Systemic vasculitides are a heterogeneous group of diseases. Having only a partial understanding of the aetiologies and pathogenetic mechanisms of these disorders explains the difficulties encountered in classifying and treating patients. Nevertheless, some important points have been established. Classification is mainly based on the size of vessels affected and, from the polyarteritis nodosa group, microscopic polyangiitis (MPA) has been separated from classic polyarteritis nodosa (c-PAN). The latter is a rare disease which is, in a small number of cases, the consequence of hepatitis B or C v…

The presence of cardiac arrhythmias may at times suggest a specific underlying heart disease or even noncardiac disease. The most common and important cause of cardiac arrhythmias is coronary artery disease. In the past three decades, according to decrease in the incidence of rheumatic heart disease, the cause of atrial fibrillation is also going to change. Various classification of arrhythmias are available, however, a simple one is related anatomical portion of the heart in which they arise. Recently, the Sicilian gambit has introduced a classification which is based on there electrophysiolo…

OBJECTIVES: We sought to develop and apply a new scheme for the classification of death to be used in trials of antiarrhythmia treatments.
BACKGROUND: Because presently accepted classifications of death do not fully describe or tabulate all significant aspects of terminal events, nor do they consider unique aspects of arrhythmia investigations, a new classification scheme that addresses these issues is desirable.
METHODS: A classification scheme of deaths that occur in antiarrhythmia trials was developed using the following categories: 1) primary organ cause (cardiac [arrhythmic, nonarrhythm…

A classification of deformations of the nose is proposed. All deformations are classified into 5 degrees: I) deformations involving one compartment of the nose, II) two compartments, III) three compartments, IV and V) combined deformations involving more than 4 compartments. This classification helps the physician to be better oriented in the great variety of combined deformations of the nose and permits a more accurate planning of the steps of surgery. This classification is not a final or exhausting, it is merely an attempt to present a scheme of the great variety of combined deformations of…

The International Classification of Childhood Cancer (ICCC) updates the widely used Birch and Marsden classification scheme. ICCC is based on the second edition of the International Classification of Diseases for Oncology (ICD-O-2). The purpose of the new classification is to accommodate important changes in recognition of different types of neoplasms, while preserving continuity with the original classification. The grouping of neoplasms into 12 main diagnostic groups is maintained. The major changes are: (1) intracranial and intraspinal germ-cell tumours now constitute a separate subgroup wi…

Classification of dementia involves the recognition of its presence, followed by the differential diagnosis of its cause. Informant-based methods for dementia detection can be highly sensitive, even when cognitive impairment is mild. Alzheimer's disease (AD) is by far the leading cause of dementia; standardized clinical criteria result in high diagnostic accuracy rates. Classification of other dementing disorders is less satisfactory, particularly because there is frequent clinical and pathological overlap with AD. After AD, the most common causes of dementia are vascular dementia and dementia…

Hyperparathyroidism is caused mainly by three different conditions: namely, secondary hyperplasia, primary hyperplasia, and adenoma with only a few cases due to carcinomas. Histological distinction among these diseases is still difficult. In an attempt to characterize the three conditions, 17 cases from patients with hyperparathyroidism and 12 with normal glands were investigated immunohistochemically using antibodies against PTH, PTHrP, Ki-67 (MIB-1), and chromogranin A. The normal glands showed a diffuse staining pattern for PTH, and focal staining for PTHrP and for chromogranin A. Secondary

The present national classification of pneumoconioses was adopted 15 years ago. During this time span clinical morphology, a new branch of medical science, was formed and developed. So, the new approaches inspired classic pathologic anatomy and experimental pulmonology to assemble more precise and somehow refined knowledge of pneumoconioses, including the classification. Pneumoconiosis as diffuse pneumonitis can develop 2 main types: interstitial and granulomatous. Both types are characterized by progressive stages of morphogenesis, which are different in various loci of lungs. Those stages ar…

