Inside the clinical evaluation of sleepiness: subjective and objective tools.

PURPOSE: To critically review the available tools for evaluating excessive daytime sleepiness (EDS) in clinical practice. METHODS: Objective tests and subjective scales were divided into three groups in accordance with the different dimensions of sleepiness they measure, namely physiological, manifest, and introspective. Strengths, weaknesses, and limitations of each test have been analysed and discussed along with the available recommendations for their use in clinical practice. RESULTS: The majority of the tests developed for sleepiness evaluation do not have practical usefulness outside t

Intelligence without intuition: a mixed-methods pilot study on reasoning models in musculoskeletal physiotherapy for low-back pain.

Musculoskeletal pain, especially low-back pain, is highly prevalent and often challenging to manage due to its multifactorial nature. Effective diagnosis and therapy require clinicians to integrate biopsychosocial information within an evidence-based clinical reasoning framework. Large language models that "think" before responding, so-called reasoning models, show promise to support such complex decision-making, yet their validity and reliability in this setting remain unclear. In our work, we present a comprehensive human evaluation of reasoning models for clinical reasoning. Our results ind…

A novel approach to clinical thinking training for medical students: the combined World Café discussion and case-based learning experience introduction.

It is essential for modern medical students to continuously enhance their clinical thinking abilities. This study aims to evaluate the efficacy of the combined World Café discussion and case-based learning (CBL) approach within the clinical thinking training course. The clinical thinking training course incorporated the combined World Café discussion and CBL approach. The assessment of the accuracy and rationality of clinical symptoms, medical examination, pathological processes, diagnostic results, diagnostic basis, and drug use was conducted through case-related queries. Feedback from studen

The need for clinical judgement in the application of evidence-based medicine.

BACKGROUND: Evidence-based medicine (EBM) has no doubt resulted in great improvements in the practice of medicine. However, there are problems with overly zealous application of EBM, that for some amounts to religious practice. When good evidence exists, it should guide therapeutic and diagnostic choices. However, when evidence is lacking for a given patient, medicine is best practised by extrapolation from available evidence, interpreted in the light of the pathophysiology of the condition under consideration, and effects of various therapies in relation to that pathophysiology. OBJECTIVE: T

Comparing the utility of clinical risk scores and integrated clinical judgement in patients with suspected acute coronary syndrome.

AIMS: The utility of clinical risk scores regarding the prediction of major adverse cardiac events (MACE) is uncertain. We aimed to directly compare the prognostic performance of five established clinical risk scores as well as an unstructured integrated clinical judgement (ICJ) of the treating emergency department (ED) physician. METHODS AND RESULTS: Thirty-day MACE including all-cause death, life-threatening arrhythmia, cardiogenic shock, acute myocardial infarction (including the index event), and unstable angina requiring urgent coronary revascularization were centrally adjudicated by two

Clinical Evaluation Before MRI Referral: Frequency and Association with Diagnostic Yield.

PURPOSE: To evaluate how often history taking and physical examination are omitted before MRI referral and whether their omission is associated with clinical reasoning quality and MRI diagnostic yield. MATERIALS AND METHODS: In this prospective study, adults undergoing MRI at a tertiary academic hospital were surveyed before imaging to determine whether the referring clinician had taken their history and performed a physical examination. Multivariable regression was used to assess determinants of omission and associations with clinical reasoning quality (defined as agreement between the suspe…

Small Bowel Obstruction in the Virgin Abdomen.

This article highlights the quagmire of the surgeon who encounters a small bowel obstruction in the absence of previous abdominopelvic surgery. Historic literature implies urgent surgical intervention is required; however, there is no current standard of care to guide management. Key principles of general surgery apply, and definitive management is based on the provider's clinical judgment after synthesizing key clinical history and additional diagnostic imaging studies.

AI-powered simulated patients with automated feedback for enhancing headache history-taking skills: a convergent mixed-methods study.

BACKGROUND: Medical students often struggle with headache disorders, which are common but diagnostically challenging due to limited clinical exposure and feedback during early clinical training. This study evaluated the feasibility and effectiveness of an AI-powered simulated patient (AISP) chatbot in enhancing history-taking and clinical reasoning for three primary headache disorders-migraine, tension-type, and cluster headache-among medical students at a tertiary university hospital in Thailand. METHODS: We developed a simulated patient platform that provided automated, rubric-based feedbac…

Inductive and deductive reasoning in obsessive-compulsive disorder.

