One case of divergent dislocation of the elbow is reported. It was associated with other lesions of the upper limb due to the violence of the initial trauma. The authors found 6 other cases reported in the literature. They discuss the mechanism, diagnosis and treatment of such lesions.

Using conventional techniques with cutaneous electrodes, 14 parameters of antidromic, motor and mixed segmentary sensory conduction of the cubital nerve were assessed in the first 45 patients presenting to us with some form of electroneurographic abnormality. Changes suggestive of axonal degeneration due to a decrease in amplitude or conduction velocity of the distal segment were found in 17. The remaining 28 patients showed only signs of change in conduction velocity at the elbow and could be classified according to electroneurographic degrees of progressive abnormality. Changes in antidromic…

Lateral epicondylitis, one of the most common lesions of the arm, affects some 50% of tennis players. This condition poses a problem in clinical management because treatment is dependent not only on proper medical therapy but also on correction of the improper on-court biomechanics. The most common flaw is a late contact on the backhand groundstroke, forcing the player to extend the wrist with the extensor muscles. This action predisposes to trauma of the tendon fibers at the lateral epicondyle. Understanding the biomechanics will better prepare the physician to advise the patient and to commu

Arthroscopy : the journal of arthroscopic & related surgery : official publication of the Arthroscopy Association of North America and the International Arthroscopy Association1993eng
Thirty-four patients had one or more loose bodies in the elbow that were extracted arthroscopically in 33, with a small arthrotomy in one. The pain was improved in 85% of patients, locking in 92%, and swelling in 71% of the patients. The number of flexion contractures was reduced from 14 to 5. However, crepitus was improved in only 47% of patients. Overall, 89% of patients had significant improvement by the arthroscopic procedure. There were two transient symptoms of numbness of the forearm but no permanent complications. Overall, arthroscopic removal of loose bodies from the elbow was a succe…

Distinguishing between calcific soft-tissue masses and intra-articular loose bodies about the elbow can be difficult. We present the case of a 59-year-old woman with a slowly enlarging soft-tissue mass about the elbow. History, physical examination, and diagnostic studies did not establish a definite diagnosis, nor did they rule out the possibility of a neoplastic process. Excisional biopsy revealed a loose body contained in a cystic mass with elbow joint communication.

The clinical presentation of patients with elbow dislocations was reviewed to identify those factors indicating an increased risk for arterial injury. Sixty-two patients were treated for 63 elbow dislocations between January 1981 and July 1991. Eight patients (13%) sustained a concomitant arterial injury involving the brachial (7) and radial (1) arteries. Three clinical findings, absence of a radial pulse, open dislocation, and presence of systemic injuries, were correlated with arterial injury. A palpable radial pulse was absent in six (75%) patients with an arterial injury but in only two (4…

Clinical and radiographic follow-up study of the elbow and shoulder in 19 children and adults with Apert syndrome demonstrates that these two joints are affected to varying degrees. Shoulder motion is never normal and may be quite restricted, with the progression of growth abnormalities of the proximal humerus and accompanying glenoid dysplasia. Limitation of abduction is secondary to an impingement of an overgrown greater tuberosity upon the acromion. Clinically, these children appear to have anterior subluxations. Elbow motion is limited in a much smaller percentage of patients and was not a…

The importance of nerve compression syndrome in the upper extremity for making a diagnosis or differential diagnosis in orthopedics, surgery, especially hand surgery, and neurology is evident. We review the characteristics of this entity, demonstrate the pathophysiological findings, and stress their importance on the diagnosis, therapy, indications for operation, and operative techniques in our patients. With special regard to the topographic anatomy each of these nerve compression syndromes of the elbow and the wrist region is demonstrated systematically and consequences are discussed for sur…

A case of rheumatoid arthritis, whose presenting problem was a non-infective fistula of the elbow joint, is described. Penicillamine therapy was ineffective, but synovectomy led to healing.

Between 1969 and 1985 26 patients with destructive lesions of the distal humerus were treated by endoprosthetic replacement; each implant was custom-made and incorporated part of the distal humerus or the entire bone as well as a hinged total elbow replacement. Recurrence occurred in three of the patients with tumours, and three prostheses were removed because of deep infection in patients with previously compound injuries of the elbow. Another three loosened without infection, but none needed revision or removal and no amputations resulted. Other complications included nerve palsies, but the …

Synovectomy of twenty-three elbows was done in eighteen patients, eight to twenty-five years old, who had severe hemophilia and were followed for eighteen to seventy months. Episodes of bleeding recurred in four elbows, and moderate pain persisted in three. A significant improvement in mobility was observed for pronation-supination in nine elbows and for flexion-extension in fourteen. No radiographic evidence of arthritis was seen. Synovectomy of the elbow, performed through a single lateral incision, appears to be a valuable surgical procedure in hemophiliacs in whom non-operative treatment h…

The investigation of roentgenograms made after a preliminary contrast study of the elbow joint has shown that traumas may be followed by pathological changes of the cartilaginous tissue, elbow bursa and filling of the fossae of the olecranon and coronary processes by scarry and osseous tissue. The pathological changes mentioned above result in disturbances of the movement of the elbow joint.

If clinical examination and radiological diagnosis fail to identify undiagnosed complaints in the elbow joint, arthroscopy is indicated. When it is clearly indicated, and when performed by a practised examiner, arthroscopy can make a very useful contribution to the diagnosis of elbow complaints.

A review is made of the literature of the mechanics of the elbow joint and the types of operations available. It is clear that the elbow is a complex joint and that in patients with rheumatoid arthritis destruction and disability occurs relatively late in the disease. Of the operations available, synovectomy and excision of the radial head are best done early in order to obtain the movement of supination and pronation. Total elbow replacement, particularly the unconstrained type is producing encouraging results and seems rational in view of the structure of the elbow. However it is early days …

Twenty-six patients with old unreduced dislocation of the elbow treated by open reduction were reviewed. All patients improved after surgery. It is felt that any patient with an elbow dislocation which is less than 3 months old should be treated by open reduction. The operative technique using a medial and lateral approach and keeping the dissection extraperiosteal is described.

Three generations of a family manifest similar skeletal abnormalities: proximal symphalangism with several unusual features, metacarpophalangeal synostoses, massive tarsal and carpal fusions and abnormalities of the elbows (radial head dislocation, radiohumeral synostosis). Two members of this family were previously reported by Pearlman et al. as examples of the Nievergelt syndrome, but the affected individuals lack the mesomelic dysplasia that is an integral part of that entity. Moreover, the Nievergelt syndrome does not include symphalangism. The term "Nievergelt-Pearlman syndrome" should be…
