A rare complication of the duodenal diverticulum.

UNLABELLED: Introduction: An 80-year-old woman presented to the emergency department with severe right-sided abdominal pain that had started after her last meal. Physical examination revealed fever (38.6°C) and rebound tenderness in the right upper quadrant of the abdomen. The laboratory studies showed a leucocyte count of 11.3×109/L (normal, 3.7-8.0×109/L) and a C-reactive protein level of 2.34 mg/dL (normal, <0.03 mg/dL). There were no other significant findings. A CT scan of the abdomen with contrast revealed retroperitoneal air around a duodenal diverticulum (figures 1 and 2).Figure 1Compu

Duodenal Diverticulitis: To Operate or Not To Operate?

Duodenal diverticulum (DD) is a common incidental finding, which rarely causes complications. Perforation is one of the most feared and the least common complications. Surgery is the mainstay for complicated duodenal diverticulum, but with the advancement of medical treatment and intensive care, nonoperative management has been reported. We present a rare case of perforated DD that failed medical management and subsequently underwent surgical intervention. A 77-year-old, healthy female presented with right-sided abdominal pain with low-grade fever and leukocytosis. Computed tomography (CT) of

EMOTIONAL FACTORS IN DUODENAL ULCER.

Anxiety and depression levels in 25 duodenal ulcer patients were compared with equal numbers of age and sex matched normal controls, hospitalised patients with nonpsychosomatic medical illnesses and inpatients with neurosis. Sinha's anxiety scale and Hamilton's rating scale for depression were used to measure the levels of anxiety and depression respectively. Patients with duodenal ulcer were significantly more anxious and depressed as compared to normal subjects and patients with medical illnesses. Duodenal ulcer patients were significantly less anxious and depressed as compared to neurotics.

Duodenal carcinoid with carcinoid syndrome.

Carcinoid tumours are uncommon well-differentiated neuroendocrine tumours. Primary duodenal carcinoids account for less than 2% of all gastrointestinal carcinoids. Duodenal carcinoids are seldom associated with carcinoid syndrome. We report a rare case of duodenal carcinoid presenting as a carcinoid syndrome in a middle-aged man with upper abdominal pain, hot flushes, diarrhoea and dry cough. Endoscopy-guided biopsy and 24 h urine 5-hydroxyindoleacetic acid (5-HIAA) proved the diagnosis. He was further evaluated and managed with definitive surgical treatment.

Pathophysiology of duodenal ulcer disease.

: Before the discovery of Helicobacter pylori infection some 12 years ago, three major disturbances in gastric physiology had been identified in patients with duodenal ulcer disease. These abnormalities were: impaired acid inhibition of gastrin release from the antral mucosa, increased basal and stimulated acid secretion by the body of the stomach and increased acid load in the duodenum. Some of these abnormalities in gastric function can now be explained by the effects of H. pylori infection. The increased release of gastrin by the antral mucosa in duodenal ulcer patients, for example, can be

Gastric and duodenal neuroendocrine tumours.

Gastric neuroendocrine neoplasms (NENs) are increasing in frequency and have a varied spectrum with regard to histology, clinicopathologic background, stage, and prognosis. They are usually discovered incidentally, are for the most part benign and are associated with hypergastrinaemia (secondary either to chronic atrophic gastritis or rarely Zollinger-Ellison syndrome; types 1 and 2, respectively) or more rarely sporadic type 3. Applications of recent staging and grading systems - namely using Ki-67 proliferative indices - (from ENETS and WHO 2010) can be particularly helpful in further catego

Histopathology of the minor duodenal papilla.

The minor duodenal papilla, which is the orifice of the accessory, or dorsal, pancreatic duct/Santorini duct, mostly accompanied by pancreatic tissue, is situated about 2 cm ventroproximal to the major duodenal papilla. The patency of the terminal accessory pancreatic duct (APD) is recognized in about half or more of cases, and is related to the degree of fibrosis. The APD is lined with simple columnar epithelium and encircled by a smooth muscle layer. It is still controversial whether or not these muscle tissues comprise a sphincter muscle. Pancreatic tissue was found in about 80% of cases in

Duodenal stenosis secondary to tuberculosis.

An 18-year-old man presented with complaints of epigastric pain, vomiting after meals, loss of appetite and weight, evening rise of temperature, and irregular bowel movements. Barium meal follow-through revealed a dilated stomach and first and second parts of the duodenum and stenotic lesion in the second part of the duodenum. Endoscopic biopsy of the stenotic lesion revealed acute on chronic nonspecific duodenitis with fibrosis. Isoperistaltic antecolic gastrojejunostomy was done to bypass the obstruction, and enlarged lymph nodes were biopsied. The biopsy of the enlarged lymph nodes revealed…

Acquired duodenal obstruction in children.

Traumatic intramural hematoma of the duodenum is a rare cause of acquired duodenal obstruction in children, and a high degree of suspicion is therefore required to make an early and accurate diagnosis. We report a 6-year-old boy whose epigastrium was impacted by the handlebar of his bicycle during a traffic accident. The boy then experienced epigastralgia. Six days later, progressive bilious vomiting suggestive of gastrointestinal obstruction was noted. Imaging studies revealed a large hematoma extending from the fourth portion of the duodenum to the jejunum. Conservative methods of treatment
