The Halofixator for the Treatment of Unstable Fractures of the Upper Cervical Spine - Our Own Experience and Narrative Review.

INTRODUCTION: In the treatment of upper cervical spine injuries, the semiconservative procedure of the halofixator is now of only secondary importance. Older studies from the 2000 s showed unsatisfactory rates of consolidation as well as high rates of complications. However, due to current data on therapy effectiveness, the literature is inconsistent. The aim of this work is to compare our own experiences and results of treatment with the current literature and to help to clarify the role of the halofixator. MATERIAL AND METHODS: In a monocentric retrospective cohort study, patients with unst

Neck injuries among the elderly in Sweden.

Neck injuries are some of the most important injuries as they have the potential to influence the spinal cord. A previous national survey of neck injuries in Sweden revealed that injury incidence was increasing for the population over 65 years of age, although it was decreasing for the population as a whole. The aim of this study was therefore to further clarify the magnitude, severity, and external causes of neck injuries in the elderly people in Sweden. A national incidence study, with focus on the age group above 65 years, was undertaken with data from the injury surveillance program at the

Posterior atlanto-axial dislocation in rheumatoid arthritis.

Seven patients with rheumatoid arthritis and posterior atlanto-axial dislocation are presented. The patients were elderly women who had suffered with the disease a long time. In all cases earlier cervical radiographs were available, excluding any predisposing congenital anomaly. Fracture and arthritic destruction of the dens, of parts of the body of the axis, or of the anterior arch of the atlas proved to be predisposing factors of the lesion.

Complex posterior atlantoaxial instability in pediatric os odontoideum: A novel stabilization strategy.

INTRODUCTION: Os odontoideum (OO) is a rare anatomic variant of the axis characterized by the separation of a part of the axis. It may cause cervical instability, atlantoaxial dislocation, myelopathy, or permanent paralysis. We present an extremely rare case of an OO with posterior atlantoaxial dislocation in a child. CASE REPORT: A nine-year-old female with OO, cervical instability, and posterior atlantoaxial dislocation had intermittent neck pain for several years. The patient was surgically treated by performing a C2-C3 laminectomy with C1-C5 posterior fixation for stabilization. CONCLUSI

Acute-Phase Intraoperative Findings of Traumatic Spinal Cerebrospinal Fluid Leakage.

The acute-stage intraoperative findings of the spinal dural laceration in patients with traumatic cerebrospinal fluid (CSF) leakage have not been clearly revealed so far. Herein, we report an interesting case of traumatic CSF leakage in a patient who underwent acute-stage direct closure for a dural laceration similar to a stab at the C1/2 level. Because a 41-year-old man strongly twisted his neck, he had a typical orthostatic headache. Whole-spine T2 fat-saturated magnetic resonance imaging and computed tomographic myelography were examined, and fluid collection between the intrathecal site an

Retro-odontoid Degenerative Pseudotumour Causing Spinal Cord Compression and Myelopathy: Current Evidence on the Role of Posterior C1-C2 Fixation in Treatment.

BACKGROUND: A retro-odontoid pseudotumour compressing the spinal cord and causing myelopathy is often associated with an inflammatory condition such as rheumatoid arthritis. A degenerative non-inflammatory retro-odontoid pseudotumour responsible for clinically relevant spinal cord compression is a rare condition described in small clinical series and is likely associated with craniovertebral junction hypermobility or instability-like conditions. For several years, direct removal of the lesion through an anterior or lateral approach has been advocated as the best surgical option. However, in th

The effect of C1 bursting fracture on comparative anatomical relationship between the internal carotid artery and the atlas.

PURPOSE: To describe the effect of the C1 bursting fracture on the location of the internal carotid artery (ICA) around the atlas. METHODS: The authors analyzed the morphology of the atlas and the ICA in 15 patients with C1 bursting fracture and compared with control group (77 patients) without any pathology. All patients were evaluated with CT angiography for the anatomical assessment. The laterality of the ICA, the distances of the ICA from the midline, anterior tubercle, and ventral surface of the C1 lateral mass were compared between two groups. The distance between the lateral margin of

Transoral approach to extradural non-neoplastic lesions of the craniovertebral junction.

BACKGROUND: The transoral approach allows for an unobstructed anterior view of the craniovertebral junction from the lower clivus to C1 and C2. It can be applied to a heterogeneous spectrum of pathological lesions involving this area including craniovertebral junction malformations, atlanto-axial synovial cysts, pseudoarthrosis following odontoid fractures, selected cases of retro-odontoid pannus, and vertical translocation in rheumatoid patients. METHODS: Microsurgical strategy is dictated by the nature and site of the target lesion. Atlas preservation during transoral approach (atlas-sparin

Biomechanics of sports-induced axial-compression injuries of the neck.

