Heart transplantation outcomes in arrhythmogenic right ventricular cardiomyopathy: a contemporary national analysis.

AIMS: Heart failure is an increasingly recognized later stage manifestation of arrhythmogenic right ventricular cardiomyopathy (ARVC) that can require heart transplantation (HT) to appropriately treat. We aimed to study contemporary ARVC HT outcomes in a national registry. METHODS AND RESULTS: The United Network for Organ Sharing registry was queried for HT recipients from 1/1994 through 2/2020. ARVC patients were compared with non-ARVC dilated, restrictive, and hypertrophic cardiomyopathy HT patients (HT for ischaemic and valvular disease was excluded from analysis). Post-HT survival was ass

Prognostic value of natriuretic peptides and restrictive filling pattern before surgical ventricular restoration.

OBJECTIVE: Both increased natriuretic peptide levels and restrictive filling pattern (RFP) are important risk predictors in patients with heart failure. The aim of this study was to examine the role of the combined use of natriuretic peptide and RFP for the prognostic stratification of patients with ischemic cardiomyopathy undergoing surgical ventricular restoration in the Biomarker Plus study. METHODS: A total of 186 patients (aged 64 ± 10 years) underwent echocardiographic study and N-terminal pro-B-type natriuretic peptide assay at baseline (before surgical ventricular restoration). Patien

Epidemiology, pathophysiology, and etiology of congestive heart failure in older adults.

OBJECTIVES: To review the epidemiology, pathophysiology, and etiology of congestive heart failure (CHF) in older adults. METHODS: Published reports relevant to the epidemiology, pathophysiology, and etiology of CHF were systematically reviewed. Studies involving older adults and more recent studies were emphasized. RESULTS: More than 75% of patients with CHF in the United States are older than 65 years of age, and CHF is the leading cause of hospitalization in older adults. CHF is also a major cause of chronic disability, and annual expenditures for CHF currently exceed $10 billion. In addit

Metabolic syndrome and breast cancer: a dangerous association for postmenopausal women.

BACKGROUND: Breast cancer(BC) is the most common cancer in women worldwide, the relationship between metabolic syndrome(MetS) and BC needs to be better clarified. Today the early diagnosis of breast cancer(BC) is yet a challenging problem in clinical practice, so the evidence that a well identified population of postmenopausal women, affected by MetS, presents a high risk, of breast cancer occurrence, is useful for breast cancer prevention. Our study aims to assess the prevalence of metabolic syndrome, diagnosed according to current guidelines, in postmenopausal women with breast cancer, and i

Ventricular hypertrophy in cardiomyopathy.

Semantic difficulties arise when hypertrophic obstructive cardiomyopathy is seen without obstruction and with congestive failure, and also when congestive cardiomyopathy is seen with gross hypertrophy but without heart failure. Retention of a small left ventricular cavity and a normal ejection fraction characterizes hypertrophic cardiomyopathy at all stages of the disorder. Congestive cardiomyopathy is recognized by the presence of a dilated left ventricular cavity and reduced ejection fraction regardless of the amount of hypertrophy and the presence or not of heart failure. Longevity in conge

Cardiomyopathy associated 5 (CMYA5) implicated as a genetic risk factor for radial hemimelia in Siamese cats.

OBJECTIVES: The present study aimed to determine the inheritance pattern and genetic cause of congenital radial hemimelia (RH) in cats. METHODS: Clinical and genetic analyses were conducted on a Siamese cat family (n = 18), including two siblings with RH. Radiographs were obtained for the affected kittens and echocardiograms of an affected kitten and sire. Whole genome sequencing was completed on the two cases and the parents. Genomic data were compared with the 99 Lives Cat Genome data set of 420 additional domestic cats with whole genome and whole exome sequencing data. Variants were consid

Native T1 mapping in diffuse myocardial diseases using 3-Tesla MRI: An institutional experience.

AIMS: Newer cardiac magnetic resonance techniques like native T1 mapping are being used increasingly as an adjunct to diagnose myocardial diseases with fibrosis. However, its full clinical utility has not been tested extensively, especially in the Indian population. The purpose of this study was to find native T1 values in healthy individuals without cardiac disease in our 3-Tesla MRI system and examine whether native myocardial T1 values can be used to differentiate between normal and diffuse myocardial disease groups. SUBJECTS AND METHODS: After approval from the institutional ethics commit

Pharmaceutical management of decompensated heart failure syndrome in children: current state of the art and a new approach.

Prompt initiation of appropriate and intensive treatment in children with decompensated heart failure is crucial to avoid irreversible end-organ dysfunction. Initial management of these children includes transfer to the pediatric cardiac intensive care unit, basic hemodynamic monitoring, and establishment of intravenous access. Inotropic support should be instituted peripherally before obtaining central venous and arterial access. The team should be prepared for emergent intubation and initiation of mechanical circulatory support. Two experienced physicians should work together to obtain vascu

[Cardiomyopathies due to defective energy metabolism: morphological and functional features].

