BACKGROUND: Population aging leads to rising chronic disease burden and healthcare costs. The metropolitan area of Genoa (Liguria), the oldest in Italy, is particularly affected. Since 2017, ASL3 has implemented a comprehensive health promotion strategy including risk-factor reduction initiatives and multidisciplinary care pathways for multimorbid patients.
AIMS: To evaluate the impact of the comprehensive health promotion program on the prevalence of heart failure (HF), chronic kidney disease (CKD), and chronic liver disease (CLD), and on healthcare resource use.
METHODS: Using the ASL3 dat

BACKGROUND: Advances in antiretroviral therapy have transformed HIV into a chronic condition, leading to a growing population of adults aged 50 years and older living with HIV in Canada and globally. These individuals experience higher rates of multimorbidity, frailty, cognitive changes, and polypharmacy than their HIV-negative peers, and many rely on caregivers for emotional and practical support. Caregiving often occurs within chosen families of partners, friends, and community members, yet these caregivers remain largely unrecognized in policies that prioritize bio-legal family structures.

Health literacy is increasingly understood to be a mediator of chronic disease self-management and health care utilization. However, there has been very little research examining health literacy among incarcerated persons. This study aimed to describe the health literacy and relevant patient characteristics in a recently incarcerated primary care patient population in 12 communities in 6 states and Puerto Rico. Baseline data were collected from 751 individuals through the national Transitions Clinic Network (TCN), a model which utilizes a community health worker (CHW) with a previous history o

Cardiovascular-kidney-metabolic (CKM) syndrome represents the complex interplay between cardiovascular disease (CVD), chronic kidney disease (CKD), and metabolic disorders such as type 2 diabetes and obesity. These conditions share overlapping risk factors, pathophysiological mechanisms, and clinical outcomes, leading to progressive organ dysfunction, as well as increased morbidity and mortality. Traditional disease-specific care models, in which cardiology, nephrology, and endocrinology operate in separate clinics, are increasingly inadequate for addressing the integrated nature of CKM syndro

Hidradenitis suppurativa (HS) is a chronic, painful inflammatory skin disease affecting 0.1% of the US population. Limited understanding of HS biology and ineffective treatments leave patients dissatisfied, facing misdiagnosis, and diagnostic delays. To address these challenges, the Rockefeller University Center for Clinical Translational Science, Clinical Directors Network, and the HS Foundation launched an initiative to foster engagement among stakeholders. Three full spectrum town halls (FSTH) were designed to engage patients, scientists, and clinicians bidirectionally. Topics spanned the t

New treatments have transformed multiple myeloma into a chronic disease. Hence, optimal management of treatment and disease-related complications remains a critical component of survivorship care. Survivorship care model in cancers requiring a fixed-duration therapy may not be applicable to myeloma, since patients are exposed to multiple lines of continuous therapy along the disease trajectory. The two most common therapy-related causes of death, which require special consideration, are infection and second cancers. Identifying patients at a high risk of toxicities will facilitate individualiz

Population ageing and rising chronic diseases have coincided with rapid growth in public health spending in China, but health returns may depend on spending composition. This study evaluates associations of spending scale and the demand-side share of government health expenditure with older adults' chronic disease burden. Data come from six waves (2002-2018) of the Chinese Longitudinal Healthy Longevity Survey, pooled as repeated cross-sections of adults aged 65 years and older (79,103 person-wave observations) and linked to one-year-lagged national spending indicators. Outcomes included chron

Substance Use Disorder (SUD) is widely recognized as a chronic, relapsing medical condition by major authorities, including the World Health Organization (WHO) and the Centers for Disease Control and Prevention (CDC), supported by clinical and neuroscientific evidence. Despite this recognition, health systems often respond through episodic, crisis-driven, or punitive models of care. Like other chronic diseases, SUD unfolds through stability, relapse, and recovery, requiring sustained, coordinated, person-centred care. Formalizing SUD as a chronic disease may reduce stigma, attract resources, n

Chronic pancreatitis (CP) is a fibro-inflammatory condition defined by permanent anatomical changes in the pancreas. The causes of CP are described by the TIGAR-O classification system: toxin-related, idiopathic, genetic mutations, autoimmune disorders, episodes of recurrent acute pancreatitis, and obstructions. Pain is multifactorial in nature, and common psychopathological consequences of CP, including depression and anxiety, complicate the clinical picture of chronic pancreatitis. As a result, the quality of life of patients with CP is decreased. This review describes the pathophysiology of

