Intracranial-Intracranial Bypass for Aneurysms: Quantitative Intraoperative Assessment of Flow Preservation.

BACKGROUND: Intraoperative flow measurement has proven utility in extracranial-intracranial bypass, particularly in assessing the adequacy of donors by measurement of cut flow. The nature of intracranial-intracranial (IC-IC) bypass precludes cut flow measurement, but quantitative intraoperative flow measurements may evaluate augment assessment of the bypass. OBJECTIVE: To retrospectively evaluate flow measurements performed in IC-IC bypass to determine the adequacy of the constructs in preserving flow. METHODS: With institutional review board approval, we performed a retrospective review of

[Resection of a giant fusiform aneurysm of the pericallosal artery with «hemi-bonnet bypass» procedure (case report and literature review)].

A 30-year-old woman with giant fusiform aneurysm of the azygos anterior cerebral artery is reported. Clipping of the aneurysm followed by modeling of pericallosal artery was performed in 2017. However, further enlargement of the aneurysm has been observed for subsequent 2 years. The patient underwent redo surgery with excision of the aneurysm followed by «hemi-bonnet bypass» procedure (anastomosis between superficial temporal artery and anterior cerebral artery with radial artery as an interposition graft). Literature data on reconstructive surgery in the treatment of complex pericallosal arte

Internal Maxillary to Middle Cerebral Artery Bypass Using an Anterior Tibial Artery Graft, Performed Using a 3-Dimensional Exoscope: 2-Dimensional Operative Video.

Fusiform aneurysms remain challenging entities to treat, as maintenance of flow and prevention of branch occlusion are concerns. Use of endovascular stents may risk nearby branch occlusion. We present a 31-yr-old male with a prior subarachnoid hemorrhage from a left middle cerebral artery (MCA) M1 aneurysm. Informed consent for use of the operative video and information was obtained from the patient. Initially, open clipping was performed with noted residual because of risk of nearby branches. On angiography, a growing fusiform segment distal to the initial aneurysm was noted. The patient was

Bilateral encephaloduroarteriosynangiosis (EDAS) in a 6-year-old female for moyamoya disease: case report and technical note with 12-year follow-up.

PURPOSE: To demonstrate that despite disease progression, indirect cerebral revascularisation via EDAS for moyamoya disease is able to maintain sufficient collateral blood supply over a 12-year period. METHODS: Case report of an individual patient with surgically managed moyamoya disease followed up with imaging and outpatient clinic assessments. RESULTS: Follow-up imaging and assessment over 12 years concluded with a good functional and radiographic outcome. CONCLUSION: The extensive follow-up in our case of a 6-year-old female treated with bilateral EDAS procedures for moyamoya disease de

The frequency of postoperative stroke in moyamoya disease following combined revascularization: a single-university series and systematic review.

OBJECT: Although combined direct and indirect anastomosis in patients with moyamoya disease immediately increases cerebral blood flow, the surgical procedure is more complex. Data pertinent to the postoperative complications associated with combined bypass are relatively scarce compared with those associated with indirect bypass. This study investigated the incidence and characteristics of postoperative stroke in combined bypass and compared them with those determined from a literature review to obtain data from a large population. METHODS: A total of 358 revascularization procedures in 236 p

Reconstruction of intracranial vertebral artery with radial artery and occipital artery grafts for fusiform intracranial vertebral aneurysm not amenable to endovascular treatment: technical note.

BACKGROUND: Symptomatic fusiform intracranial vertebral artery aneurysms pose a formidable treatment challenge when not amenable to endovascular treatment. In this paper, we illustrate the microsurgical management of such an aneurysm. METHODS: To prevent neurological deterioration, anatomical reconstruction preserving all vessels including posterior inferior cerebellar artery and perforators is essential. In this case illustration, the occipital artery was used as a donor to a perforator originating from the aneurysmal segment. This bypass was performed in an end-to-side fashion. Subsequently

Cranial burr hole for revascularization in moyamoya disease.

Currently, superficial temporal artery-middle cerebral artery (MCA) anastomosis, encephalomyosynangiosis (EMS), and encephalo-duro-arterio-synangiosis are used to treat moyamoya disease and are reported to effectively improve ischemic symptoms. All are methods of reversing the flow of blood from the external carotid artery system into the cortical branches of the MCA. As moyamoya disease advances, these operations alone will predictably not correct the deterioration in blood flow in the territory of the anterior cerebral artery. It was noted in a case of moyamoya disease with intraventricular

Outcomes of Medical Management Alone for Adult Patients with Cerebral Misery Perfusion Due to Ischemic Moyamoya Disease.

OBJECTIVES: Although revascularization surgery is recommended for adult patients with moyamoya disease (MMD) who present with ischemic symptoms due to hemodynamic compromise, the clinical course of such patients who are treated with medical management alone remains unclear. Here, we report outcomes of adult patients with cerebral misery perfusion due to ischemic MMD who received medical management alone. MATERIALS AND METHODS: We prospectively followed up patients who showed misery perfusion in the symptomatic cerebral hemisphere on 15O gas positron emission tomography (PET) and received stri

Spontaneous Resolution of Dural and Pial Arteriovenous Fistulae Arising After Superficial Temporal Artery to Middle Cerebral Artery Bypass for Moyamoya Disease.

