[Mesenteric inflammatory veno-occlusive disease is a rare nosological entity: Literature review and the authors' clinical observation].

Mesenteric inflammatory veno-occlusive disease (MIVOD) is a rare disease. Its basis is inflammation of the small and medium-sized mesenteric veins, which is generally complicated by thrombosis and intestinal ischemia. Arteries are not therewith involved in the process. According to the location, either ischemic jejunitis (ileitis) or segmental colitis develops. The etiology and incidence of MIVOD are unknown. Only a few tens of descriptions of clinical cases have been published. MIVOD is differentially diagnosed with Crohn's disease and vasculitides (Behçet's syndrome, Buerger's syndrome, rheu

Association of histologic and clinical activity with major adverse cardiovascular events in patients with inflammatory bowel disease: A cohort study.

OBJECTIVES: Inflammatory bowel disease (IBD) is a chronic disorder linked to cardiovascular disease (CVD). However, the impact of histologic and clinical activity on this association remains unclear. METHODS: We conducted a nationwide cohort study in Sweden involving 59,168 IBD patients diagnosed in 1969-2017 with histologic evaluation and 91,800 patients diagnosed in 1969-2020 with assessment of clinical activity in 2006-2021. The primary outcome was incident major adverse cardiovascular events (MACE), a composite outcome encompassing ischemic heart disease, stroke, and heart failure. Cox pr

Adrenomedullin: A vasoactive agent for sporadic and hereditary vascular cognitive impairment.

Adrenomedullin (AM) is an endogenous peptide mainly secreted from endothelial cells, which has multiple physiological actions such as anti-inflammation, vasodilation, vascular permeability regulation and angiogenesis. Blood AM levels are upregulated in a variety of pathological states including sepsis, severe COVID-19, acute ischemic stroke and vascular cognitive impairment with white matter changes, likely serving as a compensatory biological defense response against infection and ischemia. AM is currently being tested in clinical trials for ulcerative colitis, Crohn's disease, severe COVID-1

[Contrast-Enhanced Ultrasound as a Diagnostic Tool in Diseases of the Digestive Tract].

Contrast-Enhanced Ultrasound as a Diagnostic Tool in Diseases of the Digestive Tract Abstract. Contrast-enhanced ultrasound is a good diagnostic tool in certain gastrointestinal diseases. Inflammation of the gastric and the bowel wall can often be distinguished from neoplastic alterations. Gastric and duodenal stenosis can be depicted with the use of oral contrast, and after stenting the patency can be documented. Abscesses are perfectly delineated, and after drainage the exact location of the tube and possible complications can be documented. In patients with Crohn's disease inflammatory acti

[Intestinal endometriosis].

Endometriosis is a chronic, benign gynaecological disorder that is frequent in women of a child-bearing age. It is estimated that there is some degree of endometriosis in as many as 15% of pre-menopausal women, associated with a history of infertility, caesarean antecedents, dysmenorrhoea and abnormality in uterine bleeding. It is believed to be due to the rise of menstrual contents through the Fallopian tubes (retrograde menstruation). In the intestinal affectation, the colon is the segment most frequently affected, above all at the rectosigmoidal level. The clinical features are unspecific,

[Cholesterol embolism: report of one case].

We report a 72 year-old male, that after a coronary angiogram presented pain, reduced distal skin temperature of both limbs, cyanosis of toes with preserved peripheral pulses and a rapidly progressive renal failure. Afterwards, the patient suffered a sudden bilateral amblyopia and hematochezia. Cholesterol embolism was confirmed with a skin biopsy and fundoscopy. A colonoscopy showed a possible ischemic colitis. After six months of follow up, the patient lost the distal phalanges of three toes, and renal failure stabilized, with a serum creatinine of 2.5 mg/dl. The diagnosis of cholesterol emb

Unusual causes of abdominal pain: sickle cell anemia.

