Ischemic bowel disease characterized by colorectal mucosal necrosis induced by cardiac arrest.

The patient was a 60-year-old male. He developed impaired consciousness after drinking alcohol more than 20 days ago and was found 12 hours later to have an altered level of consciousness with vomiting. The vomit was brown in color. Cardiopulmonary resuscitation (CPR) was performed immediately after calling the emergency number (120) and finding the patient in respiratory and cardiac arrest. After 20 minutes of resuscitation, the patient regained supraventricular rhythm but remained in a deep coma. He was transported to the hospital for treatment and was admitted to the intensive care unit (IC

Effects of arterial or portal ischemia on survival and metabolism of partially and totally depancreatized dogs.

The role of arterial blood flow in hepatic metabolic functions was compared to that of portal flow in two groups of totally depancreatized dogs. Survival times and glucose and nitrogen excretion were significantly greater in dogs with ligation of the hepatic artery than in dogs with an Eck fistula. The dogs with ligated hepatic arteries also showed a significantly slower rise in plasma ketones. The course of diabetes was compared in three additional groups of partially depancreatized dogs consisting of a) dogs with ligated hepatic arteries, b) dogs with Eck fistulas, and c) controls. Hepatic a

Impact of Scleral Lens Front Surface Eccentricity on Visual Acuity, Contrast Sensitivity, and Higher-Order Aberrations in Eyes With Keratoconus.

PURPOSE: To investigate the effect of front surface eccentricity (FSE) of scleral lenses (SLs) on corrected distance visual acuity (CDVA), contrast sensitivity (CS), and higher-order aberrations (HOAs) in keratoconic eyes. METHODS: Fifteen keratoconic eyes fitted with BostonSight SCLERAL lenses of different eccentricity values (FSE0=0, FSE1=0.30, and FSE2=0.60). CDVA, CS, and HOAs were measured at baseline and after 30 min of SL wear. CDVA and CS were measured with the Early Treatment Diabetic Retinopathy Study and Pelli-Robson chart, respectively, and iTrace aberrometer was used to evaluate

Hyperglycemia is associated with increased risk of morbidity and mortality after colectomy for cancer.

BACKGROUND: The relationship of hyperglycemia to general surgery outcomes is not well-understood. We studied the association of operative day and postoperative day 1 (POD1) blood glucose (BG) with outcomes after open colectomy for cancer. STUDY DESIGN: We retrospectively analyzed the 2000-2005 Veterans Affairs Surgical Quality Improvement Program database, linked with Veterans Affairs Decision Support System BG values. Median BG was categorized as hypoglycemic (<80 mg/dL); normoglycemic (BG 80-120 mg/dL); or mildly (BG 121-160 mg/dL), moderately (BG 161-200 mg/dL), or severely (BG >200 mg/dL)

From Mild Gait Difficulties to a Sudden Coma: A Rare Case of Marchiafava-Bignami Disease.

In this case report we describe the case of a 66-year old man with subacute gait difficulties, with a progression to confusion coma with multiple generalised epileptic seizures during the following days. Biochemical analysis showed hyperglycaemia, cerebrospinal fluid (CSF) testing showed a mild lymphocytic pleocytosis and an elevated protein and lactate. Broad-spectrum antibiotics and antiviral therapy where initiated. However, all other CSF testing remained negative. Magnetic resonance imaging of the brain showed remarkably symmetric hyperintense T2 white matter lesions most noticable in the

Tolosa-Hunt Syndrome: A Case Report.

Tolosa-Hunt syndrome (THS) is a rare neurological condition characterized by ophthalmoplegia preceded by retroorbital pain or headache. It mainly involves cranial nerves III, IV, and VI. The majority of the time, the cause is unknown; it is believed that the cause is nonspecific granulomatous inflammation of the cavernous sinus and superior orbital fissure or rarely beyond. THS is an exclusionary diagnosis. It is diagnosed clinically according to the patient's symptoms, neuroimaging test results, and response to corticosteroid therapy. The mainstay of treatment is corticosteroid therapy. A 62-

N-3 PUFA and Pregnancy Preserve C-Peptide in Women with Type 1 Diabetes Mellitus.

Type 1 diabetes (T1DM) is an autoimmune disease characterized by the gradual loss of β-cell function and insulin secretion. In pregnant women with T1DM, endogenous insulin production is absent or minimal, and exogenous insulin is required to control glycemia and prevent ketoacidosis. During pregnancy, there is a partial decrease in the activity of the immune system, and there is a suppression of autoimmune diseases. These changes in pregnant women with T1DM are reflected by Langerhans islet enlargement and improved function compared to pre-pregnancy conditions. N-3 polyunsaturated fatty acids

[Research Advances in Hypothalamic-pituitary Dysfunction Related to Traumatic Brain Injury].

