Atrophy of Diaphragm and Pectoral Muscles in Critically Ill Patients.

WHAT WE ALREADY KNOW ABOUT THIS TOPIC: Muscle atrophy is common in the critically ill, and diaphragm atrophy occurs during mechanical ventilation. It is not known whether wasting of diaphragm and nondiaphragm muscle is related. WHAT THIS ARTICLE TELLS US THAT IS NEW: Ultrasound was used for serial assessment of diaphragm and pectoral muscle in 97 critically ill patients. Diaphragm and pectoral atrophy occurred in 48% and 29%, respectively, and was associated with septic shock (diaphragm) and steroid use (pectoral); atrophy of the two muscle types appears unrelated. BACKGROUND: Muscle atrophy…

Mucoepidermoid Carcinoma of Arising from a Bronchogenic Cyst of the Diaphragm.

INTRODUCTION: Bronchogenic cysts may rupture or become infected, and malignant degeneration may occur. Although various types of malignant degeneration have been described, only a few reports of mucoepidermoid carcinoma arising from a bronchogenic cyst have been published. We report such a case. CASE: A 77-year-old female was referred to our institution for evaluation of left chest pain. A computed tomography scan showed an enhancing 65 × 70 mm mass of the left diaphragm. Based on the intraoperative findings of an intradiaphragmatic tumor involving the lower lobe of the left lung, the resecti…

Obesity modulates diaphragm curvature in subjects with and without COPD.

Obesity is a common comorbidity of chronic obstructive pulmonary disease (COPD) and has been associated with worse outcomes. However, it is unknown whether the interaction between obesity and COPD modulates diaphragm shape and consequently its function. The body mass index (BMI) has been used as a correlate of obesity. We tested the hypothesis that the shape of the diaphragm muscle and size of the ring of its insertion in non-COPD and COPD subjects are modulated by BMI. We recruited 48 COPD patients with postbronchiodilator forced expiratory volume in 1 s (FEV1)-to-forced vital capacity (FVC) …

Anatomical and functional deficiencies of the crural diaphragm in patients with esophagitis.

BACKGROUND: Inspiratory esophagogastric junction (EGJ) pressure is lower in gastroesophageal reflux disease (GERD) and patients fail to increase EGJ pressure during the inspiratory effort. The aim of this study was to assess the EGJ activity during inspiratory maneuvers (high-resolution manometry, HRM) and the crural diaphragm (CD) thickness (endoscopic ultrasound, EUS) in GERD. METHODS: Twenty esophagitis patients (average age 45 years, 7 grade A, 13 grade B) had HRM and EUS. Forty-three controls were recruited; 30 had HRM (average age 33 years), and 13 had EUS (average age 40 years). The EG…

Technique for implanting intramuscular electrodes in the diaphragm by videolaparoscopy in pigs.

PURPOSE: To describe a novel approach for implanting intramuscular electrodes in the diaphragm through videolaparoscopy. METHODS: We used twelve pigs for this videolaparoscopic technique, which permits at the same time to explore the diaphragm, to locate its motor points and to fix the electrodes in the diaphragm bilaterally. In this technique we used three trocars: one portal for a 10-mm 0° viewing angle laparoscope, one portal for the manipulation of structures and another for electrode implantation. RESULTS: All animals survived the procedure without pneumothorax/capnothorax or other comp…

Altered activation of the diaphragm in late-onset Pompe disease.

Pompe disease is an inherited neuromuscular disorder that affects respiratory function and leads to dependence on external ventilatory support. We studied the activation of the diaphragm using bilateral phrenic magnetic stimulation and hypothesized that diaphragm compound muscle action potential (CMAP) amplitude and evoked transdiaphragmatic pressure (Twitch PDI) would correlate to disease severity. Eight patients with late onset Pompe disease (LOPD, aged 14-48 years) and four healthy control subjects completed the tests. Maximal Twitch PDI responses were progressively reduced in patients with…

Diaphragm Appearance: A Clue to the Diagnosis of Pulmonary and Extrapulmonary Pathology.

The diaphragm is often overlooked or incompletely evaluated when interpreting chest radiographs. Alterations in the appearance of the diaphragm on chest radiographs such as elevation, contour abnormalities, adjacent lucency, and calcifications can offer clues to pulmonary and extrapulmonary pathology. Familiarity with common causes of these alterations facilitates the appropriate selection of additional imaging and leads to the diagnosis of both benign and life-threatening processes. This article reviews normal variations in the appearance of the diaphragm as well as those associated with pulm…

Expansion of the neck reconstituted the shoulder-diaphragm in amniote evolution.

