A 61-year-old female complaining of arthralgia was repeatedly treated with antibiotics and also with prednisolone. A chronic polyarthritis was assumed. In hospital, leukocytosis of 21.000 was found one day before death as well as moderate anemia. Colonoscopy was rejected by the patient. A computer tomography revealed destructive arthritis of the symphysis, vertebral osteochondrosis L5/S1, and sigmoid diverticulosis. The patient died with clinical signs of central dysregulation. At autopsy, a covered perforation of a sigmoid diverticulum with purulent peridiverticulitis was found. The 5th lumba…

This prospective study focuses on the prediction of late outcome after acute left colonic diverticulitis successfully treated conservatively and in which the diagnosis was confirmed radiologically (computed tomography [CT] and gastrografin enema [GE]). Acute diverticulitis was diagnosed in 226 patients. Sixty-six patients (29%) were operated on during their first hospitalization, and 2 of them died (3% mortality). The remaining 160 patients, successfully treated conservatively, had a CT and a GE within 72 hours of admission and entered this study. Signs of severity on CT included the presence …

A total of 31 patients with diverticulitis were analyzed who had both computed tomography and contrast enema. There was almost equal sensitivity to abnormality of approximately 90 percent. Contrast enema produced a specific diagnosis of diverticulitis in 61 percent, using stringent positive criteria, and an additional 29 percent with suggestive findings. Comparative computed tomography specific diagnoses in those 31 cases was made in 65 percent, and suggestive in 23 percent. Computed tomography was particularly useful diagnostically in cases of retrograde obstruction on contrast enema. The aut…

The clinical course of sigmoidvesical fistula due to rupture of colonic diverticulitis is described. The patient, a 72-year-old-man, had chief complaints of cloudy urine, pneumaturia and fecaluria. The fistula was diagnosed by cystoscopy. In this case, primary resection of the sigmoid colon and wedge resection of the bladder including fistula were performed, and the sigmoidvesical fistula was completely cured. Statistical analysis was made on 43 cases of colovesical fistula due to diverticulitis in Japan including our own case.

Diverticulitis of the ascending colon is an uncommon disease which mimics appendicitis. The correct diagnosis is rarely made, but can be suggested by the patterns of signs and symptoms and confirmed by barium contrast study. Diverticulitis of the ascending colon should be treated by the same plan as diverticulitis of the left colon. If the diagnosis is established, nonoperative management is indicated initially. Operation is indicated when the diagnosis is in doubt, when perforation has occurred, or when the patient does not respond to nonoperative treatment. At operation, ascending colon dive…

In a series of 205 patients treated for colonic diverticulitis, 101 presented acute complications, 35 chronic complications and 69 attacks of diverticulitis without complications. The treatment prescribed for the acute septic complications (generalized peritonitis, perisigmoidic abscess) as well as for acute occlusions consists of two-stage surgery: drainage and emergency colostomy, then secondary colectomy in elective conditions. Massive incoercible hemorrhages were treated by emergency hemostatic resection, guided by selective mesenteric arteriography. Chronic complications (inflammatory ste…

Operation in three stages with primary defunctioning colostomy and drainage of the perforation is the procedure mostly recommended in literature, although the mortality rate ranges from 33 to 75%. Therefore, we prefer the immediate resection of the perforated bowel with primary anastomosis, or if possible, in poor risk cases Hartmann's procedure respectively. Anastomosis should be protected by a defunctioning colostomy in all but exceptional good cases. 2 of our 18 patients died, only 1 of 15 (7%) with primary bowel resection.

A case of massive haemorrhage into the gastrointestinal tract from a pancreatic pseudocyst is presented. The pseudocyst was a complication of acute pancreatitis, and control of the haemorrhage by operation was difficult because of the friability of the cyst wall. The surgical procedure called duodenal diverticulization is described and was found to be a useful adjunct to suture ligation for bleeding which was almost uncontrollable and when death of the patient seemed inevitable.

INTRODUCTION: Traditional management of uncomplicated acute diverticulitis (UAD) includes antibiotic therapy, nonsteroidal anti-inflammatory drugs (NSAIDs), and dietary restriction, albeit recent evidence questions their benefit. The aim of the present study was to evaluate the safety and efficacy of an outpatient protocol without antibiotic therapy, NSAIDs, or dietary restriction.
METHODS: A single-center prospective study was conducted within the time frame of 2022 and 2024. Patients with UAD were divided into a historic cohort (HC), treated with the traditional protocol, and a study group …

Diverticulitis is usually managed conservatively. However, recurrent or complicated cases may require surgery. Daiobotanpito (DBT) is a traditional Japanese medicine used to treat inflammatory conditions of the lower abdomen. We report a case of recurrent sigmoid diverticulitis with a history of diverticular abscess managed with DBT as an adjunctive therapy. A 54-year-old woman was treated with antibiotics and DBT, after which oral intake was resumed without a recurrence of symptoms, thus allowing for discharge without surgery. She remained recurrence-free for one year while continuing the Kam…

Meckel's diverticulum (MD) is the most common congenital anomaly of the gastrointestinal tract and affects ∼2% of the population. Although usually asymptomatic, MD may lead to complications including diverticulitis, bleeding, obstruction, and-rarely-perforation. Because perforated MD often mimics acute appendicitis, diagnosis before surgery is frequently missed. We describe a 58-year-old man who presented with right lower quadrant pain, fever, and nausea. Ultrasound excluded appendicitis and raised suspicion for MD. Contrast-enhanced CT confirmed an inflamed 3.5-cm MD with surrounding fat stra…
