Emergency surgical decompression of the orbit may rescue impaired vision even in a patient presenting late with a retrobulbar haematoma: A case report from rural southwestern Uganda.

Retrobulbar haematoma is a rare emergency that can potentially lead to blindness. Common causes include facial trauma and surgery. Timely surgical evacuation of the haematoma improves visual outcomes. In rural communities, patients often present to hospital after many hours and this increases the risk of poor visual outcomes. Radiological evaluation which is often not available in rural communities, results in further delay in surgical treatment. This case report highlights the need for urgent surgical intervention over radiological evaluation in patients with retrobulbar haematoma and orbital

Preoperative Frailty Predicts Worse Outcomes after Microvascular Decompression for Trigeminal Neuralgia, Hemifacial Spasm, and Glossopharyngeal Neuralgia: A Multicenter Analysis of 1,473 Patients from a Prospective Surgical Registry.

INTRODUCTION: Microvascular decompression (MVD) is an efficacious neurosurgical intervention for patients with medically intractable neurovascular compression syndromes. However, MVD may occasionally cause life-threatening or altering complications, particularly in patients unfit for surgical operations. Recent literature suggests a lack of association between chronological age and surgical outcomes for MVD. The Risk Analysis Index (RAI) is a validated frailty tool for surgical populations (both clinical and large database). The present study sought to evaluate the prognostic ability of frailt

The Influence of the Tissue Adhesive Material as a Surgical Wound-Closure Technique Following Carpal Tunnel Decompression on Neurological and Functional Outcomes: A Single-Center Randomized Controlled Trial.

Background Carpal tunnel syndrome (CTS) is caused by compression of the median nerve in the carpal tunnel. The effect of tissue adhesives as a material for wound closure following CTS decompression has been insufficiently investigated. This study aimed to evaluate outcomes by comparing two modalities of wound closure following carpal surgery in patients randomly assigned to either tissue adhesives or sutures. Methodology This randomized, prospective study was conducted in April 2022 at the University Hospital of Split in Croatia. Patients aged 61.56 ± 12.03 years were randomized to either tiss

An analysis of factors causing poor surgical outcome in patients with cervical myelopathy due to ossification of the posterior longitudinal ligament: anterior decompression with spinal fusion versus laminoplasty.

OBJECTIVE: We compared the surgical outcome of anterior decompression with spinal fusion (ASF) with the surgical outcome of laminoplasty for patients with cervical myelopathy due to ossification of the posterior longitudinal ligament. METHODS: The study group comprised 19 ASF patients (A-group) and 40 laminoplasty patients (P-group) treated from 1993 to 2002 with 1 year or longer follow-up. The Japanese Orthopedic Association scoring system was used to evaluate cervical myelopathy, and the recovery rate calculated 1 year after surgery. RESULTS: The mean recovery rate was 68.4% in the A-group

Refractory Lateral Leg Pain in a 25-Year-Old Competitive Runner: A Report of a Case of Fascial Herniation of the Peroneus Brevis With Complete Resolution After Surgical Decompression.

Fascial herniations of the leg occur when an intracompartmental leg muscle protrudes through weaknesses in the overlying fascial sheath. These fascial defects may be congenital or acquired from trauma involving penetrating injuries to the fascia. Increases in intracompartmental pressure, often resulting from muscular hypertrophy, can lead to muscular herniation through the weakened fascia. This may present as a leg mass which is often misdiagnosed as a hematoma, varicosity, or soft-tissue mass, leading to significant delays in treatment. We present a case of a peroneus brevis herniation in a 2

Surgical treatment of resistant tennis elbow. A prospective, randomised study comparing decompression of the posterior interosseous nerve and lengthening of the tendon of the extensor carpi radialis brevis muscle.

We compared decompression of the posterior interosseous nerve (PIN) and lengthening of the distal tendon of the extensor carpi radialis brevis (ECRB) for treatment of tennis elbow in a randomised trial of 28 patients. Fourteen underwent decompression of PIN and 14, lengthening of ERCB. The groups did not differ significantly with regard to age, sex and work activities. The average duration of preoperative symptoms was 23 months. The PIN was exposed in the groove between the brachioradialis and brachialis muscles and decompressed at the arcade of Frohse by means of a 1-2 cm incision through the

Which surgical technique has a superior clinical outcome in microvascular decompression? a systematic review and meta-analysis study of transposition versus interposition for trigeminal neuralgia and hemifacial spasm.

Microvascular decompression (MVD) remains the most effective treatment for managing trigeminal neuralgia, hemifacial spasm, and glossopharyngeal neuralgia. Despite variations in technique, the clinical outcomes of transposition and interposition for MVD have shown mixed results. Here, we use a meta-analysis approach to evaluate the clinical outcomes of transposition versus interposition techniques for trigeminal neuralgia and hemifacial spasm. We systematically retrieved articles published before August 31, 2024, from PubMed, the Cochrane Library Database, and Web of Science. Our analysis incl

Thirty-day outcomes for suboccipital decompression in adults with Chiari malformation type I: a frailty-driven perspective from the American College of Surgeons National Surgical Quality Improvement Program.