BACKGROUND: Access to effective nonpharmacologic treatments for chronic pain, including integrative therapy approaches, is a priority for the US Department of Veterans Affairs (VA) for improving pain care and promoting patient-centered care as part of the VA Whole Health System care delivery model. This study sought to assess primary care practitioner (PCP) and patient knowledge, interest, and use of integrative therapies to identify strategies to promote broader utilization.
METHODS: PCPs affiliated with the VA Ann Arbor Healthcare System and a random sample of patients with low back pain di

Bacterial wound infections contribute significantly to global mortality, with nearly half of contaminated wounds progressing to chronic states, which can result in amputations. In Tanzania, the situation is worsened by the misuse of antibiotics and the rise of drug-resistant bacteria, alongside patients' varied understanding of wound care management. This qualitative study explores patient knowledge, attitudes, and practices (KAP) regarding chronic wound infections in Tanzania. A sample of 15 patients was selected from both genders, representing a wide range of ages and wound severities. Semi-

BACKGROUND: The fate of patients with chronic limb-threatening ischaemia undergoing revascularization or a primary amputation is unclear. The aim of this study was to assess the postoperative outcomes and post-procedural healthcare resource use/costs over 1 year after revascularization or a primary amputation for chronic limb-threatening ischaemia.
METHODS: The UK Kent Integrated Dataset, which links primary, community, and secondary care for 1.6 million people, was interrogated. All patients with a new diagnosis of chronic limb-threatening ischaemia undergoing revascularization or a major am

IMPORTANCE: Failure to account for the full complexity and costs of high-need populations in the risk-adjusted capitated payment model for Medicare Advantage (MA) plans may create financial disincentives for plans to invest in comprehensive care for affected beneficiaries, potentially exacerbating health disparities.
OBJECTIVE: To evaluate the association of reinstatement of Alzheimer disease and related dementias (ADRD) hierarchical condition categories (HCCs) into the MA risk-adjusted payment model in 2020 with access, affordability, and quality of care for beneficiaries with ADRD.

Although health beliefs have been correlated with self-care adherence in other chronic conditions, little is known about skin care beliefs after spinal cord injury (SCI). The purpose of this qualitative study was to identify the skin care beliefs of individuals with SCI. The conceptual framework was the Health Belief Model (HBM), which proposes that adherence to a health regimen is motivated by beliefs about susceptibility, severity, barriers, benefits, and self-efficacy. Purposive sampling was used to recruit 22 people with SCI. Content analysis of data collected using semistructured question

BACKGROUND: Patients with chronic lung diseases (CLDs), defined as progressive and life-limiting respiratory conditions, experience a heavy symptom burden as the conditions become more advanced, but palliative referral rates are low and late. Prognostic tools can help clinicians identify CLD patients at high risk of deterioration for needs assessments and referral to palliative care. As current prognostic tools may not generalize well across all CLD conditions, we aim to develop and validate a general model to predict one-year mortality in patients presenting with any CLD.

The US healthcare system is failing our patients, our clinicians, and our communities, while costs are rapidly increasing. There is interest among practitioners, patients, and Centers for Medicaid and Medicare Services (CMS) in redesigning the healthcare system to one that focuses on prevention and reduction of chronic disease, while improving health and well-being. Whole-person care, a person-centered, relationship-based, team-based model that addresses the social, emotional, physical, spiritual, and behavioral factors that drive health and healing, would accomplish this goal. This case report presents the treatment process and recovery of a man in his 30s who presented with post-traumatic stress disorder (PTSD) symptoms as a result of and exacerbated by the ongoing violence following a period of civil unrest. The patient presented with extreme hypervigilance, intrusive thoughts, sleep disturbance and social withdrawal. Within a resource-poor community mental health context, the patient was provided with a treatment model that included psychoeducation, culturally sensitised cognitive-behavioural therapy, a selective serotonin reuptake inhibitor medication and commun

Exacerbations punctuate the natural course of chronic obstructive pulmonary disease (COPD), posing a substantial burden on patients and health-care systems. Understanding the severity of an acute exacerbation of COPD (AECOPD) is crucial for stratifying mortality risk and guiding treatment decisions. Currently, the severity of an AECOPD is primarily graded on outcomes of health-care use. Given the complex heterogeneity in pathogenesis, clinical presentation, post-event trajectory, and patient perspectives, this approach does not adequately address the causes and extent of clinical deterioration
