We used an Applied Alexis (Applied Medical Resources Corp, USA) wound protector and retractor (WPAR) instead of a "spring loaded silo" (SLS) for gastroschisis reduction because SLS is currently unavailable in Japan. Our patient was a 1,707 g female diagnosed prenatally with gastroschisis born at 35 weeks gestation by spontaneous vaginal delivery. Attempted primary closure failed and a WPAR was used as a silo. This prospective study was designed to assess the safety, cost-effectiveness, and advantages of performing posterior sagittal anorectoplasty (PSARP) without colostomy on males with intermediate imperforate anus in a developing country. Fifteen consecutive males with intermediate imperforate anus were entered into the study. Chest and abdominal x-rays, skeletal surveys, renal ultrasound scans, and invertograms were done. UNLABELLED: We report on a case of childhood coeliac disease presenting with tricophagia and trichobezoar. The combination of obstructive symptoms, severe hypoalbuminaemia and a large abdominal mass detected on CT scan warranted diagnostic gastroscopy and laparotomy, resulting in removal of a large gastric trichobezoar. Surgical recovery was uneventful although serologic studies for coeliac disease were abnormal. Coeliac disease was confirmed by subsequent biopsy.

Hirschprung's disease is a rare condition in the adult. We present three patients with Hirschsprung's disease who successfully underwent the modified Duhamel procedure. Anorectal abscess and fistula are intimately connected in anatomy, etiology, and morbidity. They have been extensively described, but only sparingly studied in randomized controlled trials. These trials, covering such issues as primary suture of abscess, whether or not to perform a fistulotomy at the time of abscess drainage, and comparison of fistulotomy and fistulectomy, are the focus of this article. Important gaps in our knowledge and subjects for new trials are also discussed.
