Influence of the level of education on cognitive function in Parkinson's disease.

Level of Education (LoE) is widely used as an indicator of cognitive reserve and is associated with risk of dementia. The aim of the present study was to know the influence of the LoE on cognitive function (CF) in patients with Parkinson´s disease (PD). Controls and PD patients from the Spanish cohort COPPADIS with a disease duration from symptoms onset ≤ 5 years, who were recruited from January/2016 to November/2017 (baseline visit; V0) and evaluated at 2 (V2), 4 (V4) and 5 (V5) years of follow-up were included. Regarding LoE, patients were classified as with primary, secondary and university…

The Association Between Temporal Atrophy and Episodic Memory Is Moderated by Education in a Multi-Center Memory Clinic Sample.

BACKGROUND: Cognitive reserve (CR) is hypothesized to partially explain the discrepancy between Alzheimer's disease related brain pathology and cognitive performance. Educational attainment is often used as a proxy for CR. OBJECTIVE: To examine the association of years of education and the relationship between atrophy in the medial temporal lobe and episodic memory, in a cross-sectional ecological multi-center memory clinic cohort. METHODS: Included patients (n = 702) had undergone memory clinic examination and were diagnosed with subjective cognitive impairment (n = 99), mild cognitive impa…

Stronger bilateral functional connectivity of the frontoparietal control network in near-centenarians and centenarians without dementia.

Centenarians without dementia can be considered as a model of successful ageing and resistance against age-related cognitive decline. Is there something special about their brain functional connectivity that helps them preserve cognitive function into the 11th decade of life? In a cohort of 57 dementia-free near-centenarians and centenarians (95-103 years old) and 66 cognitively unimpaired younger participants (76-79 years old), we aimed to investigate brain functional characteristics in the extreme age range using resting-state functional MRI. Using group-level independent component analysis

Aversive distractors modulate affective working memory in frontoparietal regions.

Rumination and worry are prominent symptoms of many psychiatric disorders. These symptoms compete for the same working memory (WM) storage space as those required for task-goals, leaving few cognitive resources available for successful goal-directed behavior. Once lodged in WM, negative information reduces cognitive control and further biases cognition toward affective stimuli. However, few studies have examined the neural mechanisms associated with maintaining affectively negative information in WM and the filtering of aversive distractors. Here, subjects completed an affective verbal WM task

Is verbal episodic memory in elderly with amyloid deposits preserved through altered neuronal function?

A potential mechanism that enables intellectual preservation in cognitively normal elderly that harbor beta-amyloid (Aβ) pathology is heightened cerebral glucose metabolism. To investigate cross-sectional inter-relationships between Aβ, glucose metabolism, and cognition, 81 subjects (mean age: 75 ± 7 years) underwent [(11)C]Pittsburgh Compound-B and [(18)F]fluorodeoxyglucose positron emission tomography scans and neuropsychological testing. They were divided into low-Aβ (n = 53), intermediate-Aβ (n = 13) and high-Aβ (n = 15) groups as defined by their global cortical [(11)C]PIB retention. Gluc

Impact of mindfulness training on emotion regulation in multiple sclerosis: Secondary analysis of a pilot randomized controlled trial.

UNLABELLED: Purpose/Objective Research: This secondary analysis of a pilot randomized controlled trial in people with multiple sclerosis (PwMS) aimed to compare mindfulness-based training (MBT), adaptive cognitive training (aCT), and a waitlist control (WL) on the use of emotion regulation strategies during daily worries and ruminations. Further, we examined cognitive functioning as a moderator of training effects. RESEARCH METHOD/DESIGN: Sixty-one PwMS were randomized into an MBT, aCT, or a WL control group for four weeks. Participants completed daily diaries assessing their use of emotion r

The latent factor structure underlying regional brain volume change and its relation to cognitive change in older adults.

OBJECTIVE: Late-life changes in cognition and brain integrity are both highly multivariate, time-dependent processes that are essential for understanding cognitive aging and neurodegenerative disease outcomes. The present study seeks to identify a latent variable model capable of efficiently reducing a multitude of structural brain change magnetic resonance imaging (MRI) measurements into a smaller number of dimensions. We further seek to demonstrate the validity of this model by evaluating its ability to reproduce patterns of coordinated brain volume change and to explain the rate of cognitiv

Height as a marker of childhood development and late-life cognitive function: the Honolulu-Asia Aging Study.

OBJECTIVE: Growing evidence suggests that structural and functional brain reserves, thought to develop in childhood and adolescence, may be crucial in determining when cognitive impairment begins. The purpose of this report is to examine the relationship of height, as a marker of childhood development, to late-life cognitive function in a sample of elderly Japanese-American men. METHOD: Cognitive performance was assessed from 1991 to 1993 in the Honolulu-Asia Aging Study in 3733 men aged 71 to 93 years and related to height that was measured 25 years earlier. RESULTS: Among the study sample,

Epigenetic memory and allostatic integrity: a theoretical integration of the hierarchical-circular model using a longitudinal methylation signature of dementia.