The two main classifications of mood disorders currently used are the American system, DSM, the fourth edition of which came out in 1994, and the International Classification of Diseases, the tenth edition of which was published in 1993. These classifications are based on the following broad principles: simple description with no hypothetical aetio-pathogenic discussion, distinction between the various clinical forms in accordance with variety and intensity of symptoms, co-existence of other more or less serious somatic disorders, individual identification of bipolar forms, etc. The key change…

PURPOSE: Retrospective analysis in cooperative study of hydrocephalus at institutions of members of the Research Committee on Intractable Hydrocephalus sponsored by the Ministry of Health and Welfare of Japan was performed to determine the functional prognosis for all types of hydrocephalus and thus to clarify the outcome.
METHODS: In preparation of this study, we have proposed the definition, clinical classification and diagnostic criteria of hydrocephalus. We have classified non-tumoral hydrocephalus into eight types based on its etiology and the time of onset. To establish the diagnosis in…

OBJECTIVE: The establishment of classification criteria for polymyositis (PM) and dermatomyositis (DM).
METHODS: Questionnaires inquiring about patients with DM, PM, systemic lupus erythematosus, progressive systemic sclerosis and noninflammatory neuromuscular diseases were distributed to the main medical institutes in Japan. Data were collected and analyzed by computer.
RESULTS: Among skin lesions of DM, heliotrope rash, Gottron's sign and erythema or purpura on the extensor surfaces of the extremity joints were shown to be distinguishing criteria. In both DM and PM, proximal muscle weaknes…

The classification and treatment of Menière's disease are especially difficult because the natural history of Menière's disease is so variable, and not possible to relate to the cochlear and vestibular pathology present, which is constantly changing and cannot be known during life. In the many new papers written each year on Menière's disease and the hundreds of older papers already in the medical literature, there is no good agreement about the definition of Menière's disease, the pathology and natural history, and the real benefit of the medical and surgical treatments given. Only recently, …

Infarction remains the cause of stroke in 75 to 80% of patients. The leading cause is embolism from one of several sources, although the source of the particles is often not discovered despite prolonged search. Brain infarction associated with high-grade stenosis or occlusion of major arteries may also be caused by local embolism but in one-third of patients, the process of infarction seems attributable to perfusion failure, with a more leisurely evolution of the syndrome than that typical of embolism. Lacunar disease accounts for roughly 20% of strokes.

The conventional distinction between schizophrenia and manic depression has received little objective support from recent studies of phenomenology, outcome, or familial homotypy. Instead, much clinical, epidemiological, and morphological evidence suggests that within the broad range of Schneiderian schizophrenia there exists one form (congenital schizophrenia) that can be distinguished from other types, the manifestations of which are confined to adult life. We hypothesize that congenital schizophrenia is a consequence of aberrant brain development during fetal and neonatal life. Such patients

A classification system based on pathologic anatomy distinguishing between three types of lumbar stenosis is reported: stenosis of the spinal canal, isolated stenosis of the nerve root canal, and stenosis of the intervertebral foramen. Each type of stenosis is classified as primary, secondary, combined or mixed. Pathomorphological pictures which may represent the vertebral bodies, the neural arch and the discoligamentous structures in the various forms of stenosis, among which degenerative spondylolisthesis, are examined. Finally, the authors describe the pathologic modifications occurring in …

New, published in 1989-90, international classifications of gastritis take into consideration its topography, the degree of morphological changes and etiology. This approximates the morphological diagnosis of gastritis to the nosological one. The most frequent cause of the gastritis is pyloric Helicobacter. Their presence should be indicated in the morphological diagnosis as this predetermines the therapeutic policy.

Neurofibromatosis (NF) represents not an entity but a group of several forms which differ as to symptoms, prognosis and inheritance. In 1982 Riccardi suggested a classification into eight categories. Two of these--Von Recklinghausen NF-1 and bilateral acoustic NF-2- were defined in 1987 by a National Institutes of Health Consensus Development Conference. We describe 13 patients whose symptoms do not fit the diagnostic criteria for NF-1 and NF-2. 6 subjects can be classified as segmental NF-V. One of these patients was remarkable in having iris hamartomata, while his daughter was affected with …
