Left Ventricular Pacing-Associated Cardiomyopathy Revealing Myocardial Vulnerability in Congenital Complete Heart Block.

BACKGROUND: Mechanisms underlying cardiac manifestations of immune-mediated congenital complete heart block (CCHB) remain unclear. Dilated cardiomyopathy (DCM) in immune-mediated CCHB has been associated with RV pacing, whereas reports of LV pacing-associated DCM are lacking. CASE SUMMARY: A girl with immune-mediated CCHB showed endocardial fibroelastosis (EFE) at 20 weeks of gestation before heart block developed at 22 weeks. After LV basal-lateral pacing, she developed DCM with LV dyssynchrony. Cardiac resynchronization therapy (CRT) improved LV function and induced reverse remodeling. CON…

Cardiovascular causes for sudden infant death.

Although sudden infant death syndrome (SIDS) is a cause for sudden infant death, other causes should be ruled out before diagnosing SIDS. Cardiac causes for sudden infant death include viral myocarditis, congential heart disease particularly congential aortic stenosis, endocardial fibroelastosis, and anomalous origin of the left coronary artery from the pulmonary artery. Other cardiac conditions that may result in sudden death include rhabdomyomas of the heart in tuberous sclerosis and conduction system disorders. The most frequent conduction system disorders resulting in sudden death include …

In Vitro Models to Study Fibroelastic Remodeling of the Endocardium in Congenital Heart Disease.

Congenital heart defects (CHD) are characterized by pathological intracardiac blood flow, exposing the endocardium to disturbed shear stress. This contributes to the development of endocardial fibroelastosis (EFE), a restrictive fibrotic remodeling process associated with diastolic dysfunction. Despite the pathomechanistic role of the endocardium in CHD, EFE-derived endocardial endothelial cells (EdEECs) remain largely understudied. No standardized protocol exists for isolating and purifying them from human pediatric tissue. Tissue from N = 15 pediatric patients with (1) hypoplastic left heart…

Neonatal atrial fibrillation after surgical repair of tracheoesophageal fistula with esophageal atresia.

Atrial fibrillation is rare in childhood that had not been reported in neonates with normal cardiac morphology and function. The authors pre … valve disease, Marfan's syndrome with mitral regurgitation, infective endocarditis, cardiomyopathy, endocardial fibroelastosis, paroxysmal atrial tachycardia of infants, and cardiac tumors [2]. All these patients had underlying cardiac disease.

[A rare cause of myocardial infarction: papillary fibroelastoma of the aortic valve].

Papillary fibroelastoma is a rare, benign endocardial tumour usually located on the cardiac valves. Before echocardiography, these tumours were chance findings either at surgery or at autopsy. With the advent of echocardiography, the diagnosis has become commoner and they are often the cause of systemic embolism justifying surgical ablation. In this case, an aortic valve papillary fibroelastoma presented with myocardial infarction in a 78 year old woman with normal coronary angiography. The diagnosis was strongly suspected at echocardiography and confirmed by histological analysis of the surgi

Cardiac transplantation in an 8-month-old female infant with subendocardial fibroelastosis.

On Nov 1, 1984, an orthotopic cardiac transplantation was performed in an 8-month-old female infant with subendocardial fibroelastosis. Because of the advanced state of cardiac failure, the operation was done despite a positive tissue crossmatch for antitoxic donor-specific antibodies. Immunosuppression consisted of high doses of cyclosporine (up to 550 mg/m2 or 30 mg/kg) and steroids. Hypertension and tremor of the extremities, which were attributed to cyclosporine, occurred during the first week but resolved after seven days. No signs of nephrotoxic effects have been noted; however, a severe…
