OBJECTIVE: Bifrontal stimulation in electroconvulsive therapy (ECT) has been increasingly administered and evaluated over the past years. However, no attention has been paid to cardiac side effects of this novel form of treatment during the application of the electrical stimuli.
METHOD: Cardiac responses to different electrode positions were compared intraindividually, using synchronised electrocardiograms and waveforms of pulse oximetry.
RESULTS: Unilateral ECT gave rise to a marked cardiac response during stimulation. Intervals between two consecutive heartbeats increased from 0.6 ± 0.0 se…

Pulmonary edema after electroconvulsive therapy (ECT) is a rarely reported condition that can result in serious morbidity and even death if not promptly recognized and treated. We report the case of 21-year-old man with FG syndrome and schizophrenia who developed negative-pressure pulmonary edema after his 28th ECT. The patient developed acute hypoxemia requiring positive-pressure ventilation and was observed overnight in the intensive care unit. He recovered fully and received 43 subsequent ECT treatments without complication. This case illustrates the importance of taking steps to prevent ai…

Oxyhaemoglobin saturation was monitored continuously during electroconvulsive therapy in two groups of patients. All patients were anaesthetised using methohexitone (40-100 mg) and suxamethonium (25-50 mg). In group 1 (n = 48), the patients' lungs were ventilated manually with oxygen in air (40-50%) during the modified convulsion. In group 2 (n = 47), the convulsion was allowed to subside before recommencing manual ventilation with a similar gas mixture. Eight patients in group 1 and seven patients in group 2 received more than one shock and were excluded, leaving 40 patients in each group. In…

In the course of a series into the effects of unilateral electroconvulsive therapy on memory functions,double-blind cross-over intraindividual comparison were performed after the second and third treatment in patients suffering from depressive syndrome. The main aim of the project, which is still in course, was to explore the possibility of a further reduction of the side-effects of this antidepressant method. Three separate comparisons were performed between unilateral nondominant temporo-parietal ECT and (a) unilateral dominant temporo-parietal ECT, (b) unilateral non-dominant fronto-parieta…

Arterial blood gases and vital signs were monitored in a patient receiving electroconvulsive therapy (ECT) during the third trimester of pregnancy. Alterations in blood pressure and heart rate were similar to those noted in prior studies. Pretreating with 100 percent oxygen (02) and assisting ventilation until return of adequate spontaneous respirations, prevented hypoxemia, significant hypercarbia, and cardiovascular changes. The administration of succinylcholine prevented the systemic manifestations of the electrically-induced seizure. One fetal arrhythmia occurred, apparently unrelated to c…

Magnetic seizure therapy (MST) and electroconvulsive therapy (ECT) are effective treatments for schizophrenia (SCZ) and major depressive disorder (MDD), with MST offering advantages in preserving cognitive function. Although cognitive profiles are known to influence treatment outcomes, their role in moderating response to seizure therapies remains unclear. We conducted an open-label, non-randomized controlled trial to compare the efficacy of MST and ECT across different cognitive profiles. From October 2020 to June 2024, we recruited 263 patients with SCZ and 135 patients with MDD, identifying…

Electroconvulsive therapy is a reliable and safe option for patients with psychiatric disorders who are resistant or intolerant to medication as these patients usually respond quickly to this form of treatment. For clinicians, the management of these often severely ill patients is an important challenge given the high rate of relapse of these disorders. Maintenance electroconvulsive therapy is an interesting treatment option for these patients. Until now, only few controlled studies exist about the long-term outcome of this treatment in depressive disorders and other psychiatric disorders. Alt…

BACKGROUND: The default pulse width setting on a commonly used electroconvulsive therapy (ECT) device, 0.5 ms (ms), is untested against the proven effectiveness of conventional 1.0-1.5 ms brief pulse widths.
METHODS: We examined the association between pulse width and clinical response using registry data from 87 centres in the United Kingdom and Republic of Ireland in patients (N = 1956) receiving consecutive acute courses of ECT ending during 2023 for any clinical indication. The outcome was clinical response, defined as an end-of-treatment Clinical Global Impression‒Improvement score of 1 …

