The development of video-laparoscopic techniques and surgical experience in this field have led to a resurgence of laparoscopy diagnostic purposes and have made it often the first action of surgical therapy with totally mini-invasive procedure. The purpose of this paper is to underline the clinical pictures of non traumatic acute abdomen that can profit from laparoscopy diagnostic ally and in related problems. Laparoscopy, which has been well known as a diagnostic procedure for more than a century, has recently established itself as an important therapeutic procedure in several branches of surgery. The purpose of this retrospective study was to evaluate the results of the laparoscopic surgical treatment of abdominal emergencies. From May 1991 to September 1995, 200 patients operated by laparoscopy for an acute abdomen were included in this study. The application of therapeutic ERCP and interventional radiology has significantly altered the treatment of biliary tract emergencies. Although surgery is the principal treatment for acute cholecystitis, nonoperative alternatives exist for high-risk patients. New, prospective, randomized trials have demonstrated that endoscopic management of severe cholangitis is superior to surgery, and that endoscopic sphincterotomy and stone extraction are superior to conservative treatment in severe gallstone pancreatitis. ERCP allows nonoperative management of postoperative bile leaks as well.

This study presents the files of nine pregnant women with severe neurosurgical diseases (traumatic, vascular, and tumoral) who were treated in the Neurosurgical Clinic, from 1982 to 1988. Conclusions were drawn after a survey according to the optimal therapy applied, to the mother or fetus priority, to the age of patients and to the severity of the neurological impairment. The early intervention of a team including the neurosurgeon, obstetrician and the specialist in the intensive care unit saves one life or both. A delay condemns both.

The attainment of profound local anesthesia in the management of pulpal and periradicular pain often eludes even the most skilled clinician; various reasons for this include, but are not limited to, localized inflammation, osseous and neural variations, and poor technique of the operator. As the bone marrow is a vascular tissue which cannot collapse, it may be used as a vascular access to treat life-threatening emergencies especially in children. Emergencies involving the respiratory tract are not uncommonly encountered in infants and young children. The spectrum of disorders causing respiratory distress varies from self-limited disorders such as bronchiolitis to potentially life-threatening conditions such as acute epiglottitis. This article outlines the clinical manifestations of some of the common respiratory emergencies presenting in infants (beyond the neonatal period) and young children, as well as strategies for management.

Careful telephone triage of women with significant obstetric and gynecologic emergencies is the optimal way to avoid devastating complications in the office. With practical knowledge and some specialized office equipment and medications, most obstetric and gynecologic emergencies can be managed effectively in the office or stabilized while arranging transport to a hospital. Management of the most common complications likely to be encountered is outlined in this article.
