Rituximab desensitization in three patients with severe rituximab allergy.

Rituximab is a chimeric monoclonal antibody that targets CD20 positive B cells and has a positive effect on both overall and progression-free survival in B-cell lymphoid malignancies. Combination rituximab with chemotherapy treatment provide survival improvement. Although rituximab is an important treatment option in hematological malignancies, the risk of allergic reactions is high. These reactions are usually IgE-mediated and can be varied in regard of severity from urticaria to anaphylaxis. It is an option to interrupt the treatment and ommit rituximab therapy who had allergic reactions. Dr

Allergic syndrome in chronic cholecystitis and colitis, the efficiency of specific hyposensitivity with microbic allergen.

In more than 1000 patients with noncalculous cholecystitis, chronic colitis (nonulcerative, nonspecific ulcerative, Crohn's disease of the bowel) and with dyskinesia of intestine the participation of allergic components was investigated in the development and chronic duration of the diseases. More often was observed the sensibilization to bacteria of conditionally pathogenetic microflora (E. coli, staphylococcus, streptococcus, proteus) that was confirmed by positive skin tests, sensibilization of lymphocytes to microbic allergens, increase of the titer of microbic antibodies in blood. The sen

Probiotics in allergic disease: from adjunct supplement to immune-modifying strategy (2026 update).

PURPOSE OF REVIEW: Allergic diseases continue to increase globally, and accumulating evidence implicates early-life microbial exposures as central determinants of immune tolerance. This review synthesizes advances from 2024 to 2026 regarding probiotic-mediated immune modulation and their translational implications in allergy prevention and therapy. RECENT FINDINGS: Recent studies confirm strain-specific expansion of Foxp3+ regulatory T cells, suppression of Th2 polarization, reinforcement of epithelial barrier integrity, and durable epigenetic stabilization mediated by short-chain fatty acids

Vitamin D status and supplementation as potential modifiers of allergen immunotherapy outcomes in respiratory allergy: A structured narrative review.

Allergen immunotherapy (AIT) represents a cornerstone in managing IgE-mediated allergic diseases, such as allergic rhinitis and asthma. By administering controlled doses of allergens over time, AIT aims to readjust immune tolerance, thereby modifying allergic responses. However, clinical responses to AIT remain heterogeneous, with a subset of patients achieving only partial benefit. This heterogeneity underlines the need for novel adjunctive approaches to potentiate the immunomodulatory effects of AIT. In this context, vitamin D has gained considerable interest, given its well-established role

Military Standards for Anaphylaxis: Aligning Allergy Science, Operational Risk, and Waiver Documentation.

PURPOSE OF THE REVIEW: Allergic disease is common in the United States, and a growing number of otherwise qualified applicants and service members present with histories of food allergy, venom allergy, medication reactions, physical urticarias, or prior anaphylaxis. The challenge for clinicians is to translate a nuanced allergy-immunology evaluation into strict military accession and retention standards designed for readiness, austere care, and deployability, rather than for diagnosis alone. RECENT FINDINGS: The current accession standard disqualifies a history of anaphylaxis other than to a

Tolerance induction for food allergy using immunoglobulin/histamine complex: A case report.

Food allergy is defined as an immune-mediated adverse reaction to dietary proteins and is a major health problem with an increasing prevalence. Immunoglobulin/histamine complex (IHC) is an anti-allergy therapeutic that acts primarily by reducing histamine levels (histaminopexy). A 5-year-old male Korean patient with a food allergy was transferred for immunotherapy. Open food challenge (OFC) for wheat before treatment revealed positive reactions after challenge with 0.5g of wheat. Under a diagnosis of food allergy for wheat, allergic rhinitis, and atopic dermatitis, treatment with HistobulinTM

Omalizumab: First Approved Anti-IgE Therapy for Multiple Food Allergies.

BACKGROUND: Approximately 10% of adults and 7.6% of children in the United States have food allergies. Allergic reactions to foods can be life threatening, which requires patients and caregivers to be constantly diligent in avoiding foods that may trigger an immune-mediated event. Until recently, the only FDA-approved treatment for food allergy was oral immunotherapy (OIT), which involves daily ingestion of small doses of antigen to gradually build up a patient's tolerance. This therapy can be burdensome, requiring multiple office visits, and it is not without serious risk, including anaphylax

[Allergic rhinoconjunctivitis].

Allergic rhinoconjunctivitis (AR) is an increasingly prevalent disease with considerable impact on daily activities, quality of life and the course of atopic comorbidities. In this review, we argue that the diagnosis should be based on detailed history and evidence of IgE-sensitisation. The severity can be assessed using a visual analogue scale (VAS), where scores less-than 5 indicate mild and ≥ 5 indicate moderate-to-severe disease, and treatment should be tailored accordingly. Intranasal corticosteroids remain first-line in moderate-to-severe AR, while allergen immunotherapy is the only dise

Practical guideline for the management of allergic rhinitis in Japan 2024.