OBJECTIVE: Status epilepticus (SE) is a neurologic emergency. Although evidence-based treatments exist for SE, treatment of refractory status epilepticus (RSE) and super refractory status epilepticus (SRSE) lacks evidence. Electroconvulsive therapy (ECT) is safe and efficacious when used to treat psychiatric disorders. Seizure control attributed to ECT in the setting of SRSE has been reported in case reports and case series. The objective of this scoping review was to understand the current knowledge regarding the safety and efficacy of ECT in aborting SRSE in the adult and pediatric populatio…

BACKGROUND: Electroconvulsive therapy (ECT) is the most effective treatment of major depression, but autobiographical memory loss may limit its use. Despite previous attempts to synthesise the literature, the nature of autobiographical memory loss after ECT is still debated.
AIMS: To provide an overview of the effect of ECT on autobiographical memory in patients with depression and explore whether the effect is temporary or permanent. Furthermore, we wanted to analyse if ECT parameters or clinical information are associated with this effect.
METHOD: PubMed, EMBASE, PsycINFO and Web of Scienc…

Electroconvulsive therapy (ECT) remains one of the most effective interventions for treatment-resistant psychiatric disorders, particularly major depressive disorder and bipolar disorder. Despite extensive clinical and preclinical investigations, the precise neurobiological mechanisms underlying ECT's therapeutic effects are not fully understood. This review explores the molecular and cellular pathways involved in ECT, emphasizing its impact on neurotrophic signaling, oxidative stress, apoptosis, and neuroplasticity. Evidence suggests that ECT modulates brain-derived neurotrophic factor and ot…

Retrograde amnesia for autobiographical memories is a commonly self-reported cognitive side-effect of electroconvulsive therapy (ECT), but it is unclear to what extent objective performance differs between ECT-exposed and ECT-unexposed patients with depression. We investigated the association between exposure to brief-pulse (1.0 ms) bitemporal or high-dose right unilateral ECT and retrograde amnesia at short- and long-term follow-up, compared with inpatient controls with moderate-to-severe depression without lifetime exposure to ECT and receiving psychotropic pharmacotherapy and other aspects …

BACKGROUND: Fragile X syndrome (FXS) is associated with autonomic dysfunction and ion channel abnormalities that increase the risk of arrhythmias. Electroconvulsive therapy (ECT), used to treat catatonia in FXS, can trigger a sympathetic surge, potentially inducing ventricular tachycardia.
CASE SUMMARY: A 52-year-old man with FXS and catatonia developed recurrent wide complex tachycardia and cardiac arrest during ECT sessions, despite normal electrolytes, no structural heart disease, and a normal baseline electrocardiogram. The patient had recurrent cardiac arrest despite scheduled oral amiod…

Memory impairment is a serious cognitive side effect of electroconvulsive therapy (ECT) in the treatment of major depressive episodes (MDEs) and has garnered widespread attention in clinical practice, but its underlying evolution pattern during the course of ECT remains rarely understood in detail. Associative memory (AM) is a core indicator that reflects memory impairment in ECT. This study aimed to identify the dynamic trajectory of AM impairment and explore associated predictive factors. 405 intensive longitudinal AM data from 81 patients with MDE were collected at the baseline, after the f…

BACKGROUND: Electroconvulsive therapy (ECT) induces a generalized seizure under anesthesia with an electrical current for treatment-resistant patients and may be underutilized. To our knowledge, no large-scale, American, nationwide hospital system retrospective study has examined how ECT affects psychiatric rehospitalization chances.
METHODS: We analyzed initial inpatient encounters for adults aged > 18 years at HCA Healthcare Behavioral Health Units from 2016 to 2021 with diagnoses of major depressive disorder with/without psychosis, bipolar disorder with/without psychosis, schizoaffective d…
