The syndrome of hypoparathyroidism, deafness, and renal anomalies.

OBJECTIVE: We review the syndrome of hypoparathyroidism, deafness, and renal anomalies (HDR syndrome). METHODS: The current understanding and relevant literature pertaining to the background, genetic considerations, clinical features, prognosis, and treatment of HDR syndrome are reviewed. RESULTS: The combination of hypoparathyroidism, deafness, and renal anomalies constitutes an unusual syndrome associated most commonly with haploinsufficiency in GATA3, which encodes a transcription factor that binds to the (A/T) GATA (A/G) consensus DNA sequence. Sensorineural hearing loss is the most cons…

Lead and the deafness of Ludwig van Beethoven.

OBJECTIVES/HYPOTHESIS: To reexamine the cause of Beethoven's hearing loss because of significant recent articles. DATA SOURCES: Medical and … disease. The absence of mercury in Beethoven's hair and bone samples leads us to conclude that his deafness was not due to syphilis because in that era syphilis was treated with mercury. High levels of lead deep in the bone suggest repeated exposure over a long period of time rather than limited exposure prior to the time of death. The finding of shrunken …

[GJB2 gene mutation in deaf patients].

OBJECTIVE: To detect the GJB2 gene mutation in patients with autosomal-recessive deafness, and analyze the relationship between clinical phenotype and gene mutation. METHODS: Forty-two patients were examined clinically by pure tone audiometry, acoustic impedance and auditory brainstem response. The complete coding region of the GJB2 gene was amplified by polymerase chain reaction (PCR) and the PCR products were subjected to automatic DNA sequencing. RESULTS: Two cases had homozygous mutation of 235delC. One of them had sensorineural hearing loss while the other had mixed hearing loss. Hetero…

[Auditory neuropathy in deaf school students].

OBJECTIVE: To investigate incidents and clinical features of auditory neuropathy in Nanjing deaf school students. METHOD: Three hundred and fifty-eight deaf students in the school accepted the first examination including otoscopic examination, tympanometry and transiently evoked otoacoustic emissions (TEOAE) screening. Detailed audiological and vestibular evaluations including pure-tone audiometry, immittance audiometry and acoustic reflex measures, transiently evoked otoacoustic emissions (TEOAE), distortion product otoacoustic emissions (DPOAE), auditory brain stem response (ABR), electrony

A cast-iron guarantee of ataxia and deafness.

Superficial siderosis of the central nervous system (CNS) is a rare condition that is thought to be caused by repeated subarachnoid haemorrhages. The major clinical features are progressive bilateral hearing loss, cerebellar ataxia and pyramidal dysfunction. Ante-mortem diagnosis is now possible due to MRI and post mortem reports of this condition are becoming rare. Despite the occurrence of typical clinical features the diagnosis is often missed during life if CT scanning alone is performed. We describe a case of superficial siderosis of the CNS with clinical and post mortem findings as a rem

Recent advances in the treatment of sensorineural deafness.

In the developed world, there are currently more than 100 million people afflicted with hearing loss. In the United States and European Community alone, there are an estimated 21 million people with significant conductive hearing loss, whilst there are over 90 million suffering from moderate to severe sensorineural hearing loss. Of these, more than 65 million hearing-impaired persons are without treatment. This article presents a review of the latest technological advances and treatment options for the hearing-impaired, including external and middle ear devices, bone-anchored hearing aids, coc

Oscillucusis and sudden deafness in a migraine patient.

Migraine is a complex disease that includes neurologic, gastrointestinal and autonomic symptoms, although headache is most common feature. In a portion of cases headache is preceded by focal neurologic symptoms termed auras. Auditory symptoms only rarely occur as part of an aura. We describe a patient whose 13-year migraine history that included the abnormal perception an oscillation of the intensity of ambient sounds (oscillucusis). During a migraine attack immediately after oscillucusis, the patient developed acute and permanent sudden deafness. Clinical and neurologic examinations revealed …

[A case of subarachnoid hemorrhage complaining of deafness].

A 64-year-old female came to our department because of a sudden onset of bilateral deafness 2 days before. She had sudden onset of mild headache, nausea and vomiting 9 days before, but was diagnosed as food poisoning by her home doctor. Her symptoms disappeared on the following day. Neurological examination revealed bilateral deafness, right facial palsy of central type and very slight neck stiffness. CT showed inconspicuous subarachnoid hemorrhage, but lumber puncture revealed definite subarachnoid hemorrhage. Another important finding of CT was old left temporal lobe infarction. Cerebral ang…

A threshold model analysis of deafness in Dalmatians.

To elucidate the inheritance of deafness in Dalmatian dogs, 825 dogs in 111 litters were evaluated for abnormalities in hearing through the brainstem auditory evoked response (BAER). Recorded along with their quality of hearing (normal, unilaterally deaf, or bilaterally deaf) were the sex, coat color, eye color and the presence or absence of a color patch. The analysis considered deafness an ordered categorical trait in a threshold model. The underlying, unobservable continuous variate of the threshold model was assumed to be a linear function of sex of dog, coat color (black or liver and whit…

[Effect of weather on the incidence of sudden deafness].

BACKGROUND: The etiology of sudden idiopathic hearing loss is not known. Among other causes, vascular disease is discussed as a possible pathomechanism in sudden hearing loss. Since other vascular diseases such as myocardial infarction, migraine, and open-angle glaucoma show a clear meteorotropy, it was the aim of the present prospective study to investigate the dependence of sudden hearing loss on weather parameters. PATIENTS AND METHODS: The incidence, the extent, and the remission of sudden idiopathic hearing loss in 128 patients were correlated with atmospheric pressure and temperature du

Ocular survey of deaf-mute children.

Through routine ocular examination of 279 Deaf-mute students in Lie De School in Guangzhou in 1988 by visual chart, Yu Zi Ping Pseudoisochromatic plate, portable slit lamp and direct ophthalmoscope, 100 persons (35.8%) were found to have ocular defects in one or both eyes. The incidence of ocular defects was higher than that of the normal populations. Among the ocular abnormalities, fundus defects occupied 28.6%, error of refraction, 17.9%. 17.9% of the eyes had the vision lower than 1.0 and the error of refraction was the main cause. Cases of hereditary syndromes were found: Usher syndrome, 2

[The development of occupational perceptual deafness in blacksmiths].

Analysis of hearing of 140 blacksmiths from three workships (280 ears) revealed considerable differences in the development of occupational perception deafness between different individuals and also as regard the affection of the right and left ear. The author worked out the decibel increment and average audiogram after 40 years exposure to noise after dividing the aers into five groups with regard to the affection on the main four frequencies. In the majority of ears the audiometric curve is not higher than 60 dB which is due to the numerical atrophy of the external ciliated cells. A drop of …

The cochlear nerve in various forms of deafness.

Operative specimens of cochlear nerve of 56 patients with profound deafness of varying origin were examined under the light microscope and the number of myelinated nerve fibers in each specimen was counted. The transverse fascicular area of each nerve was also determined. When the resulting neuron populations were evaluated with regard to the cochlear implant program the only truly unsuitable ears for artificial stimulation were found to be those with cholesteatoma involving the cochlear nerve and ears with local pathology in the internal auditory canal severe enough to cause atrophy of the co…