BACKGROUND: Superficial temporal artery (STA)-to-middle cerebral artery bypass is frequently performed for moyamoya disease. We discuss an unusual case in a moyamoya patient complicated by the development of dural and pial arteriovenous fistulae (AVF). Both AVF then spontaneously resolved 2 years after surgery. CASE DESCRIPTION: A patient in the fifth decade of life presented after multiple strokes resulting in right-sided weakness and numbness. Magnetic resonance imaging revealed remote strokes, and angiography revealed Suzuki grade 3 moyamoya angiopathy bilaterally. With a diminutive left S

Complex aneurysm formation in the proximal segment of the posterior cerebral artery: a report of two cases.

Posterior cerebral artery (PCA) aneurysms are rare and often fusiform. We describe two cases of complex proximal PCA aneurysm in two women in their 60's, which probably resulted from segmental arterial degeneration. Both presented with subarachnoid hemorrhage and had common angiographic and intraoperative findings: tortuous configuration of the affected P1 segment, whitish or yellowish appearance of a portion of the lesion, lesion calcification, and multiple aneurysms in the segment. Interestingly, no significant atherosclerotic changes were noted in other cerebral arteries. The ruptured aneur

Superficial Temporal Artery-Posterior Cerebral Artery Bypass Through Zygomatic Anterior Temporal Approach for Complex Posterior Cerebral Artery Aneurysm.

BACKGROUND: Posterior cerebral artery (PCA) aneurysms are rare, and most are giant, dissecting, or fusiform in morphology. Proximal occlusion of the PCA without revascularization causes high risk of ischemic complications. This study aimed to evaluate the safety and validity of using superficial temporal artery (STA)-PCA bypass through zygomatic anterior temporal approach in complex PCA aneurysms. METHODS: Trapping or resecting of aneurysms and reconstruction of distal PCA through a zygomatic anterior temporal approach were performed in 6 patients from June 2017 to August 2020. Postoperative

Superficial temporal artery to anterior cerebral artery direct anastomosis in patients with moyamoya disease.

Some patients with Moyamoya disease require revascularization not only to the territory of the middle cerebral artery (MCA) but also to that of the anterior cerebral artery (ACA). To revascularize the ACA territory, we performed superficial temporal artery (STA)-ACA direct anastomosis in five patients with Moyamoya disease. After paramedian frontal craniotomy (approximately 5 cm diameter), STA-ACA anastomosis was carried out at the convexity using a cortical branch of the ACA as a recipient. An interposed STA graft was used in four patients: all of the grafts were shorter than 4 cm. Bypass flo

Indications for surgical intervention in middle cerebral artery obstruction.

The clinical, angiographic, and surgical characteristics of 31 patients with high-grade middle cerebral artery (MCA) stenosis or occlusion (jointly termed "MCA obstructions") referred for cerebral revascularization by extracranial-intracranial (EC-IC) bypass are reviewed. Overall, 12 (66%) of the 18 patients with stenosis and all 13 (100%) of those with complete occlusion experienced clinically evident infarctions. Twenty-five of these patients underwent arteriography at least twice during their clinical course. Eleven (44%) demonstrated significant improvement in flow or complete resolution o…

Treatment of hemodynamic insufficiency in chronic CCA occlusion using a short saphenous vein interposition graft: diagnostic and technical considerations. An illustrative case report.

Extracranial-intracranial bypass surgery has been shown to reverse hemodynamic insufficiency on the basis of steno-occlusive disease of the internal carotid artery (ICA) or middle cerebral artery. In contrast, chronic occlusion of the common carotid artery (CCA) without extracranial donor vessels affords alternative revascularization procedures as well as a more elaborate preoperative workup. This case is intended to illustrate the specific diagnostic approach and considerations as well as a beneficial treatment modality in the setting of pronounced hemodynamic insufficiency as a consequence o

Moyamoya disease associated with Graves disease: special considerations regarding clinical significance and management.

OBJECT: In moyamoya disease (MMD), ischemic events are usually precipitated by activities associated with hyperventilation or physical strain. The authors report on four patients with a rare combination of Graves disease-associated thyrotoxicosis and MMD, whose cerebrovascular ischemic events occurred while in a thyrotoxic state. The clinical correlation between MMD and Graves' thyrotoxicosis, and outcome after surgical intervention are described. METHODS: Four young women, ages 22 to 25 years, presented with profound cerebrovascular ischemic accidents. They had clinical and radiological feat

Thalamic anastomosis as a potential marker of perioperative ischemic risk in moyamoya disease.

Revascularization surgery is a well-established intervention for both ischemic and hemorrhagic moyamoya disease (MMD). Perioperative ischemic stroke occurs in 5-18% of surgeries, however, evidence-based information for its prevention and related factors remain insufficiently established. The development of collateralization pathways in MMD may play a significant role in optimizing cerebral perfusion as much as possible, therefore, during cerebral revascularization surgery, the tolerance to ischemia may vary depending on the developmental pattern of the preoperative collateral circulation. The …

Chronic low-perfusion state in children with moyamoya disease following revascularization.

The distribution pattern of cerebral blood flow (CBF) was evaluated in 13 children with moyamoya disease after revascularization. Combined surgery involving encephaloduroarteriosynangiosis and encephalomyosynangiosis was performed in 21 hemispheres. CBF was assessed by dynamic imaging using N-isopropylp[123I]iodoamphetamine-single photon emission CT (IMP-SPECT) at 46 months on average after revascularization. None of the patients suffered recurrent ischemic attacks after the bilateral combined surgery, and 62% showed excellent active daily life. However, 70% showed poor school performance with
