[A case of parkinsonian syndrome caused by normal pressure hydrocephalus accompanied by the cauda equina neurinoma].

A case of parkinsonian syndrome caused by normal pressure hydrocephalus (NPH) accompanied by cauda equina neurinoma is reported. A 69-year-old woman presented with typical symptoms of parkinsonism, including akinesia, resting and postual tremor, and cog-wheel rigidity. CT scan of the brain revealed dilatation of ventricles, but she did not present dementia and urinary incontinence that are common symptoms in NPH. Her cerebrospinal fluid (CSF) pressure was normal, and her protein level was high at 2,970 mg/dl. An electroencephalogram (EEG) showed diffuse slow waves. An IMP-SPECT images of the b

Lumbar disc herniation and cauda equina syndrome. Considerations on a pathology with different clinical manifestations.

The authors describe six cases of cauda equina syndrome (CES) with different clinical manifestations, etiopathogenetic causes, and degrees of disc prolapse. In three of the cases, the clinical onset was dramatic and acute, in the others it was a hemisyndrome (with acute onset in only 1 case). The site of the disc prolapse was L4-L5 in 3 patients, L3-L4 in 2 patients, and L5-S1 in 1 patient. The authors emphasize the need for the early recognition of the syndrome with a careful clinical examination and history of the patient, as well as timely treatment within 24 hours. For the sake of preventi…

Increased tone of the rectal wall in response to feeding persists in patients with cauda equina syndrome.

The aim was to study fasting and postprandial rectal tone in patients with cauda equina injury. Electromechanical barostat measurement of rectal tone was made in 13 healthy volunteers and in five patients during a 10 min recording, while fasting and for 1 h after a 1000 kCal intake. A prompt decrease of rectal volume was observed in all control subjects and patients. The delay between the end of the meal and the onset of the rectal response was always less than 3 min in the five patients as well as in the control group. The rapidity of the rectal response to feeding observed in our five patien

[Cauda equina syndrome due to recurrent malignant lymphoma of the spinal cord. A case report].

A 50-year-old man noticed a mass in the right cervical region and presented to our hospital. He underwent biopsy of a cervical lymph node, w … alignant lymphoma to recur in the spinal cord, particularly the cauda equina. It is well known that cauda equina syndrome can be caused by vertebral lesions and primary spinal cord tumors, but it is also necessary to keep malignant lymphoma of the cauda equina in mind.

The role of calcium deposition in the ligamentum flavum causing a cauda equina syndrome and lumbar radiculopathy.

Our report concerns five patients who underwent lumbar decompressive surgery for cauda equina syndrome and radiculopathy secondary to degenerative stenosis associated with calcium deposition in the ligamentum flavum. The resected ligamentum flavum showed diffuse to massive calcium crystal deposition with a histologically marked degeneration in the elastic fibres. Energy dispensive radiographic microanalysis confirmed the crystal to be calcium pyrophosphate dihydrate in all patients, hydroxyapatite mixed in three and calcium orthophosphate mixed in one. The deposition appeared to be a change th…

[Ankylosing spondylarthritis and partial cauda equina syndrome. Apropos of 4 cases and review of the literature].

Four new cases of ankylosing spondylitis complicated by a cauda equina syndrome are reported. Similarly to the previous descriptions, the patients present with long-standing and severe spondylitis at the time when neurological symptoms, mainly radicular pain, develop. Computed tomography of the lumbar spine demonstrates in all cases typical laminar erosions and posterior arachnoid diverticula. Up to now, magnetic resonance imaging has rarely been performed in this particular pathology, though it is helpful in determining the anatomical relations and the nature of the lesions. Based on the prev…

Cauda equina syndrome of long-standing ankylosing spondylitis. Case report and review of the literature.

Cauda equina syndrome as a neurological complication of long-standing ankylosing spondylitis was first reported in 1961. The syndrome is relatively uncommon and its pathophysiology is still poorly understood. Based on their experience with such a case, the authors review the clinical, electrographic, histological, and radiographic features of the syndrome, including the findings of magnetic resonance (MR) imaging. The addition of MR imaging to the evaluation of patients with ankylosing spondylitis and the cauda equina syndrome not only aids in the diagnosis of the syndrome but may also provide…

[Paraplegia with a cauda equina syndrome due to stenosis of the spinal canal caused by tabes arthropathy].

The authors present a case of cauda equina paraparesis due to compression by tabetic osteo-arthropathy. Diagnosis was confirmed by the typical clinical picture of tabes and by the history of a chancre 53 years earlier. Radiography of the spine showed vertibral osteo-arthropathy. Opaque contrast myelography showed compression at the D 11/D 12 and L 4/L 5 levels, with poor contrast filling athe the L 1/L 3 level. Surgical treatment by decompressive laminectomies from D 12 to L 4 led to a satisfactory result with motor recovery and regression of the cauda equina syndrome.

Examination of The Predictive Power of Electromyography and Urodynamic Study in Patients with Cauda Equina Syndrome (Horse Tail Syndrome).