Sickle cell disease is characterized by chronic hemolytic anemia and vaso-occlusive painful crises. The vascular occlusion in sickle cell disease is a complex process and accounts for the majority of the clinical manifestation of the disease. Abdominal pain is an important component of vaso-occlusive painful crises. It often represents a substantial diagnostic challenge in this population of patients. These episodes are often attributed to micro-vessel occlusion and infarcts of mesentery and abdominal viscera. Abdominal pain due to sickle cell vaso-occlusive crisis is often indistinguishable f

Multivessel Coronary Artery Disease in a Patient Treated With Tofacitinib: A Case Report.

Tofacitinib is a second-generation selective Janus kinase (JAK) inhibitor targeting the JAK1 enzyme, which is used to treat several autoimmune diseases, including rheumatoid arthritis (RA), psoriatic arthritis, ankylosing spondylitis, idiopathic arthritis, and ulcerative colitis. JAKs are enzymes that play a key role in inflammatory and immune signaling pathways. The medication is particularly useful as it can be given orally. However, over time, there has been much data that links this drug with an increased risk of cardiovascular events. We present a case of a 73-year-old male with a history

Current indications for the Hartmann procedure.

The Hartmann procedure is used in the case of left-sided colonic disease, especially in the setting of emergency where intraoperative conditions contraindicate completion of an anastomosis. This procedure has been initially described for the management of colorectal cancer and is based on a sigmoïdectomy without restoration of intestinal continuity, including a left-sided iliac terminal stoma and closure of the rectal stump. Both procedure and underlying risk factors explain high rates of mortality and morbidity, around 15 and 50% respectively, and a low overall rate of subsequent restoration

Multiorgan with renal infarction following treatment of cerebral infarction.

Acute renal infarction is a rare disease and it is often difficult to make a clinical diagnosis due to the non-specific clinical presentations and lack of the physicians' awarenesses. We experienced a case of a 72-year-old man who was diagnosed as multiorgan with renal infarction during the bridge therapy of cerebral infarction with atrial fibrillation. Computed tomogram (CT) with intravenous contrast of the abdomen and pelvis revealed left renal infarction with renal artery occlusion, multifocal splenic infarction, and ischemic colitis on rectum and sigmoid colon. The patient was treated with

Mesenteric panniculitis presenting with acute non-occlusive colonic ischemia.

BACKGROUND: The role of positron emission tomography (PET) of the mesentery as a diagnostic modality in cases of mesenteric panniculitis is unclear. CASE PRESENTATION: A 67-year-old woman presented with rectal bleeding due to nonocclusive colonic ischemia. Abdominal CT showed features of mesenteric panniculitis. PET-CT demonstrated no abnormal fluorine-18 fluordeoxyglucose uptake in the affected mesentery or any surrounding lymph nodes. Laparoscopic biopsies from a thickened segment of mesenteric fat excluded neoplastic infiltration. CONCLUSIONS: In cases of unexplained ischemic colitis, pan

Inflammatory bowel disease and cerebrovascular arterial and venous thromboembolic events in 4 pediatric patients: a case series and review of the literature.

Inflammatory bowel disease has been linked to cerebrovascular lesions, but the mechanisms of these vascular complications and their frequency among children with inflammatory bowel disease are unclear. We present 4 children with inflammatory bowel disease who developed ischemic or hemorrhagic stroke or cerebral sinovenous thrombosis. All 4 patients were female; 3 had Crohn's disease and 1 had indeterminate colitis. All of the patients had additional risk factors for thrombosis including thrombocytosis, severe dehydration attributable to an inflammatory bowel disease exacerbation, and, in 2 ins

Effect of basic fibroblast growth factor, trafermin, on entero-related fistulae, report of two cases.