INTRODUCTION: The Hierarchical-Circular Model of Biological Memory (HCM) proposes that dementia results from disrupted circular information flow across five levels: genetic, epigenetic, allostatic, PNEI network, and interoceptive. Allostatic integrity-the adaptive capacity for circular flow-determines whether pathology translates into dementia. OBJECTIVE: To evaluate whether a recently published blood DNA methylation signature for incident dementia is consistent with the predictions of the HCM. METHODS: We systematically mapped Zhang et al.'s findings (44 CpGs, 44 DMRs, pathway enrichments,

p-Tau/Aβ42 ratio associates with cognitive decline in Alzheimer's disease, mild cognitive impairment, and cognitively unimpaired older adults.

OBJECTIVE: Among the cerebrospinal fluid (CSF)-based measures of Alzheimer's disease (AD) pathology, the ratiometric measure of p-tau/Aβ42 shows the best diagnostic accuracy. However, few studies have linked the p-tau/Aβ42 ratio to cognition directly. The goal of this study was to examine whether a CSF-based p-tau/Aβ42 ratio predicts changes in global cognitive functioning, episodic memory, and executive functioning over a 2-year period in cognitively unimpaired (CU) older adults, in individuals with mild cognitive impairment (MCI), and in those with AD. METHOD: This study involves secondary

Compensatory mechanisms in higher-educated subjects with Alzheimer's disease: a study of 20 years of cognitive decline.

A better knowledge of long-term trajectories of cognitive decline is a central feature of the study of the process leading to Alzheimer's dementia. Several factors may mitigate such decline, among which is education, a major risk factor for Alzheimer's disease. The aim of our work was to compare the pattern and duration of clinical trajectories before Alzheimer's dementia in individuals with low and high education within the PAQUID cohort involving 20 years of follow-up. The sample comprises 442 participants with incident Alzheimer's disease (27.2% were male)--171 with low education (mean age=

Bilingualism predicts executive function resilience after COVID-19 in aging.

Bilingualism has been associated with enhanced executive functions (EFs), particularly attentional control, and may confer protection against cognitive decline in older age. At the same time, the COVID-19 pandemic has emerged as a factor negatively affecting EF in older adults. Bilingualism might offer resilience against these COVID-related cognitive declines, especially in late adulthood, by bolstering cognitive reserve. The present study collected data from 312 community-dwelling individuals spanning the adult lifespan (aged 18 to 80) to address two goals. Goal 1 was to identify a latent str…

Inhibitory control underpins the relationship between cognitive and psychological inflexibility after a moderate to severe traumatic brain injury.

OBJECTIVE: Cognitive flexibility is proposed as being one "building block" of psychological inflexibility/flexibility, yet empirical studies examining these associations are scarce. This study aims to examine the relationship between these constructs in those with a moderate to severe traumatic brain injury who demonstrate impairments in cognitive flexibility. METHOD: A total of 66 individuals with a traumatic brain injury were administered a battery of cognitive flexibility measures in conjunction with their standard neuropsychological assessment, general (Acceptance and Action Questionnaire

Inflammaging and neurovascular unit dysfunction in cognitive ageing: mechanisms, biomarkers, and therapeutic opportunities.

Age-related cognitive decline is increasingly recognised as arising from interacting immune and neurovascular vulnerabilities rather than from isolated insults. In this Review, we synthesise current human and mechanistic evidence within a convergence framework in which inflammaging and neurovascular unit (NVU) dysfunction are conceptualised as a feedback-coupled system. Persistent immune remodelling may promote endothelial activation, redox imbalance, pericyte-astrocyte support failure, microglial priming, and impaired neurovascular coupling, thereby reducing cerebrovascular reserve and destab

Clonidine for Management of Agitation in Delirious Patients.

PURPOSE OF REVIEW: This review explores clonidine as a potential treatment for agitation across various clinical contexts, focusing on its application in patients with limited cognitive reserve, hyperactive delirium, or conditions where standard treatments may exacerbate underlying symptoms, such as Parkinsonism. The review evaluates the pharmacological properties, efficacy, and practical considerations for using clonidine in managing agitation, including its role in other psychiatric and medical conditions. RECENT FINDINGS: Clonidine, a centrally acting alpha-2 adrenergic agonist, reduces nor…

Anesthetic management of geriatric patients.

The number of elderly patients who frequently access health care services is increasing worldwide. While anesthesiologists are developing the expertise to care for these elderly patients, areas of concern remain. We conducted a comprehensive search of major international databases (PubMed, Embase, and Cochrane) and a Korean database (KoreaMed) to review preoperative considerations, intraoperative management, and postoperative problems when anesthetizing elderly patients. Preoperative preparation of elderly patients included functional assessment to identify preexisting cognitive impairment or