The neck acquired flexibility through modifications of the head-trunk interface in vertebrate evolution. Although developmental programs for … rtle embryos. In mammals, this infolding process is particularly important because it separates the diaphragm from the shoulder muscle mass. In turtles, the expansion of the cervical lateral body wall affects morphogenesis of the shoulder. Our findings highlight the cellular expansion in developing amniote necks that incidentally brought about the novel ada …

Diaphragm disease of the small intestine: an interesting case report.

Diaphragm disease of small intestine usually presents with nonspecific clinical features. Radiological investigations often fail to differentiate it from small intestinal tumors and inflammatory bowel disease. It is therefore diagnosed on final histology after surgical resection. We hereby report an interesting case of a suspected small bowel tumor later diagnosed as diaphragm disease on histology.

Duplicated right crus of the diaphragm: a cadaveric case report.

The lumbar part of the diaphragm arises from the lumbar vertebrae by right and left crura. The duplication of crura of the diaphragm is rarely reported in the past. During regular dissection classes to the medical students, we came across a case of duplicated right crus of the diaphragm. The right crus of the diaphragm was duplicated completely and presented two separate crura; medial right crus & lateral right crus. The medial right crus was attached to the anterolateral surfaces of the superior three lumbar vertebral bodies and intervertebral discs and merged with the anterior longitudinal l…

Development of the diaphragm -- a skeletal muscle essential for mammalian respiration.

The mammalian diaphragm muscle is essential for respiration, and thus is one of the most critical skeletal muscles in the human body. Defects in diaphragm development leading to congenital diaphragmatic hernias (CDH) are common birth defects and result in severe morbidity or mortality. Given its functional importance and the frequency of congenital defects, an understanding of diaphragm development, both normally and during herniation, is important. We review current knowledge of the embryological origins of the diaphragm, diaphragm development and morphogenesis, as well as the genetic and dev…

Is there a case for diaphragm pacing for amyotrophic lateral sclerosis patients?

Respiratory pacing has advanced the long-term management of respiratory failure secondary to neurological disorders. It has an established role in curtailing invasive mechanical ventilation after upper motor neuron lesions such as spinal cord injury. There is increasing interest to expand the application of intramuscular diaphragm pacing to amyotrophic lateral sclerosis (ALS), a progressive and fatal neurodegenerative disease. Although diaphragm pacing has been offered to ALS patients, evidence-based data to determine its benefits remain lacking. The limited current literature indicates progre…

A review of patch options in the repair of congenital diaphragm defects.

Repair of congenital diaphragmatic hernia (CDH) continues to pose a dilemma for the pediatric surgeon. While the management of CDH has evolved from surgical urgency to delayed repair after medical optimization with substantial improvements in mortality, morbidity continues to perplex clinicians. Significant morbidity occurs with recurrence, re-recurrence and complications such as obstructions, principally with mesh repair. When primary closure is not possible, mesh repair is indicated. While there are several non-absorbable prosthetic, absorbable biosynthetic and composite mesh types available…

The importance of lacrimal diaphragm and periosteum suturation in external dacryocystorhinostomy.

PURPOSE: To evaluate retrospectively the cosmetic and functional effects on success of lacrimal diaphragm and periosteum suturation after the anastomosis of anterior flaps before skin closure in external dacryocystorhinostomy (ext-DCR). METHODS: During ext-DCR, lacrimal diaphragm was separated from periosteum over anterior lacrimal crest before entering lacrimal fossa. At the end of ext-DCR, the separated periosteum and lacrimal diaphragm were sutured in the study group (270 patients) and were left unsutured in the age-matched control group (96 patients). The effects of this approach on posto…

Multimodality imaging of the pediatric diaphragm: anatomy and pathologic conditions.

Apart from serving as an important landmark for description and staging of pathologic conditions, the diaphragm is also affected by various types of pathologic conditions in children. Congenital abnormalities affecting the diaphragm include aplasia or hypoplasia, accessory diaphragm, eventration, and hernias. Congenital diaphragmatic hernias (CDHs) include Bochdalek, Morgagni, and hiatal hernias. Although survival rates are improving with the advent of new therapies, there is still significant morbidity and mortality associated with CDH. The morbidity and mortality depend on the associated con…

The pattern of the thoracic splanchnic nerves as they pass through the diaphragm.

The formation and structure of the greater, lesser, and least thoracic splanchnic nerves is highly variable in their intrathoracic as well as their subdiaphragmatic portion. Splanchnicectomies for pain control of otherwise intractable upper abdominal pain and other surgical procedures are dependent on the detailed knowledge of the anatomy of these nerves and their variations. Many commonly used anatomical illustrations depict the passage of the thoracic splanchnic nerves through the diaphragm uniformly as three nerves penetrating the crura in three separate locations along a rough superoinferi…
