Treatment of facial acne scars in Asian skin with the single-spot, 2940-nm Er:YAG dual-mode laser.

Modalities for the treatment of atrophic facial acne scars have been studied extensively. One, an erbium:yttrium-aluminum garnet (Er:YAG) laser device that generates both short, ablative pulses of high fluence and long, coagulative pulses of low fluence, has been shown to achieve tissue contraction, control intraoperative bleeding and deliver energy quickly and uniformly. The investigators were able to achieve significant depth and ablation with repetitive pulses at the same site and remove the epidermis with a single pass. Subsequent studies showed that facial acne scars of patients with dark

[Abdominal wall necrosis as expression of Munchausen's syndrome: a case report].

Münchausen's syndrome is a psychiatric disease with behaviour disorders including self-injuries. Self mutilations interest all the body, and among others the abdominal wall, who can mimic complications of surgery. We report the history of a woman consulting for an abdominal wall panniculitis two years after a caesarean. Medical history, histological patterns and the psychological history of the patient excluded the diagnosis of a surgical complication and Münchausen's syndrome was strongly evocated. The treatment was surgical, with excision of the cutaneous necrosis. The healing was obtained b

Skin wound closure with a novel shape-memory alloy fixator.

BACKGROUND: Previously, we have described novel, thermally deployable tissue fixators based on the shape-memory properties of nickel titanium. The present study reports on the in vivo wound closure with these fixators as a preliminary to evaluating their use for bowel anastomoses. METHODS: Twenty adult Sprague-Dawley rats had 2.0-cm dorsal skin incisions approximated either with shape-memory alloy (SMA) fixators or conventional skin staples on a random basis. Electrical resistance heating was used to transform and deploy the SMA fixators into the wound. The rats were killed on day 14 or day 2

The safe face lift with bony anatomic landmarks to elevate the SMAS.

The risk for facial nerve injury has been reported to be increased with the inclusion of superficial musculoaponeurotic system (SMAS) elevation as compared with a skin-only face lift. The facial nerve courses through the parotid gland. The SMAS is elevated superficial to the parotid gland. However, in elevating the SMAS anterior to the parotid gland, the facial nerve is at risk of injury where its branches emerge from the anterior edge of the parotid gland. The purpose of this study was to identify bony anatomic landmarks to predict the location of the anterior edge of the parotid gland to avo

The "iatrogenic-hanging columella": preserving columellar contour after tip retroprojection.

This article discusses a method for treating the ultraprojecting tip by the resection of columellar skin in open rhinoplasty. Lack of postoperative contraction of columellar skin and soft tissue may result in an "iatrogenic-hanging columella." Columellar skin resection frequently produces its own deformities because of a discrepancy in the width of the columellar base side and the infralobular flap side. The ultraprojecting tip was present in 56 of 660 consecutive rhinoplasty patients (8 percent) over 8 years (1991 to 1998). Of these 56 patients, 48 underwent partial resection of the infralobu

Multiple primary melanoma.

BACKGROUND: Incidence rates of cutaneous malignant melanoma (CMM) have been increasing for decades among Caucasian populations worldwide. Multiple factors identify persons at increased risk of CMM, including those with a family history of melanoma and those with atypical moles. Approximately 6-12% of melanomas are familial and approximately 12% of patients with familial melanoma have multiple primary melanomas. OBJECTIVE: To report a case of a patient with atypical moles and with 17 multiple primary melanomas. To review the literature on multiple primary melanomas as well as to review the gen

Eccentric skin resection and purse-string closure for skin reduction with mastectomy for gynecomastia.

Skin redundancy following mastectomy for gynecomastia does not shrink and resolve in a predictable manner. Excess skin can be addressed best with resection at the time of the original operation if the scar can be minimized. Previously described techniques for resection of skin have shortcomings that include unacceptable scars, poor nipple positioning, and bulkiness of redundant, folded dermal tissue. The current technique for grade 2 and some grade 3 gynecomastias is performed with the creation of a 20-mm-diameter nipple-areolar complex based as a cephalad flap and designed eccentrically aroun

Partial wound closure after surgical correction of equinovarus foot deformity.

Full correction of severe equinovarus foot deformities is frequently lost at the end of surgical release when the surgeon closes the skin incision. We retrospectively review 31 feet in 22 patients whose medial skin incisions were left open (typically 10 mm) to heal by secondary intention. The criterion to leave a wound open was if primary closure with the foot in full correction might compromise circulation to the skin or if closing the incision would require loss of corrected position. One or two cast changes were performed under outpatient anesthesia at 7- to 14-day intervals for wound care.

Repair of laparoscopic injury to abdominal wall arteries complicated by cutaneous necrosis.

Operative laparoscopic techniques requiring placement of large-bore cannulas through the abdominal wall lateral to the midline result in increased numbers of injuries to abdominal wall vessels. Five cases of inferior epigastric artery hemorrhage were controlled by percutaneous transabdominal placement of polypropylene sutures, allowing the procedure to be completed with the cannula in place. In two patients the sutures were left in place for 72 to 96 hours, and cutaneous necrosis occurred requiring debridement and delayed primary closure. In the other three women, removal of the sutures less t

A lateral skin incision reduces peripatellar dysaesthesia after knee surgery.