Traumatic brain injury(TBI)is a major cause of hypothalamopituitary dysfunction. TBI-related hypothalamopituitary dysfunction is more common in the acute phase. Disturbance of pituitary/gonadal axis and growth hormone axis,as well as posterior pituitary dysfunction including central diabetes insipidus and syndrome of inappropriate antidiuretic hormone secretion, are the most frequently seen. During the chronic phase of TBI,anterior hypopituitarism is the major concern, which affects the quality of life. Risk factors for hypothalamopituitary dysfunction following TBI include low Glasgow score, hig

A standard set of person-centred outcomes for diabetes mellitus: results of an international and unified approach.

AIMS: To select a core list of standard outcomes for diabetes to be routinely applied internationally, including patient-reported outcomes. METHODS: We conducted a structured systematic review of outcome measures, focusing on adults with either type 1 or type 2 diabetes. This process was followed by a consensus-driven modified Delphi panel, including a multidisciplinary group of academics, health professionals and people with diabetes. External feedback to validate the set of outcome measures was sought from people with diabetes and health professionals. RESULTS: The panel identified an esse

Salty, Sweet and Difficult to Treat: A Case of Profound Hypernatremia in the Setting of Hyperosmotic Hyperglycemic State.

Hyperosmolar hyperglycemic state (HHS) is a disorder that occurs most frequently in type 2 diabetics and is associated with high mortality - up to 50%. Hypernatremia, when associated with HHS, worsens the prognosis. Encephalopathy is evident at a serum sodium level greater than 160 mOsm/kg. Additional symptoms include lethargy, weakness, seizures, and coma. Rhabdomyolysis can rarely occur in hyperosmolar states. Here we describe a case of severe hypernatremia in the setting of HHS leading to profound encephalopathy and report to the best of our knowledge the highest serum sodium level publishe

Candida albicans brain abscesses in an injection drug user patient: a case report.

BACKGROUND: Fungal brain abscess is an uncommon disease, mostly associated with immunocompromised states and poorly controlled diabetes. Its incidence, however, is rising as a result of the increasing use of immunosuppressive agents, corticosteroids and broad-spectrum antimicrobial therapy. Candida species have emerged as the most prevalent etiologic agents of brain abscesses in autopsy studies. CASE PRESENTATION: A 46-year-old male with a history of injection drug abuse, chronic hepatitis C and diabetes mellitus presented to the Emergency Department of our hospital following a generalized to

Initial predictive factors of outcome in severe non-accidental head trauma in children.

OBJECT: The aim of this study is to evaluate the outcome of young children hospitalized for non-accidental head trauma in our PICU, to evaluate PRISM II score in this sub-population of pediatric trauma and to identify factors that might influence the short-term outcome. MATERIALS AND METHODS: Files of all children less than 2 years old with the diagnosis of non-accidental head trauma over a 10-years period were systematically reviewed. We collected data on demographic information, medical history, clinical status, and management in the PICU. Three severity scores were then calculated: PRISM I

[Pancreatic function and quality of life after resection of the head of the pancreas in chronic pancreatitis. A prospective, randomized comparative study after duodenum preserving resection of the head of the pancreas versus Whipple's operation].

Given an indication for surgery in patients with chronic pancreatitis, such as distal common bile duct obstruction, duodenal stenosis, or dilated pancreatic duct with stones and congestion, the surgeon must decide the type of operation to perform. A duodenopancreatectomy, the Whipple procedure, is widely considered to be the gold standard. It is highly effective in relieving pain and eliminating the structural abnormalities noted above. Duodenum-preserving resection of the head of the pancreas (DPRHP) seems to be an attractive alternative to pancreaticoduodenectomy (PD) in the treatment of chr

Hematoma expansion in intracerebral hemorrhage: Development of a preoperative risk prediction model.

Intracerebral hemorrhage (ICH) is one of the most lethal and disabling forms of acute cerebrovascular disease. Hematoma expansion is a critical predictor of early deterioration and poor prognosis in ICH patients, making accurate preoperative risk prediction essential for clinical decision-making. This study aims to develop a preoperative risk prediction model for hematoma expansion in ICH patients to provide robust decision support for clinicians. A total of 156 patients with spontaneous ICH admitted to our hospital between January 2022 and July 2024 were included. The cohort was randomly divi

Anesthetic Management of a Diabetic Patient With a Suspected Hyperosmolar Hyperglycemic State: A Case Report.

The hyperosmolar hyperglycemic state (HHS) is an acute metabolic complication of diabetes mellitus type 2 (DMT2) and a life-threatening emergency. A 76-year-old man with DMT2 that was well controlled with oral medications underwent oral cancer surgery. Postoperatively, a surgical site infection and aspiration pneumonia developed, exacerbating the patient's diabetes. His blood glucose level increased to >500 mg/dL, and both dehydration and hyperosmolality were observed. His effective plasma osmolality exceeded 300 mOsm/kg and corrected serum sodium level was 159 mEq/L, which were suggestive of