CONTEXT: Head-first sports-induced impacts cause cervical fractures and dislocations and spinal cord lesions. In previous biomechanical studies, researchers have vertically dropped human cadavers, head-neck specimens, or surrogate models in inverted postures. OBJECTIVE: To develop a cadaveric neck model to simulate horizontally aligned, head-first impacts with a straightened neck and to use the model to investigate biomechanical responses and failure mechanisms. DESIGN: Descriptive laboratory study. SETTING: Biomechanics research laboratory. PATIENTS OR OTHER PARTICIPANTS: Five human cadav

Osteoid osteoma of the first 2 cervical vertebrae. Report of 4 cases.

Osteoid osteoma affects the spine in only 10% of cases. More than 50% of the spinal cases involve the lumbar and cervical vertebrae. Involvement of C-1 and C-2 vertebrae has previously been reported only very rarely in the published literature. The authors report 4 cases of upper cervical osteoid osteoma, 1 involving C-1 and the other 3 C-2, and they discuss different aspects of management in similar cases. The patients were 14, 17, 35, and 46 years old, and all presented with neck pain and various degrees of painful limitation of head rotation not ameliorated by ordinary analgesics. Radionucl

Anatomic considerations for the pedicle screw placement in the first cervical vertebra.

STUDY DESIGN: Anatomic bony measurements were manually performed on 50 dry atlantoaxial vertebral complexes with an electronic digital caliper, and a reliable landmark for insertion of a pedicle screw in C1 vertebra was described and evaluated. OBJECTIVES: To investigate the feasibility of placing a screw in the C1 pedicle and evaluate the reliability of the C2 lateral mass as a landmark for determining the optimal site of screw entry. SUMMARY OF BACKGROUND DATA: The use of C1-C2 pedicle screws with the additional rods or plates has been an alternative method of stabilization for patients wh

Cerebellar infarction secondary to os odontoideum.

A 43-year-old man developed cerebellar infarction due to atlantoaxial dislocation associated with os odontoideum. Cervical X-ray films confirmed os odontoideum, and conventional angiography revealed irregular narrowing of the right vertebral artery at the axis level. A correlation between the vertebral artery lesion and atlantoaxial instability was shown with 3-dimensional (3-D) computed tomographic (CT) angiography. This is the first case of atlantoaxial dislocation with vertebral artery stenosis demonstrated 3-D CT angiography. 3-D CT angiography can support the conventional angiography with

Multiple unstable cervical fractures with cord compromise treated nonoperatively: a case report.

STUDY DESIGN: The case of a 17-year-old boy who sustained an unusual pattern of multiple unstable cervical spine fractures with a spinal cord injury is presented. OBJECTIVE: To review the management and treatment of multiple unstable cervical spine fractures. SUMMARY OF BACKGROUND DATA.: Combined unstable cervical spine injuries are uncommon. Treatment of multiple-level cervical spine fractures remains controversial. METHODS: The case of a 17-year-old boy who sustained a ring fracture of the atlas, an atypical traumatic spondylolisthesis of the axis, bilateral pedicle fractures of C3 with si

Craniocervical junction as a focus for craniofacial growth studies.

The craniocervical junction is a highly specialized unit simultaneously supporting head during movements in all planes and protecting the spinal cord. Anatomically, it includes an atlantoaxial complex, part of which embryonically arises from the occipital region of the skull. This review deals with the gross anatomy, kinematics, and growth reactions associated with functional alteration in this complex. Particular attention is paid to the atlas, the connecting element between the head and the vertebral column proper. From several studies it is concluded that the horizontal growth of the atlas

Atlantoaxial dislocation as the presenting symptom of ankylosing spondylitis.

A 27-year-old woman presented with nontraumatic atlantoaxial dislocation that required a C1-C2 fusion. A comprehensive investigation initially failed to reveal any evidence of infectious or rheumatologic disease and she remained well for 3 1/2 years, at which time a diagnosis of ankylosing spondylitis was established. There are no other similar cases reported in the medical literature. When dealing with a patient who has experienced a spontaneous atlantoaxial dislocation, a thorough assessment looking for an underlying infectious or inflammatory etiology is mandatory. Furthermore, the authors