BACKGROUND AND OBJECTIVES: This study examined the hypothesis that participants diagnosed with obsessive-compulsive disorder (OCD) show a selective deficit in inductive reasoning but are equivalent to controls in deductive reasoning. METHODS: Twenty-five participants with OCD and 25 non-clinical controls made inductive and deductive judgments about a common set of arguments that varied in logical validity and the amount of positive evidence provided (premise sample size). A second inductive reasoning task required participants to make forced-choice decisions and rate the usefulness of diverse

Reasoning-driven large language models in medicine: opportunities, challenges, and the road ahead.

Developments in large language models (LLMs) in the past 2 years have shifted the focus from text, image, and audio generation to LLMs capable of multistep reasoning (thinking). The development of LLMs is particularly important for medicine and health care, but the translation of these models has been limited by the black-box nature of previous LLMs. New reasoning-driven LLMs incorporate chain-of-thought prompting and reveal intermediate reasoning steps, offering transparency and traceability, potentially improving the clinical adoption and utility of LLMs. In this Viewpoint, we examine four e

A Theoretical Framework for Shared Reasoning Fragility in Clinician-Chatbot Interactions Through the Example of Antibiotic Prescribing.

General-purpose large language model (LLM)-based chatbots are increasingly used by clinicians to discuss medical problems, including antibiotic prescribing. Their use creates an unprecedented setting for clinical reasoning in which diagnostic and therapeutic thinking becomes dynamically shared between human and machine. Here, we propose a theoretical framework, intended for subsequent empirical assessment, around the concept of shared reasoning fragility, defined as the potential instability arising from the interaction between clinician reasoning and the chatbot's opaque, association-based pr…

Med-CoReasoner: Reducing Language Disparities in Medical Reasoning via Language-Informed Co-Reasoning.

While reasoning-enhanced large language models perform strongly on English medical tasks, a persistent multilingual gap remains, with substantially weaker reasoning in local languages, limiting equitable global medical deployment. To bridge this gap, we introduce Med-CoReasoner, a language-informed co-reasoning framework that elicits parallel English and local-language reasoning, abstracts them into structured concepts, and integrates local clinical knowledge into an English logical scaffold via concept-level alignment and retrieval. This design combines the structural robustness of English re

Enhancing spatial reasoning by anodal transcranial direct current stimulation over the right posterior parietal cortex.

Spatial reasoning is essential for an agent's navigation and the cognitive processing of abstract arrangements. Meta-analyses of neuroimaging data reveal that both the right posterior parietal cortex and left dorsolateral prefrontal cortex (PPC and DLPFC, respectively) show increased activation during spatial relational reasoning. To investigate whether participants' reasoning performance can be modified and potentially enhanced, anodal transcranial direct current stimulation (tDCS) was applied over either region. 51 healthy adult participants solved spatial reasoning problems after the applic

International Framework for Red Flags for Potential Serious Spinal Pathologies.

The International Federation of Orthopaedic Manipulative Physical Therapists (IFOMPT) led the development of a framework to help clinicians assess and manage people who may have serious spinal pathology. While rare, serious spinal pathology can have devastating and life-changing or life-limiting consequences, and must be identified early and managed appropriately. Red flags (signs and symptoms that might raise suspicion of serious spinal pathology) have historically been used by clinicians to identify serious spinal pathology. Currently, there is an absence of high-quality evidence for the dia

Med-SORA: Symptom to Organ Reasoning in Abdomen CT Images.

Understanding symptom-image associations is crucial for clinical reasoning. However, existing medical multimodal models often rely on simple one-to-one hard labeling, oversimplifying clinical reality where symptoms relate to multiple organs. In addition, they mainly use single-slice 2D features without incorporating 3D information, limiting their ability to capture full anatomical context. In this study, we propose Med-SORA, a framework for symptom-to-organ reasoning in abdominal CT images. Med-SORA introduces RAG-based dataset construction, soft labeling with learnable organ anchors to captur…

Serum sepsis, not sickness.

Rarely taught in medical schools, clinical reasoning is the ability to discern the important from the unimportant and to arrive at accurate and efficient clinical conclusions. Identifying errors in reasoning is difficult; however, undetected clinical reasoning errors can have exponential consequences. As quality and patient safety come into focus, identifying and preventing clinical reasoning errors have become imperative. The authors present a case of a man sent for admission from a subspecialty clinic diagnosed with infliximab-induced serum sickness. Not countering the expert's diagnosis, in…
