One session of high frequency repetitive transcranial magnetic stimulation (rTMS) to the right prefrontal cortex transiently reduces cocaine craving.

BACKGROUND: Cocaine dependence is a public health problem affecting 2 million individuals in USA. Craving is a predictor of subsequent cocaine use and is related to changes in brain activity in networks involving the prefrontal cortex. METHODS: We investigated the efficacy of one session of high frequency repetitive transcranial magnetic stimulation (rTMS) to reduce craving in cocaine addicted subjects. Six patients underwent two sessions of 10Hz rTMS over left or right dorsolateral prefrontal cortex (DLPFC). Before, immediately after and 4h after rTMS we measured craving using visual analogu…

Fixed-dose, extended-release combination of naltrexone and bupropion reduces craving and body weight: A pooled post hoc analysis.

BACKGROUND: The fixed-dose, extended-release combination of naltrexone and bupropion (NB-ER), approved for obesity treatment, is hypothesized to affect the reward system and reduce food cravings. METHODS: A pooled, on-treatment, post hoc analysis was performed on data from 1967 patients who completed a phase 3 trial of NB-ER (NCT00532779, NCT00567255, NCT00456521, NCT00474630). Changes in weight and cravings based on Control of Eating Questionnaire (CoEQ) and Food Craving Inventory (FCI) scores, as well as mood and craving control, were examined for NB-ER vs placebo at weeks 8, 16, 28, and 56…

Combined Attention Bias Modification and Working Memory Training for Reducing Craving in Methamphetamine Use Disorder: A Randomized Controlled Study.

Background: Craving is central to relapse risk in methamphetamine use disorder (MUD), yet the affective-cognitive processes related to craving remain unclear. Objectives: This study examined whether negative affect was associated with craving and whether attentional bias and working memory capacity (WMC) were involved in this association, and subsequently evaluated the effectiveness of a combined emotion-cue-based attentional bias modification (ABM) and adaptive working memory training (WMT) intervention. Methods: Fifty individuals with MUD completed measures of negative affect (DASS-21), emot…

Effects of Omega-3 Fatty Acids on Tobacco Craving in Tobacco Users: A Single-blind, Randomized, Placebo-controlled Study.

BACKGROUND: Tobacco use and its smoke produces oxidative stress in the body, which eventually triggers cell damage by lipid peroxidation. Smokers report lower levels of omega-3 fatty acids (FAs) in their serum as compared to nonsmokers. Omega-3 deficiency impairs neurotransmission, resulting in hypofunctioning of the mesocortical system, which is a reward and dependency system that can raise tobacco cravings, disrupting tobacco quitting efforts. Omega-3 polyunsaturated fatty acid (PUFA) regulates stress, anxiety, and negative emotions that are associated with tobacco urges. Limited research ha…

Adjunctive oral naltrexone for breakthrough craving in patients receiving extended-release injectable naltrexone for alcohol use disorder: Three cases.

Craving is a subjective, poorly characterized phenomenon strongly associated with return to use in alcohol use disorder and remains difficult to manage. Extended-release naltrexone is a U.S. Food and Drug Administration-approved, evidence-based treatment, yet some patients experience brief, intense "breakthrough" craving despite adherence. Although expert guidance references oral naltrexone supplementation as an option during extended-release naltrexone maintenance, published peer-reviewed case descriptions documenting this clinical scenario are lacking. We describe three adults with severe al…

Virtual Reality Trier Social Stress and Virtual Supermarket Exposure: Electrocardiogram Correlates of Food Craving and Eating Traits in Adolescents.

BACKGROUND/OBJECTIVES: Acute stress is known to influence food-related motivation and decision-making, often promoting a preference for energy-dense, palatable foods. However, traditional laboratory paradigms have limited ecological validity. This study examined the relationship between stress-induced physiological changes, eating behavior traits, and food cravings using a virtual reality (VR) adaptation of the Trier Social Stress Test (VR-TSST) followed by a VR supermarket task in adolescents. METHODS: Thirty-eight adolescents (mean age 15.8 ± 0.6 years) participated in the study. Physiologi…

Virtual reality-based cue exposure therapy reduces psychological craving in men with methamphetamine use disorder: a randomized controlled trial.

Reducing psychological craving is critical for preventing relapse in methamphetamine use disorder (MUD). This study aimed to evaluate the efficacy of virtual reality (VR)-based cue exposure therapy (CET) and cue exposure with aversion therapy (CETA) in reducing methamphetamine craving in men with MUD. In this randomized controlled trial, 89 men with MUD were assigned to three groups: VR-based cue exposure therapy (CET, n = 30), VR-based cue exposure combined with aversion therapy (CETA, n = 29), and neutral scenes (NS, n = 30). The intervention comprised 16 sessions over 8 weeks. Primary outco…

Cannabidiol alters psychophysiological, craving and anxiety responses in an alcohol cue reactivity task: A cross-over randomized controlled trial.