Entero-related fistulae are often intractable. Recently, growth factors were generally in sight with the development of regenerative medicine. Basic fibroblast growth factor (bFGF) possesses potent angiogenic activity. In Japan, recombinant bFGF, also known as Trafermin, has been administered for intractable skin ulcer for its strong effect on tissue granulation. Here, we report the effect of bFGF for entero-related fistulae. Two intractable cases were treated by applying bFGF in the fistulae after performing the enterostomy. Case 1: Postoperative intractable vesicorectal fistula was treated b

[Gastroenterology: recent advances and perspectives].

Among the significant current topics in gastroenterology, the endoscopic capsule, virtual colonography, and ischaemic colitis deserve special considerations. Main indications of the endoscopic capsule are the diagnosis of small intestinal (bleeding) angiodysplasias, Crohn's lesions and tumours. The main drawbacks are gastrointestinal blockage of the capsule due to stenosis and flat batteries before the capsule reaches the distal small intestine. Future advantages may stem from a wider pre-use of an auto-digestible capsule capable of pre-cautionary excluding stenotic gastrointestinal segments a

D-lactate as an early marker of intestinal ischaemia after ruptured abdominal aortic aneurysm repair.

BACKGROUND: Patients with a ruptured abdominal aortic aneurysm (AAA) are at risk of developing colonic ischaemia after surgery. It is difficult to diagnose this ischaemia at an early stage. D-lactate is produced by intestinal bacteria after ischaemia. L-lactate is released in increased amounts during hypoxia by anaerobic metabolism. This study investigated both variables as a marker for intestinal ischaemia in patients with a ruptured AAA. METHODS: Twenty-four patients with ruptured AAA were divided retrospectively into two groups with and without ischaemic complications, as verified by colon

Idiopathic mesenteric phlebosclerosis associated with use of Chinese herbal medicine: Two case reports.

RATIONALE: Idiopathic mesenteric phlebosclerosis (IMP) is a rare form of ischemic colitis. It is more common in the Asian population people with Asian ancestry. Disease pathogenesis and etiology are not fully elucidated but may be associated with the long-term intake of toxins and other substances, including Chinese herbs. The disease has typical radiological and endoscopic features. Radiologic examination combined with endoscopy can lead to a conclusive diagnosis. PATIENT CONCERNS: We present 2 cases of IMP: in male patients aged 66 and 79 years. The first patient presented with diarrhea and

Colonic ischemia and perforation associated with enteral feeding through an ileal tube.

Reported complications of enteral feeding through a jejunostomy include diarrhea, intraperitoneal leaks, bowel obstruction, fistula formation, wound infection, tube occlusion, and other mechanical malfunctions. However, the incidence of these complications is very low, and many physicians prefer to feed their patients by means of a jejunal tube instead of parenteral nutrition. A potentially lethal complication is ischemia of the bowel distal to the site of insertion of the feeding catheter. The described cases of bowel ischemia secondary to enteral nutrition invariably occurred at the level of

[Current status of ultrasound in gastroenterology--bowel and upper gastrointestinal tract--part 2].

Ultrasound has gained acceptance in the diagnosis of diseases of the gastrointestinal tract beside the classical methods such as endoscopy and X-ray. In a previous publication we discussed the use of ultrasound in emergency diagnostics (e. g., acute appendicitis, diverticulitis/peridiverticulitis, ileus, invagination and perforation) (part 1). Because of the vast extent of this topic, in this overview we will focus on the current role of ultrasound in the detection and assessment of chronic inflammatory bowel diseases, rare forms of colitis (e. g., bacterial, pseudomembranous and neutropenic c

[Pathomorphology of ulcerative colitis].

Microscopic and macroscopic appearances of ulcerative colitis (UC) by its phase of inflammation were summarized. The most characteristic microscopic findings of active phase UC is diffuse lymphoplasmacytic infiltration, essentially associated with basal plasmacytosis. Although inflammation of UC is basically limited to the mucosa, active inflammation extends into the submucosa in some instance, and acute ischemic change is overlapped to cause toxic megacolon. In remission phase, inflammation is reduced and goblet cell mucus is fully recovered but evidences of the past inflammation such as irre