In a series of 64 ligamentous injuries of the knee, 33 were explored through a medial incision and 31 through a long lateral incision. Sensibility of the skin, the occurrence of neuromas, and discomfort during kneeling were evaluated three and six years postoperatively. The lateral incisions produced less dysaesthesia, fewer neuromas and less discomfort on kneeling.

Is There a Smoking Gun for Nicotine? A Review of the Role of Nicotine in Dermatologic Surgery.

BACKGROUND: Dermatologic surgeons are faced with a dilemma when counseling actively smoking patients who require dermatologic surgery: recommend total cessation of all nicotine that is associated with extremely high rates of cessation failure or recommend nicotine replacement therapy (NRT). OBJECTIVE: To determine the safety of NRT in dermatologic surgery. MATERIALS AND METHODS: PubMed was queried: [(nicotine OR electronic cigarettes) AND (flap OR wound healing)]. RESULTS: Smoking tobacco is detrimental to wound healing, supported by ample evidence (1A). Perioperative smoking cessation redu

Pressure reducing skin pie-crusting in extremity trauma: An in-vitro biomechanical study and human case series.

BACKGROUND: Multiple small relaxing skin incisions oriented parallel to the longitudinal axis (so-called "pie-crusting") near traumatic lacerations or surgical incisions in edematous tissue beds have been utilized to achieve primary closure when edema or skin loss would otherwise have made this difficult. Our study hopes to demonstrate (1) biomechanical evidence that pie-crusting decreases wound closure tension and (2) provide a case series with data showing clinical results. MATERIALS AND METHODS: This study is a biomechanical cadaveric study and retrospective small series cohort using 16 po

Nipple-sparing skin-reducing mastectomy with reconstruction for large ptotic breasts.

BACKGROUND: Breast reconstruction is routinely used to alleviate the psychological adverse effects of mastectomy. Nipple preservation further improves the cosmetic result, and causes less trauma on the body surface. Nipple-sparing mastectomy, however, comes with challenges, especially in the case of large, ptotic breasts to the degree that large-sized breasts have conventionally been a contraindication for nipple preservation. In this report, we describe a novel technique for nipple preservation in immediate reconstruction of large, ptotic breasts. METHODS: From 2013 to 2018, 24 patients (30

ADM-assisted prepectoral breast reconstruction and skin reduction mastectomy: Expanding the indications for subcutaneous reconstruction.

BACKGROUNDS: Recently a flood of new techniques and studies have been released for prepectoral reconstruction, following strict criteria for selection of patients undergoing skin or nipple sparing mastectomy. Nevertheless a big population of patients that would otherwise benefit from prepectoral techniques has been excluded so far. PATIENTS AND METHODS: Between January 2016 and February 2017, patients undergoing skin reducing mastectomy (SRM) were enrolled at our Institution. We selected patients with large and ptotic (grade II-III) breasts undergoing SRM, followed by a new technique, which i

Facial Nerve Injury and Repair: A Practical Review for Cutaneous Surgery.

BACKGROUND: The facial nerve and its branches are at risk of injury during dermatologic surgery. Few publications in the dermatologic literature discuss facial nerve injury and management. OBJECTIVE: To review facial nerve injury and management, including static and dynamic repair techniques, and to review outcomes in facial nerve reconstruction. METHODS: Two detailed literature reviews were performed using PubMed. First, articles reporting facial nerve injury and/or management in the dermatologic literature were identified. In addition, articles pertaining to outcomes in facial nerve recons

[Spiradenocarcinoma of the forehead : A rare skin tumor in the head and neck region].

Spiradenocarcinomas are rare malignant tumors that originate from the sweat glands of the skin and demonstrate aggressive growth. We report the case of an 86-year-old female patient presenting with a growth on the forehead which had been apparent for 2 years. After surgical removal of the tumor, histological workup culminated in the diagnosis of a spiradenocarcinoma. Surgical margins were free of tumor on pathological examination. Metastasis was excluded by positron-emission tomography-computed tomography (PET-CT). Due to the advanced age of the patient and the absence of metastatic disease, n

Esthetic and functional reconstruction after parotidectomy in pediatric patients - A case series.

Parotidectomy is a mainstay of treatment for benign and malignant parotid lesions in children and adults. Depending on surgical methods used and tumor size, parotidectomy may result in significant facial disfigurement as well as functional challenges. We describe a series of four pediatric patients, ages 13-16 who presented to our clinic with a parotid mass. All patients underwent parotidectomy with immediate reconstruction by local tissue rearrangement or free fat graft. Esthetic and functional reconstruction after parotidectomy is not well described in pediatric otolaryngology literature. A

Multifocal cutaneous alternariosis in a 70-year-old Kenyan renal transplant patient.

Alternaria species are a group of dematiaceous fungi that are ubiquitous in nature and are becoming an increasingly important cause of disease in immunocompromised patients. We present a case of a 70 year old renal transplant recipient with multiple areas of cutaneous Alternaria infections likely introduced during local trauma. Treatment has required a combination of systemic therapy and surgical excision. This case illustrates the importance of recognizing fungal infections with cutaneous manifestations, such as alternariosis, in immunosuppressed patients.
