Supervised Toothbrushing Programmes: Understanding Barriers and Facilitators to Implementation.

OBJECTIVES: Supervised toothbrushing programmes (STPs), whereby children brush their teeth at nursery or school with a fluoride toothpaste under staff supervision, are a clinically and cost-effective intervention to reduce dental caries. However, uptake is varied, and the reasons unknown. The aim was to use an implementation science approach to explore the perspectives of key stakeholders on the barriers and facilitators at each level of implementation of STPs. METHODS: This qualitative study involved individual interviews and focus groups with a purposive sample of stakeholders involved at a

Oral and Oropharyngeal Cancer Mortality in Brazil: Estimates and Projections up to 2042.

OBJECTIVES: To assess the effect of age-period-birth cohort on mortality rates related to lip/oral and oropharyngeal cancer (LOOPC) in Brazil from 1980 to 2019 and to estimate the future mortality rate for 2042. MATERIALS AND METHODS: Mortality rate per 100 000 inhabitants, and age-standardised mortality rate (ASR) per 100 000 inhabitants were estimated. The Prais-Winsten regression model was used to estimate the trends and the annual percent change (APC%). The age-period-cohort effects were calculated using the Poisson regression model. Lee-Carter model was employed to perform projections.

Assessing the Impact of Periodontal Therapy on Tooth Loss: A Register-Based Longitudinal Study in Denmark.

OBJECTIVES: While regular periodontal care is advocated to reduce tooth loss considerably among periodontitis patients, evidence from observational studies is often limited to small single-center studies. This study aims to quantify the effect of periodontal care on tooth extractions among 40-year-old new periodontitis patients over 20 years. METHODS: A nationwide register-based cohort study was conducted, encompassing 40-year-old individuals with incident periodontitis in 2001, tracked through Danish registers until the end of 2021. Receiving any periodontal care was determined annually, and

Association of Prolonged Breastfeeding With Early Childhood Caries Using Propensity Score Matching in the French Longitudinal Study of Children (ELFE Cohort).

OBJECTIVES: The aim of this study was to investigate the link between prolonged breastfeeding (≥ 12 months) and early childhood caries (ECC) using propensity score matching (PSM) to account for observed confounders, reduce bias, and provide a more reliable estimate of this relationship. METHODS: This study utilised data from the French Longitudinal Study of Children (ELFE Cohort), comprising 11 718 participants. PSM was employed to pair children who were breastfed for 12 months or longer with those breastfed for less than 12 months or not at all, controlling for shared risk factors such as so

Cost-Effectiveness of Applying Fluoride Varnish to Preschoolers in a Brazilian Scenario: An Economic Modelling Study.

BACKGROUND: International economic evaluations have not found convincing evidence that the application of fluoride varnish (FV) in preschool children is a cost-effective anti-caries measure, and there is a lack of economic evaluations of FV in the Brazilian context. AIM: This study evaluated the cost-effectiveness (CE) of standard care plus FV for Brazilian preschoolers in the general population, comparing it to standard care in terms of prevention of cavitated caries lesions and disability-adjusted life years (DALY) outcomes. METHODS: Markov models were used, with a 4-year time horizon and

Model-Based Cost-Effectiveness of Direct Restorations: Amalgam Dominates.

OBJECTIVES: A phase-down of dental amalgam use has been mandated and the feasibility of its phase-out in England by 2030 is being explored. Amalgam use in English National Health Service (NHS) care still predominates for posterior restorations, though access to this care is increasingly limited. The objective of this study was to quantify the relative long-term costs and consequences of amalgam versus composite direct posterior restorations in adult permanent teeth in the English NHS setting. METHODS: A microsimulation model of restoration failure and reintervention was constructed and parame

Change in Dental Visits Among Eligible Children Under the Impact of the Child Dental Benefits Schedule in Australia.

OBJECTIVES: In Australia, although there have been some improvements, child oral health continues to be a major public health issue. The Australian Government introduced the means-tested Child Dental Benefits Schedule (CDBS) in 2014 to support access to dental services for children and adolescents aged 0-17 years from low-income families. There is a lack of evidence documenting whether the CDBS improved the dental attendance rate. This study aimed to evaluate the impact of the CDBS on dental visits among eligible children and adolescents in Australia. METHODS: The study analysed the data set

Self-perceived quality of social roles, activities and relationships predicts incident gingivitis.

OBJECTIVES: While physical health status is known to impact social functioning, a growing literature suggests that social well-being may affect oral health. This investigation evaluated whether self-perceived quality of social roles, activities and relationships (social well-being) influences gingival inflammation. METHODS: Data were from the Population Assessment of Tobacco and Health Study, a nationally representative cohort of US adults, biennial waves 4 (2017) to 6 (2021). Social well-being was derived from the validated PROMIS Global-10 survey instrument, categorized for this longitudina

Applying current European periodontitis clinical practice guidelines is not feasible even for the richest countries in the world.

Clinical practice guidelines aim to enhance the quality, equality and consistency of care but often demand more time than is available, rendering adherence impractical and exceeding feasible resources. The 2017 introduction of a new periodontal classification system by the American Academy of Periodontology (AAP) and the European Federation of Periodontology (EFP) sought to refine clinical and epidemiological practices by serving as the basis for clinical practice guidelines and epidemiological investigations around the world. Following this classification, the EFP recommends supportive period
