Traumatic nerve injuries about the elbow.

Although not common, nerve injuries about the elbow occur because of the proximity of the three upper extremity nerves or because of the relationship of the median, ulnar, and radial nerves to the bony and soft-tissue structures about the elbow joint. Nerve injuries at and about the elbow joint occur more frequently with fractures than with any other kind of trauma. Nerve injuries may be found with periarticular fractures, dislocations, gunshot wounds, lacerations, and other iatrogenic causes.

Nonsurgically treated terrible triad injuries of the elbow: report of four cases.

This case series describes 4 patients with terrible triad injury of the elbow (dislocation with fractures of the radial head and coronoid) who were treated nonsurgically. Following nonsurgical treatment in this select group of patients, the elbow was well aligned, and the patients regained good elbow function. Three of these 4 patients had good results. One patient had surgery for residual stiffness, ulnar neuropathy, and a radial head deformity. We conclude that, in selected terrible triad cases, when the elbow is well aligned and the radial head and coronoid fractures are relatively small an

Spontaneous Reduction of Elbow Dislocation with Entrapment of Medial Epicondyle in Children: A Case Report.

INTRODUCTION: Elbow dislocation accounts 3-6% of all pediatric elbow injuries. Medial epicondyle is the most common fracture related to the elbow dislocation in pediatric population. About 15-18% of medial epicondyle fractures related to dislocation present with the incarceration of medial epicondyle in the elbow joint after reduction. This situation is the only absolute surgical indication for epicondyle fracture in this population. CASE REPORT: A 15-year-old male children admitted to the emergency department with swelling, pain, and limitation of elbow range of motion after falling off a ho

Fractures of the radial head treated by internal fixation: late results in 26 cases.

Twenty-six patients, ranging in age from 14 to 57 years (average 29 years), were evaluated an average of 7 years and 4 months (range 1-14 years) after open reduction and internal fixation of a displaced radial head fracture. Using Mason's classification, there were 11 type II fractures, 9 type III fractures, and 6 type IV fractures with associated dislocation of the elbow. Seven patients had ipsilateral extremity injuries that included fractures of the coronoid process, capitellum, humerus, and distal radius. Using the Broberg and Morrey elbow score, good or excellent results were achieved in

MR imaging of osteochondral and articular lesions.

MR imaging provides clinically useful information in assessing the elbow joint for osteochondral and articular lesions. Post-traumatic osseous abnormalities well seen by MR imaging include radiographically occult fractures, stress fractures, bone contusions, osteochondritis dissecans, and chondral defects. Intraarticular loose bodies can be identified with MR imaging, especially if fluid or contrast material are present within the elbow joint. MR imaging can also provide additional information regarding synovial osteochondritis, osteoarthritis, and bursitis about the elbow.

Risk Factors for Stiffness After Surgical Treatment of Pediatric Humeral Lateral Condyle Fractures: Results of a Multicenter Study.

INTRODUCTION: Pediatric humeral lateral condyle fractures are the second most common elbow fractures. Their treatment presents challenges due to physeal and intra-articular involvement. Postoperative stiffness is a common concern that can limit limb functionality. This study aims to identify risk factors for postoperative stiffness in a large cohort of these fractures across multiple institutions. METHODS: A large, multicenter retrospective review of medical records from 6 level I trauma centers was conducted. Data from children aged 1 to 12 years with lateral condyle fractures treated betwee…

Good outcomes of bipolar radial head arthroplasty for comminuted radial head fractures : a cohort study of 64 patients with an average 5.2-year follow-up.

AIMS: Radial head fractures are among the most common upper limb fractures, constituting up to 30% of all elbow fractures. While many can be managed non-surgically with favourable outcomes, approximately one-third require surgical intervention, typically involving open reduction and internal fixation or radial head arthroplasty. This study aimed to assess the functional and radiological outcomes of elbow injuries involving comminuted radial head fractures treated with bipolar radial head arthroplasty. METHODS: We retrospectively analyzed 64 patients, of whom 55 participated in the follow-up, …

Open reduction and internal fixation of radial head fractures.

