The structural rearrangement of human sheathed capillaries in congestive splenomegaly with hypersplenism has been evaluated by histometric analysis. In abnormal spleens the external diameter and the wall thickness were decreased, while the internal diameter was increased. Moreover, the total length and the absolute number of sheathed capillaries were increased, though not to the same high degree as the volume of the whole red pulp. In 42 patients with a chronic venous insufficiency Stage I-III transcutaneous oxygen tension was measured above the medial ankle and the leg ulcer edges respectively at 44 degrees C. Isolated unilateral pleural effusion is uncommon presentation of ovarian hyperstimulation syndrome. The pathogenesis of this syndrome involves an increased permeability of ovarian capillaries and mesothelial vessels triggered by the release of vasoactive substances by the ovaries under human chorionic gonadotropin stimulation. Physicians should consider this potentially life-threatening diagnosis in all patients who undergo ovarian hyperstimulation. This case highlights a rare clinical manifestation of isolated hemorrhagic pleural effusion associated with ovarian hyperstimulation.