BACKGROUND: Preclinical studies have demonstrated that cannabidiol (CBD) reduces alcohol-seeking behaviors and may have potential for managing alcohol use disorder (AUD). In this study, we examined the effects of CBD versus placebo on (i) psychophysiological, craving and anxiety responses to alcohol and appetitive cues; (ii) tolerability measures including cognitive functioning. METHODS: Twenty-two non-treatment-seeking individuals with AUD (DSM-5) participated in a cross-over, double-blind, randomized trial, receiving either 800 mg of CBD or matched placebo over 3 days. A laboratory alcohol …

Impact of nurse-led anonymous coaching on alcohol craving and motivation to change behaviour among patients with alcohol dependence.

Alcohol use disorder (AUD) is a chronic condition marked by compulsive alcohol consumption despite its harmful effects. Harmful alcohol use leads to over 2.6 million deaths annually worldwide. This study assessed the impact of nurse-initiated anonymous coaching sessions on alcohol cravings and motivation to change behavior in alcohol-dependent patients. Over four weeks, 100 participants were divided into experimental and control groups, with the experimental group showing significant reductions in cravings and improvements in motivation. Thus, we show that nurse-led behavioral coaching is an e…

Exploring gender differences in tobacco cue-induced craving and heart rate variability in individuals with a tobacco use disorder.

INTRODUCTION: Women face greater challenges quitting smoking and higher health risks than men, yet gender remains understudied in tobacco use disorder (TUD). This study investigates gender differences in subjective craving and heart rate variability (HRV) following tobacco cue exposure in abstinent individuals with a tobacco use disorder. Unlike heart rate, HRV reflects parasympathetic modulation, critical for understanding risk and resilience in addiction, but has rarely been studied as a cue-reactivity biomarker. METHODS: Data from 41 men and 40 women who smoked cigarettes for more than 10 …

Neural Correlates of Stress and Alcohol Cue-Induced Alcohol Craving and of Future Heavy Drinking: Evidence of Sex Differences.

OBJECTIVE: Stress and alcohol cue reactivity are associated with poor treatment outcomes in alcohol use disorder (AUD), but sex-specific neural correlates of stress and alcohol cue-induced craving compared with neutral cue-induced craving and of heavy drinking outcomes in AUD have not been examined. Thus, this study prospectively examined these associations and assessed sex differences. METHODS: Treatment-seeking adults with AUD (N=77; 46 men and 31 women) completed a functional MRI task involving stress, alcohol, and neutral cue exposure with repeated assessments of alcohol craving. Most of …

Profiling prefrontal cortex protein expression in rats exhibiting an incubation of cocaine craving following short-access self-administration procedures.

INTRODUCTION: Incubation of drug-craving refers to a time-dependent increase in drug cue-elicited craving that occurs during protracted withdrawal. Historically, rat models of incubated cocaine craving employed extended-access (typically 6 h/day) intravenous drug self-administration (IV-SA) procedures, although incubated cocaine craving is reported to occur following shorter-access IV-SA paradigms. The notoriously low-throughput of extended-access IV-SA prompted us to determine whether two different short-access IV-SA procedures akin to those in the literature result in qualitatively similar c…

Persistent increase of accumbens cocaine ensemble excitability induced by IRK downregulation after withdrawal mediates the incubation of cocaine craving.

The incubation phenomenon, cue-induced drug craving progressively increasing over prolonged withdrawal, accounts for persistent relapse, leading to a dilemma in the treatment of cocaine addiction. The role of neuronal ensembles activated by initial cocaine experience in the incubation phenomenon was unclear. In this study, with cocaine self-administration (SA) models, we found that neuronal ensembles in the nucleus accumbens shell (NAcSh) showed increasing activation induced by cue-induced drug-seeking after 30-day withdrawal. Inhibition or activation of NAcSh cocaine-ensembles suppressed or p…

Partial recovery of the left DLPFC-right insula circuit with reduced craving in abstinent heroin users: a longitudinal study.

The phenomenon of brain recovery after long-term abstinence has been reported in substance use disorders. However, few longitudinal studies have been conducted to observe the potential recovery in heroin users, and little is known about the neural mechanism underlying the decreased craving after prolonged abstinence. The 8-month longitudinal study was carried out in 29 heroin users and 30 healthy controls. By choosing the L_DLPFC, which was activated by the heroin cue as the seeding region, different brain connection patterns were compared between healthy controls and heroin users by using Gra…
