Centre from 2018 to 2021. Four C. auris strains were isolated from blood and peritoneal samples of intensive care unit patients and identified using matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF MS). Antifungal susceptibility testing using Sensititre™ YeastOne YO10 plate showed resistance to amphotericin B (2-

Drug design strategies for the treatment azole-resistant candidiasis.

INTRODUCTION: Despite the availability of novel antifungals and therapeutic strategies, the rate of global mortality linked to invasive fungal diseases from fungal infection remains high. Candida albicans account for the most invasive mycosis produced by yeast. Thus, the current arsenal of medicinal chemists is focused on finding new effective agents with lower toxicity and broad-spectrum activity. In this review article, recent efforts to find effective agents against azole-resistant candidiasis, a common fungal infection, are covered. AREAS COVERED: Herein, the authors outlined all azole-ba

Management of early graft candidiasis in a kidney transplant recipient.

Balancing adequate immunosuppression with the risk of infection after renal transplantation remains a challenge. The presence of comorbidities adds to the challenge. Although infrequent, invasive fungal infections result in high morbidity and mortality risk in renal transplant recipients. This can be attributed to the intense immunosuppression in the first 6 months after renal transplantation, minimal symptomatology and the high mortality associated with fungal infections.Due to minimal available evidence, clinical judgement guides management of graft candidiasis. There is a need to develop ev

[Chinese consensus on the diagnosis and management of adult candidiasis].

In this consensus we mainly present the etiology, epidemiology, risk factors, microbiology, clinical diagnosis and general principles of treatment, as well as the detailed diagnosis and treatment of common candidiasis. Though mucocutaneous candidiasis such as oral, esophageal and vulvovaginal infections are known as non-invasive diseases, they are included in this consensus, considering their high prevalence and the potential to be the source of systematic infections. In order to be accordant with the practical situation in China, new technologies and drugs that have not been well developed or

T2Candida for the Diagnosis and Management of Invasive Candida Infections.

Invasive candidiasis is a common healthcare-associated infection with high mortality and is difficult to diagnose due to nonspecific symptoms and limitations of culture based diagnostic methods. T2Candida, based on T2 magnetic resonance technology, is FDA approved for the diagnosis of candidemia and can rapidly detect the five most commonly isolated Candida sp. in approximately 5 h directly from whole blood. We discuss the preclinical and clinical studies of T2Candida for the diagnosis of candidemia and review the current literature on its use in deep-seated candidiasis, its role in patient ma

Invasive fungal infection in the immunosuppressed host.

Immunosuppression, whether arising as a consequence of disease (haematopoietic and lymphoreticular malignancies) or therapy (against hemograft rejection or malignancy results in a higher than normal incidence of invasive fungal infections such as candidiasis, aspergillosis, mucormycosis and cryptococcosis. Normal host defense mechanisms, both immunologic and non-immunologic, are not fully functional and may contribute to the pathogenesis of these diseases.Candida, normally a superficial colonizer, may invade the gastrointestinal, respiratory or urinary tracts. Aspergillus and mucor species may

Overview of host defenses in fungal infections.

Humans have a broad array of host defense mechanisms against fungal invasion. Most serious mycoses occur in patients who have defects in one or more of these defenses. Defense mechanisms not related to the immune system include barriers created by the skin and mucous membrane, competition from the normal indigenous bacterial flora, mucociliary clearance mechanisms, and fungistatic products in serum such as metal chelators. Among the immune mechanisms, activation of the complement system and generation of an antibody response facilitates recognition of fungi by specific receptors on immune cell

Pulmonary mycetoma following Mycobacterium kansasii infection. Report of seven cases.

A long-term follow-up of 263 patients with pulmonary Mycobacterium kansasii infection disclosed seven cases of mycetoma. We report the clinical manifestations of these patients. The incidence was less than that of tuberculosis. All mycetomas originated in large cavity lesions of inactive M kansasii infection. Most patients had received multiple antituberculous antibiotics, including rifampin. Five patients had died, two of underlying disease, one of invasive candidiasis following massive hemoptysis, one of surgical complication, and one of a possible invasive aspergillosis.

Unlocking the role of fungal peptide cytotoxin candidalysin in candidiasis and non-candidiasis diseases.

Candida albicans is a commonly encountered opportunistic fungus that can cause both superficial discomforts and life-threatening invasive fungal infections. Ever-increasing reports have shown that C. albicans is also readily implicated in a set of metabolic and autoimmune diseases. Yeast-to-hypha transition is a hallmark of increased virulence and invasion of C. albicans, and candidalysin is the first peptide toxin secreted by C. albicans hyphae. Candidalysin contributes to fungal penetration across epithelial barrier via cytolytic activity, as well as the maintenance of commensalism and patho

Fungal extracellular vesicles as biomarkers in candidiasis: biogenesis, virulence, and CNS diagnostic applications.

Invasive candidiasis remains a life-threatening infection with mortality rates reaching 40-55%, yet current gold-standard diagnostics such as blood culture are often slow and insensitive. This review evaluates fungal extracellular vesicles (EVs) as both mechanistic drivers of pathogenesis and clinical utility as biomarkers. We first outline EV biogenesis and translocation across the fungal cell wall, emphasizing the roles of turgor pressure and enzymatic remodeling. We then discuss how EVs act as protected carriers of virulence factors, such as candidalysin and Als3, which modulate tissue inva

A novel approach to candidemia? The potential role of checkpoint inhibition.

Infections with Candida spp. cause significant morbidity and mortality despite intensive treatment with antifungal agents. Novel treatment options are urgently needed. Predominately immunocompromised patients are affected. This warrants the conclusion that strengthening host immunity may have the potential to improve outcome. Recent studies imply a potential benefit of checkpoint inhibition reversing hyporesponsiveness of innate and adaptive immunity during invasive fungal infections and invasive candidiasis in particular. We here give a brief overview of first preclinical data in vitro and in

Rationale for combination antifungal therapy.

The relentless increase of invasive fungal infections and poor outcomes associated with available antifungal agents prompted the search for better therapeutic strategies. Combining antifungal drugs was recommended as a means to enhance efficacy in a variety of invasive infections including cryptococcosis, candidiasis, and aspergillosis. With the exception of cryptococcal meningitis, data from controlled clinical trials supporting such combinations are sparse. Moreover, little consensus exists regarding which combinations are synergistic or antagonistic in vitro and in vivo. Based on available