Open reduction and internal fixation of displaced fractures of the radial head were reviewed in 14 elbows. Follow-up averaged 32 months. The average elbow score for Mason type II fractures was 96.8 points, corresponding to 100% good or excellent results. Average flexion was 142.5 degrees, and the mean fixed flexion deformity was 3.9 degrees. There was no loss in grip strength. An almost normal elbow was the expected result. Good or excellent results were achieved in only 33% of Mason type III fractures treated with open reduction and internal fixation. The average elbow score was 72.9 points,

[Hinged external fixation in orthopaedic and trauma surgery of the elbow].

PURPOSE OF THE STUDY: A hinged external fixator of the elbow provides stable fixation of the joint while maintaining the range of its motion. The aim of the study was to evaluate a group of patients in whom an external fixator was used to manage severe injuries to the elbow, namely, traumatic unstable dislocation, unstable fracture-dislocation, and elbow fractures not permitting management by primary osteosynthesis. This also involved assessment of early elbow mobilisation and a comparison of this group with a group of patients treated conservatively for less severe elbow injuries. MATERIAL A…

Radial head fractures: loss of cortical contact is associated with concomitant fracture or dislocation.

HYPOTHESIS: Among radial head fractures displaced greater than 2 mm (Broberg and Morrey modified Mason type 2), separation (complete loss of cortical contact) of at least 1 radial head fracture fragment is associated with a complex injury pattern, meaning that there are other concomitant elbow fractures or ligament injuries. MATERIALS AND METHODS: We identified 291 consecutive skeletally mature patients with 296 radial head fractures treated during a 6-year period. Of these, 121 consecutive fractures of part of the radial head displaced greater than 2 mm (type 2) were classified according to …

Fractures of the olecranon: a 15- to 25-year followup of 73 patients.

The incidence of olecranon fractures in adults and the long-term outcome of closed olecranon fractures in 45 women and 28 men (mean age, 54 and 36 years at the time of fracture, respectively), were examined at a mean of 19 years after the fracture. The uninjured elbows served as controls. Thirteen percent of the original fractures were displaced less than 2 mm, 65% more than 2 mm, and 22% were multifragmental. Primary treatment consisted of mobilization in 4%, application of a plaster cast in 12%, and open reduction and internal fixation in 84% of the elbows. The incidence of an isolated fract

Elbow and wrist injuries in sports.

Any disabling injury of the elbow or wrist should be studied roentgenographically for evidence of fracture which may not be otherwise evident but which may cause permanent disability unless the joint is immobilized for healing."Tennis elbow" may be treated with physical therapy and analgesic injection but may require splinting or tendon stripping. Elbow sprain can occur in the growing epiphysis but is rare in adults. A jarring fall on the hand may cause fracture or dislocation at the elbow. Full extension of the joint should be restored gradually by active exercise rather than passive or forci

Interventions for treating wrist fractures in children.

BACKGROUND: Approximately a third of all fractures in children occur at the wrist, usually from falling onto an outstretched hand. OBJECTIVES: We aimed to evaluate removable splintage versus plaster casts (requiring removal by a specialist) for undisplaced compression (buckle) fractures; cast length and position; and the role of surgical fixation for displaced wrist fractures in children. SEARCH STRATEGY: We searched the Cochrane Bone, Joint and Muscle Trauma Group Specialised Register (October 2007), the Cochrane Central Register of Controlled Trials (The Cochrane Library 2007, Issue 4), ME

Current treatment concepts for "terrible triad" injuries of the elbow.

Elbow fracture-dislocations destabilize the elbow, preventing functional rehabilitation. If left untreated, they commonly result in functional compromise and poor outcomes. The "terrible triad" injury is classically described as a combination of a coronoid process and radial head fractures, as well as a posterolateral elbow dislocation. Surgical treatment to restore stable elbow range of motion has evolved in the past few decades based on increased understanding of elbow biomechanics and the anatomy of these injuries. This article highlights current concepts in the treatment of these complicat

Radial head replacement or repair for the terrible triad of the elbow: which procedure is better?

BACKGROUND: The terrible triad of the elbow comprises an ulnar coronoid process fracture, a radial head (RH) fracture and posterior dislocation of the elbow. It is considered severe by many clinicians because it is difficult to repair and has a poor prognosis; therefore, it deserves more attention. METHODS: Thirty-nine patients with terrible triad of the elbow characterized by a Mason type-III RH fracture were randomly divided into either an RH repair group (n = 19) or an RH replacement group (n = 20). The complication rates, radiological outcomes, Mayo Elbow Performance Score and range of mo