OBJECTIVE: When indicated, patients with symptomatic Chiari malformation type I (CM-I) may benefit from suboccipital decompression (SOD). Although SOD is considered a lower-risk neurosurgical procedure, preoperative risk assessment and careful surgical patient selection remain critical. The objectives of the present study were twofold: 1) describe 30-day SOD outcomes for CM patients with attention to the impact of preoperative frailty and 2) design a predictive model for the primary endpoint of nonhome discharge (NHD). METHODS: There were 1015 CM-I patients who underwent SOD in the 2011-2020

A more than 20-year follow-up of pain and disability after anterior cervical decompression and fusion surgery for degenerative disc disease and comparisons between two surgical techniques.

BACKGROUND: Follow-ups more than 20 years after neck surgery are extremely rare. No previous randomized studies have investigated differences in pain and disability more than 20 years after ACDF surgery using different techniques. The purpose of this study was to describe pain and functioning more than 20 years after anterior cervical decompression and fusion surgery, and to compare outcomes between the Cloward Procedure and the carbon fiber fusion cage (CIFC). METHODS: This study is a 20 to 24-year follow-up of a randomized controlled trial. Questionnaires were sent to 64 individuals, at lea

Muscle activity and rehabilitation in spinal stenosis (MARSS) after conservative therapy and surgical decompression with or without fusion: Protocol for a partially randomized patient preference trial on rehabilitation timing.

BACKGROUND: Patients affected by lumbar spinal stenosis (LSS) suffer from a multifactorial degeneration of the lumbar spine resulting in narrowing of the neuroforamina and spinal canal, leading to various functional limitations. It remains unclear whether LSS patients after surgery would benefit from early post-operative rehabilitation, or if a delayed rehabilitation would be more advantageous. The purpose of this partially randomized patient preference trial is to evaluate the impact of post-operative rehabilitation timing as well as surgical intervention type on psychometric properties and f

Comparison of the Efficacy of Surgical Decompression Alone and Combined With Canine Adipose Tissue-Derived Stem Cell Transplantation in Dogs With Acute Thoracolumbar Disk Disease and Spinal Cord Injury.

Paraparesis and paraplegia are common conditions in dogs, most often caused by a disc herniation in the thoracolumbar spinal segments (T3-L3), which is a neurological emergency. Surgical decompression should be performed as soon as possible when spinal compression is revealed by myelography, computed tomography, or magnetic resonance imaging. Mesenchymal stem-cell therapy is a promising adjunct treatment for spinal cord injury. This study sought to compare the effects of surgical decompression alone and combined with an allogeneic transplantation of canine adipose tissue-derived mesenchymal st

[Surgical treatment of unstable, therapy-resistant and painful lumbosacral joints by distracting decompressive posterolateral spondylodesis over the divided sacral bar according to Zielke's method (author's transl)].

Here the procedure of the distracting decompressive fusion over the divided sacral bar which Zielke developed 8 years ago is being described. This procedure was applied in over 150 cases. In a follow-up only one pseudarthrosis was found in the area of L5/S1. 70% of the previously pensioned patients could at least be partially rehabilitated. 32 patients were asked of their opinion with a questionnaire. Only 2 patients (6%) assessed the operation results with "bad". Indication is in all instabilities of the lumbosacral joint. The problem of the multi-operated back can be largely solved through t

What is the average time frame of clinically meaningful improvement in surgical decompression for cervical spondylotic myelopathy? A Spine CORe™ analysis of QOD data: 5-year follow-up.

OBJECTIVE: The objective of this study was to determine the time frame of clinical improvement in patient-reported outcomes (PROs) following surgical decompression for cervical spondylotic myelopathy (CSM). Based on previously published 12-month data from this group, the authors hypothesized that the average time to minimal clinically important difference (MCID) improvement would primarily occur by 3 months postoperatively regardless of preoperative myelopathy severity. They also hypothesized that there would be minimal additional improvement between 3 months and 5 years after surgery. METHOD

Comparison of 30-day surgical outcomes of multi-level sub-axial anterior (ACDF) vs posterior (PCDF) approach to cervical decompression and fusion in the setting of pre-operative malnutrition.

Evidence continues to mount supporting the detrimental effects malnutrition has on spine surgery outcomes. However, improving nutrition with cervical pathology faces challenges, such as progressive neurological decline with surgical delays, which may force intervention despite patient malnutrition. The American College of Surgeons National Surgical Quality Improvement Project (NSQIP) database was queried to identify adult patients who underwent multi-level sub-axial anterior cervical discectomy and fusion (ACDF) or posterior cervical decompression and fusion (PCDF) from 2015 to 2022 in the set
