Congenital aplasia of the external ear canal and total dehiscence of mastoid-tympanic segment of the facial nerve as a cause of recurrent facial paralysis.

Facial nerve (FN) macrodehiscence, in contrast to microdehiscence, generally occurs as a result of chronic otitis media and is rarely seen congenitally. A patient with normal hearing who had no history of ear disease or ear operation came to the clinic with recurrent facial paralysis (FP) and frequent blockage of the external ear canal with epithelial debris complaints. In the explorative ear surgery, we observed that the posterior wall of the external ear canal or tympanic ring was absent, a large external ear cavity was covered with a thin skin, the FN was under the skin, and the tympanic-ma

[Relapsing polychondritis].

Relapsing polychondritis is a rare disease of unknown origin consisting in recurrent inflammatory episodes of cartilaginous structures as well as other organs such as the eye, the inner ear and the kidney. Symptoms of autoimmune or rheumatic disorders frequently precede those typical of relapsing polychondritis and thus the contemporaneous presence of these diseases is a common observation. Consequently some diagnostic concerns connected to the great variability of the early symptoms and also to the lack of a semiotic test do exist. It is widely recognized that a serious involvement of the tra

Ear magnetic discs to prevent cauliflower ear: a case gone wrong.

We present a case of pinna pressure necrosis secondary to the use of ear magnetic discs, used in the management of auricular haematoma. A man in his 20s sustained a left auricular haematoma while wrestling. His coach aspirated the haematoma and advised the use of commercially available compression magnets on either side of the pinna to prevent haematoma reaccumulation. 5 days later, he presented to accident and emergency with left ear pain and swelling. Perichondritis was evident on examination and the compression magnets were removed to reveal significant pressure necrosis of the pinna. The s

Combined aesthetic and functional treatment of microtia.

The reconstruction of the ear pavilion includes at least two stages, usually three, and in many cases more. Complete reconstruction must begin with the plastic surgeon, who sculpts the cartilaginous skeleton, assembles parts of three ribs, and inserts the finished framework under the expanded skin of the mastoid area. During the second stage, the otologist intervenes, performing the cophosurgery, carving an external auditory canal, and completing the ossicular chain in the middle ear. The plastic surgeon harvests the skin for lining the external auditory canal and for the retroauricular fold a

Auricular repair with autogenous rib cartilage grafts: two decades of experience with 600 cases.

The purpose of this paper is twofold: to present a sound approach to auricular construction using methods that have evolved through two decades of my personal experience with 600 cases and to discuss pertinent information I have gleaned from a questionnaire sent to my operated patients. This series comprises 546 completed ears in 500 microtia patients (46 bilateral) and 75 completed ears in acquired deformities. Follow-up ranges from 1 to 17 years. Major complications such as infection, hematoma, or skin loss with cartilage exposure occurred in only 1.6 percent of cases and were limited to the

Cartilage tympanoplasty combined with eustachian tube balloon dilatation in the treatment of adhesive otitis media.

OBJECTIVES/HYPOTHESIS: To investigate the simultaneous application of cartilage tympanoplasty combined with eustachian tube (ET) balloon dilatation in the treatment of adhesive otitis media (AdOM). STUDY DESIGN: Multicenter, prospective, double-blind, randomized, controlled clinical trial. METHODS: Patients with AdOM were randomly divided into four groups: control group (conservative treatment), ET balloon dilatation (ETBD) group, cartilage tympanoplasty (CT) group, and cartilage tympanoplasty combined with ET balloon dilatation (ETBD+CT) group. Patients were followed up at 3 months, 6 month

[Auricular malformations].

Reconstruction of complex auricular malformations is one of the longest surgical technique to master, because it requires an extremely detailed analysis of the anomaly and of the skin potential, as well as a to learn how to carve a complex 3D structure in costal cartilage. Small anomalies can be taken care of by any plastic surgeon, providing that he/she is aware of all the refinements of ear surgery. In this chapter, we analyze retrospectively 30 years of auricular reconstruction, ranging from small anomalies to microtia (2500 cases), excluding aesthetics variants such as prominent ears.

Neurotic excoriations complicating superficial parotidectomy--a case report.

A case is described in which division of the great auricular nerve during parotidectomy was complicated by perichondritis of the ear cartilage following self-inflicted injury. The desirability of preserving the posterior branch of the nerve whenever possible is stressed.

Retroauricular island flap as an innovation for microtia ear reconstruction.

The use of a local island flap, designed with retroauricular skin, is an excellent, simple, and reproducible innovation to the classic microtia ear reconstruction, allowing adequate coverage of the cartilage framework, specifically of the conchal area, sparing tense skin closure, reducing skin suffering and necrosis, with a better aesthetic result achieving the naturally depressed concha aspect. The flap design and surgical technique is described. This flap was used in 13 consecutive microtia ear reconstructions in our unit, no complications were encountered, and aesthetic results were excelle

Auricular pseudocysts.

Auricular pseudocysts are uncommon benign lesions of the pinna. The etiology is not well understood but repeated trauma is considered as a possible trigger. Exact diagnosis is important to avoid overtreatment with disfigurement. Various surgical approaches can be used in accordance to the stage of the lesion. Minor invasive procedures provide good results with low recurrence rates.

Auricular ossification resulting in external auditory canal stenosis.

We report the case of a 49-year-old man who presented with left aural fullness, hearing loss, and a stenotic left external auditory meatus. Physical examination and computed tomography revealed the presence of an idiopathic ossification of the cartilaginous auricle and external auditory canal. The patient was successfully treated with meatoplasty. Pathologic examination of the removed specimen revealed osseous metaplasia.

Management of Chondrodermatitis Nodularis Helicis: A Systematic Review and Treatment Algorithm.

BACKGROUND: Chondrodermatitis nodularis helicis (CNH) is an idiopathic benign inflammatory painful condition of the ear. Still, to date, no study has yet evaluated nor compared treatments regarding their safety and efficacy. OBJECTIVE: The objective of this study is to review all available treatment modalities of CNH that were described in the literature, compare their efficacy and propose a treatment algorithm. METHODS AND MATERIALS: A systematic review of the literature was conducted using Embase, Medline, and Cochrane databases. Articles studying the management of CNH that reported data o

Reconstruction of the crus helicis in mild microtia using a preauricular tag.

A preauricular tag accompanying a mild microtia was utilized in reconstruction of the anterior aspect of the helix and the crus helicis. We believe that three-dimensional reconstruction of the crus helicis with a deep cymba conchae in microtia reconstruction is an important component in achieving a normal-looking auricle.

The surgical repair of congenital microtia and atresia.

Auricular reconstruction for congenital microtia is a complex multi-staged procedure. The repair of congenital atresia of the ear is one of the most difficult operations an otologic surgeon may attempt. In order to accomplish both procedures without endangering the integrity of either, there must be proper planning and close cooperation between the otologist and the facial plastic surgeon. In this article, we present our philosophy concerning the management and timing of these operations. The steps of each operation, and the impact of each on the other, are reviewed. Only through careful plann

Cell-engineered human elastic chondrocytes regenerate natural scaffold in vitro and neocartilage with neoperichondrium in the human body post-transplantation.

We have developed a unique method that allows us to culture large volumes of chondrocyte expansion from a small piece of human elastic cartilage. The characteristic features of our culturing method are that fibroblast growth factor-2 (FGF2), which promotes proliferation of elastic chondrocytes, is added to a culture medium, and that cell-engineering techniques are adopted in the multilayered culture system that we have developed. We have subsequently discovered that once multilayered chondrocytes are transplanted into a human body, differentiation induction that makes use of surrounding tissue