The Practical Guideline for the Management of Allergic Rhinitis in Japan was first published in 1993. After the COVID-19 pandemic, the current 10th edition was published in 2024. The most recent collection of evidence from the literature, such as the sustained post-treatment effect of sublingual immunotherapy on Japanese cedar pollinosis, was added to the revised guideline, which incorporates evidence-based medicine. In this revised guideline, a diagram illustrating the pathogenesis of allergic rhinitis and the mechanisms of action of various pharmacological treatments has been added. Also inc

Moving towards asthma remission from mechanistic understanding to clinical practice.

PURPOSE OF REVIEW: In the past, treatment of asthma aimed primarily at "control" of symptoms and a reduction of future risk. Recently, however, the concept of remission has been introduced which is broader than control and includes the concept of disease modification by using disease-modifying antiasthmatic drugs (DMAADs) that exclude the use of systemic corticosteroids. This review highlights the definitions as well as the clinical implications of the concept of remission in asthma. RECENT FINDINGS: While in the past remission has been used mainly for the relatively rare event of spontaneous

Inulin-gel-based oral immunotherapy remodels the small intestinal microbiome and suppresses food allergy.

Despite the potential of oral immunotherapy against food allergy, adverse reactions and loss of desensitization hinder its clinical uptake. Dysbiosis of the gut microbiota is implicated in the increasing prevalence of food allergy, which will need to be regulated to enable for an effective oral immunotherapy against food allergy. Here we report an inulin gel formulated with an allergen that normalizes the dysregulated ileal microbiota and metabolites in allergic mice, establishes allergen-specific oral tolerance and achieves robust oral immunotherapy efficacy with sustained unresponsiveness in

Allergen Immunotherapy: The Evidence Supporting the Efficacy and Safety of Subcutaneous Immunotherapy and Sublingual Forms of Immunotherapy for Allergic Rhinitis/Conjunctivitis and Asthma.

Allergen immunotherapy (AIT) is a recognized key therapeutic modality for the treatment of allergic respiratory disease. Definitive studies have provided evidence-based data to demonstrate its effectiveness in allergic rhinitis and asthma due to the inhalation of proteinaceous allergic substances from specific seasonal pollens, dust mites, animal allergens, and certain mold spores. Over the ensuing decades, laboratory investigations have provided objective evidence to demonstrate immunologic changes, including production of protective IgG antibody, suppression of IgE antibody, upregulation of

Investigating Forkhead Box O Transcription Factor 1 Gene's Relation to Immunoglobulin E in House Dust Mite-Allergic Asthma Patients.

House dust mite (HDM)-allergic asthma is an abnormal immune response to extrinsic aeroallergens found in human vicinities. Studying the role of the associated immunity biomarkers and their interplay helps in discovering novel therapeutic strategies that can be used in adjunct with effective long-term immunotherapy. This study investigates the total serum IgE, FoxO1, and Sirtuin 1 (SIRT1) gene expressions in HDM-allergic asthma patients. We enrolled 40 patients for each of the following three groups: an HV group of healthy volunteers and HDM/AA and HDM/SCIT groups of HDM-allergic asthma patient

Oral Immunotherapy and Risk of Eosinophilic Esophagitis in Children: 15 Years' Experience.

OBJECTIVES: Oral immunotherapy (OIT) is an effective treatment for children with persistent food allergy, and has concerns about its safety, including eosinophilic esophagitis (EoE). The aim of this study was to evaluate the prevalence of EoE in a large cohort of children who underwent OIT in our center, and to determine if there were any clinical, endoscopic, or histologic differences depending on the food employed for the OIT. METHODS: A retrospective study was performed over a 15-year period (2005-2020). Children who underwent cow's milk (CM), egg, and/or peanut OIT and developed EoE were

A case of successful acyclovir desensitization in a bone marrow transplant patient.

INTRODUCTION: The incidence of acyclovir-induced hypersensitivity is rare. To our knowledge, there are four published case reports of oral acyclovir desensitization in adults. Evidence-based guidelines prompt the use of acyclovir for herpes simplex virus (HSV) prophylaxis and treatment. Literature on the cross-reactivity of structurally similar antiviral agents is conflicting, presenting a clinical challenge when choosing an alternative agent. This is a case of successful acyclovir desensitization in an allogeneic stem cell transplant patient. CASE REPORT: A 69-year-old female patient, diagno

Peanut Oral Immunotherapy: State of the Art.

Cumulative evidence shows that peanut oral immunotherapy (OIT) is effective at inducing desensitization through downregulation of effector pathways in the allergic reaction cascade; however, only a subset of patients achieve sustained unresponsiveness (remission), which requires redirection of the underlying allergic response toward tolerance. A recent meta-analysis of peanut OIT randomized trials found that OIT is associated with a threefold greater risk of anaphylaxis and twofold greater risk of epinephrine use than allergen avoidance. Strategies to reduce adverse events associated with OIT