Cardiomyopathies are defined as diseases of the myocardium associated with cardiac dysfunction and are classified by morphological characteristics as hypertrophic (HCM), dilated (DCM) arrhithmogenic right ventricular (ARVC) and restrictive cardiomyopathy. These were once considered as specific diagnoses but there is now considerable evidence that many different gene mutations can cause these pathologies. In recent years, big emphasis has been given to the possibility that deregulation of cardiac metabolism may play a role in the mechanisms that lead to cardiac maladaptive remodelling. Cardiac

Endomyocardial fibrosis in adults: multimodality imaging phenotype and independent predictors of mortality.

BACKGROUND: Endomyocardial fibrosis (EMF) is a rare restrictive cardiomyopathy characterized by endocardial fibrosis and apical obliteration. Data on its long-term evolution and prognosis remain limited. OBJECTIVES: To describe the clinical profile, multimodality imaging findings, longitudinal evolution and independent predictors of all-cause mortality in adults with EMF. METHODS: We conducted a retrospective and single-center cohort study of consecutive adults (≥18 years) diagnosed with EMF, between 1997 and 2024. Diagnosis was based on clinical features and characteristic transthoracic ech

Mitochondrial Cardiomyopathies.

Mitochondria are found in all nucleated human cells and perform various essential functions, including the generation of cellular energy. Mitochondria are under dual genome control. Only a small fraction of their proteins are encoded by mitochondrial DNA (mtDNA), whereas more than 99% of them are encoded by nuclear DNA (nDNA). Mutations in mtDNA or mitochondria-related nDNA genes result in mitochondrial dysfunction leading to insufficient energy production required to meet the needs for various organs, particularly those with high energy requirements, including the central nervous system, skel

Clinical evaluation of infiltrative cardiomyopathies resulting in heart failure with preserved ejection fraction.

Heart failure with preserved ejection fraction is a very common clinical problem. Its prevalence is increasing with aging of the population. A diverse group of risk factors and etiologies comprise the HFpEF syndrome. No specific therapies have been shown to improve survival for the vast majority of HFpEF cases. Restrictive cardiomyopathies account for a significant portion of HFpEF patients and are characterized by diastolic dysfunction due to infiltration of the myocardium or ventricular hypertrophy. Many of these restrictive diseases occur in the context of myocardial infiltration by other s

The effect of combined therapy with captopril, furosemide, and a sodium-restricted diet on serum electrolyte concentrations and renal function in normal dogs and dogs with congestive heart failure.

Captopril, furosemide, and a sodium-restricted diet were administered to 6 normal dogs and 10 dogs with congestive heart failure. Serum electrolyte concentrations and renal function were monitored in both groups. In the normal dogs, no clinically meaningful changes in serum electrolyte, urea nitrogen, or creatinine concentrations developed during therapy with a sodium-restricted diet and 4 weeks each of furosemide alone, captopril alone, or furosemide plus captopril. Three of 6 normal dogs on furosemide and a sodium-restricted diet had at least one serum potassium concentration above the refer

Low Albumin, Heavy Hearts: Role of Hypoalbuminemia in Infiltrative Cardiomyopathies: A Narrative Review.

Infiltrative cardiomyopathies, a subset of restrictive cardiomyopathies, are rare but clinically essential disorders often lead to heart failure, which is a major global health issue. Although interest in these conditions is growing, they remain poorly understood due to their low occurrence, vague clinical signs, and frequent underreporting. Advances in diagnosis have been made through multimodal imaging techniques-including cardiac magnetic resonance, nuclear imaging, and computed tomography-as well as genetic testing and new biomarkers for amyloidosis. However, a universal biomarker that app

Investigation of de novo variation in pediatric cardiomyopathy.

Pediatric cardiomyopathies can be caused by variants in genes encoding the sarcomere and cytoskeleton in cardiomyocytes. Variants are typically inherited in an autosomal dominant manner with variable expressivity. De novo variants have been reported, however their overall frequency is largely unknown. We sought to determine the rate of de novo, pathogenic and likely pathogenic (P/LP) variants in children with a diagnosis of hypertrophic, dilated, or restrictive cardiomyopathy (HCM, DCM, or RCM), and to compare disease outcomes between individuals with and without a de novo variant. A retrospec

Sudden death in sports.

I have stressed the known medical and environmental factors associated with unexpected, sudden death in sports in children, and I have recommended restrictions or environmental alterations to prevent sudden death associated with an individual sport. Medical problems such as cardiomyopathies, posterior mitral valve leaflet syndrome, and underlying congenital heart problems have been related, when possible, to their role in sudden deaths related to sport. I have emphasized the abnormalities of hypertrophic cardiomyopathy, Marfan's syndrome, and the need to monitor constantly the medical, environ

Exercise Participation and Rehabilitation in Cardiomyopathies: An Updated Review.

Cardiomyopathies, including hypertrophic (HCM), dilated (DCM), and arrhythmogenic (ACM) forms, represent a challenge in cardiovascular medicine, in particular regarding exercise participation and cardiac rehabilitation. Traditionally, physical activity was restricted in these patients due to concerns over arrhythmic risk and sudden cardiac death. However, current evidence suggests that individualized exercise programs, under clinical supervision, can enhance functional capacity, improve quality of life, and sometimes prognosis in selected patients. Contemporary European and North American guid