BACKGROUND: Cauda equina syndrome is a rare disorder that causes loss of Lumbar plexus function (nerve roots) lower than conus medullaris. No risk factor has been defined for this disease yet. Due to the high morbidity of Cauda equina syndrome and lack of sufficient information about the connection between the disease and urodynamic findings and EMG (Electromyography) findings, the need for this comprehensive study is felt. OBJECTIVE: The aim is to determine the predictive power of findings resulted from urodynamics and electromyography of perineal region and around sphincter in the clinical …

When Steroids Backfire: A Case of Hyperosmolar Hyperglycemic State, Pneumocystis Pneumonia, and Cauda Equina Syndrome.

Corticosteroids are potent anti-inflammatory and immunosuppressive agents frequently used in short courses for various conditions. However, prolonged or inappropriate use can lead to severe complications, including immunosuppression, metabolic derangements, and delayed diagnosis of underlying disease. Cauda equina syndrome is a neurosurgical emergency that can be masked by corticosteroid therapy, while chronic use also predisposes to opportunistic infections such as Pneumocystis jirovecii pneumonia. We present a case of corticosteroid-induced hyperosmolar hyperglycemic state, Pneumocystis jiro…

Cauda equina syndrome in an obese parturient following cesarean section under combined spinal-epidural anesthesia.

Cauda equine syndrome (CES) is the very rare complication especially after uneventful neuraxial anesthesia. A woman underwent a combined spinal-epidural anesthesia for cesarean section. Postoperatively 24 hours, she complained of paresthesia of the left leg, urinary retention, and saddle anesthesia. After checking magnetic resonance imaging (MRI), CES was suspicious and medical treatment was initiated. During the 18-month follow-up, though perineal hypoesthesia and paresthesia of the left leg were recovered, urinary and fecal incontinence remained. This case demonstrates that CES may occur eve

How do physiotherapists use Cauda Equina Syndrome safety netting techniques and what influences their practice?

BACKGROUND: Cauda Equina Syndrome (CES) is a rare but serious neurological condition requiring urgent intervention to prevent permanent harm. Although safety netting practices are recommended to mitigate risks, they are inconsistently applied, with limited guidance or evidence to inform their use. Existing research has largely overlooked the specific challenges of CES safety netting, particularly in advanced physiotherapy roles like First Contact Practitioners (FCPs) and Extended Scope Practitioners (ESPs), who play a pivotal role in managing suspected cases. OBJECTIVE: This study aimed to ad…

Rapid Diagnosis of Cauda Equina Syndrome: A Prospective Study Comparing Photon-counting CT With MRI.

PURPOSE: Early diagnosis of cauda equina syndrome is essential to prevent irreversible neurological damage, but MRI can be unavailable or contraindicated. This study aimed to evaluate whether photon-counting CT (PCCT) can be a reliable alternative to reference standard MRI for diagnosing cauda equina syndrome. METHODS: In this prospective study, participants with different conditions, including degenerative spinal canal stenosis, disk herniation, vertebral compression fracture, and intraspinal extradural tumor underwent PCCT and MRI between November 2022 and March 2024 at a university hospita…

Cauda equina syndrome-most frequent referral diagnosis to a tertiary spinal centre: a retrospective study.

BACKGROUND: Cauda equina syndrome (CES) is a rare but critical surgical emergency, with a global incidence of 1-3 cases per 100,000 annually. Despite its rarity, CES has paradoxically emerged as one of the most common referral diagnoses to tertiary spinal centres. This discrepancy between referral rates and the actual prevalence of CES raises concerns about potential over-referral and inefficiencies in clinical pathways. This study aimed to evaluate compliance with national guidelines from the British Association of Spine Surgeons (BASS) and Getting It Right First Time (GIRFT) concerning the s…

Atypical intradural extramedullary spinal schwannoma causing cauda equina syndrome: A case report and literature review.

INTRODUCTION: Spinal schwannomas are slow-growing benign tumors that are generally asymptomatic. However, we describe an atypical case in which an intradural extramedullary schwannoma presented as an acute cauda equina syndrome. PRESENTATION OF CASE: This was a 58-year-old woman with a 2-month history of severe low back pain and worsening neurological deficits and a 2-day period of acute onset of lower extremity numbness and urinary incontinence. Physical and neurological examination revealed significant lower extremity weakness, tenderness on palpation of the spine, positive straight leg tes…

Chronic cauda equina syndrome decompression surgery recovery is very "bad"? Based on patient self-assessment.

BACKGROUND: Symptoms of cauda equina syndrome (CES) secondary to degenerative lumbar spine diseases are sometimes mild and tend to be ignored by patients, resulting in delayed treatment. In addition, the long-term efficacy of surgery is unclear. OBJECTIVE: To determine the predictive factors of CES and post-operative recovery in patients with symptoms lasting > 3 months. METHODS: From January 2011 to December 2020, data of 45 patients with CES secondary to lumbar disk herniation/lumbar spinal stenosis were collected from a single center. The patients had bladder, bowel or sexual dysfunction …

Surgical Outcome of Cauda Equina Syndrome Secondary to Disc Herniation Presenting Late in Developing Countries.

Cauda equina syndrome (CES) is a serious neurologic state in which neurological dysfunction affects the lumbar and sacral nerve roots within the vertebral canal. The nerves of the cauda equine provide the muscles that manage the bladder, bowel and the legs and the dysfunction producing impairment of bladder, bowel or sexual function and perianal or saddle numbness. Identification of CES is not only important to neurosurgeons and spine surgeons, but also to prime care practitioners, emergency room physicians, physiotherapists and allied health care professionals concerned in management of back …
