Intelligent segmentation and measurement based on U-HRCT to explore the anatomical characteristics of the inner ear in unilateral Meniere's disease: a retrospective quantitative study.

OBJECTIVES: To investigate inner-ear anatomical features and their association with endolymphatic hydrops (EH) in Meniere's disease (MD) using ultra-high-resolution CT (U-HRCT) with automated segmentation and measurement. MATERIALS AND METHODS: We retrospectively analyzed U-HRCT data from 105 unilateral MD patients and 100 normal controls. All patients underwent gadolinium-enhanced MRI for EH grading. The TransUNet network segmented cochlear, vestibular, and semicircular canal structures. Cochlear dimensions were extracted via principal component analysis; vestibular morphology was approximat…

Microscale Proteomic Analysis of the Endolymphatic Sac in Menière's Disease Patients.

OBJECTIVE: Endolymphatic hydrops is the primary pathological feature of Menière's disease (MD). An imbalance between endolymphatic fluid production and absorption due to endolymphatic sac (ES) dysfunction may be the major cause of endolymphatic hydrops. This study aimed to identify and characterize global protein expression of the ES in MD patients using microscale proteomics for the first time. METHODS: Liquid chromatography-tandem mass spectrometry was used to analyze the ES samples from six MD patients and six acoustic neuroma (AN) patients (as control) for microscale proteomics and differ…

The use of intraoperative electrocochleography in Meniére's surgery.

Intra-operative electrocochleography was undertaken during surgery for Meniére's disease. It was found that the electrocochleogram (ECoG) provided a stable measure which accurately reflected changes within the inner ear during surgery. During salt osmosis of the round window, a rapid change in the summating potential versus action potential ratio (SP/AP) occurred which showed when the endolymphatic hydrops (ELH) had been altered. During a modified cochleostomy procedure, the ECoG showed when the cochlear duct had been ruptured. Forty ears were monitored during endolymphatic sac surgery. Twenty…

Human Temporal Bone Study of Vestibular Histopathology in Cochlear Implant Patients With Cochlear Hydrops.

HYPOTHESIS: Endolymphatic hydrops (EH) associated with cochlear implantation are associated with vestibular dysfunction. BACKGROUND: Vestibular dysfunction is a known risk after cochlear implantation (CI). CI has been shown to cause cochlear hydrops due to fibrosis surrounding the ductus reuniens. However, the association of cochlear hydrops with vestibular hydrops and the relationship to vestibular symptoms remain unknown. METHODS: Histopathological analysis and clinical evaluation of the vestibular end organs of 17 human temporal bones (HTB)s exhibiting cochlear hydrops from 15 CI recipien…

3D reconstruction of cochlea using optical coherence tomography.

Recently, in vivo visualization of the cochlea and the smaller structures inside of it has been achieved by optical coherence tomography (OCT). This makes it possible to use OCT imaging for diagnosis of diseases such as Meniere's disease through measuring the degree of endolymphatic hydrops. To this end, we present a novel method for 3D segmentation of these cochlear OCT images that is based on superpixels and diffusion maps. The method takes as input grayscale volumetric OCT images and outputs a binary image with the segmented cochlea. We show that the proposed method is suitable for segmenti…

Electrocochleography and the glycerol dehydration test: a case study.

The electrocochleographic changes which occurred during glycerol dehydration in an ear affected by endolymphatic hydrops due to Menière's disease are discussed. This single case study is reported because the recordings were large and stable, and because they clearly showed the electrocochleographic abnormality associated with endolymphatic hydrops; namely, a large summating potential (SP) in relation to the action potential (AP). After dehydration a dramatic improvement occurred in both the pure tone and speech audiogram. The electrocochleogram showed a decrease in the SP amplitude and little …

Bilateral electrocochleographic findings in unilateral Menière's disease.

At present, electrocochleography is the only proven investigation that can demonstrate objectively the presence of endolymphatic hydrops. The electrophysiologic recordings in response to sound stimuli show an enhancement of the negative summating potential in these cases. It is well established that patients with unilateral Meniere's disease have a high likelihood of development of the disease bilaterally in the fullness of time. Using transtympanic electrocochleography in 40 patients who manifested unilateral clinical Meniere's disease, we have recorded bilateral abnormalities indicative of e…

Endolymphatic sac obstruction. Biochemical studies.

Inner ear fluids were studied biochemically in guinea pigs from 1 to 16 weeks after producing endolymphatic hydrops by obstructing the endolymphatic duct. Fluid collected from beneath the footplate showed changes of 50% of the animals indicating distension of the saccule. There was an increase in potassium concentration and decrease in sodium concentration of the collected fluid, indicating a traumatic mixing of endolymph and perilymph produced by rupture of the saccule during collection of the fluid. The smallest changes occurred at one week, and the greatest changes were found four months af…

A Synchrotron and Micro-CT Study of the Human Endolymphatic Duct System: Is Meniere's Disease Caused by an Acute Endolymph Backflow?

Background: The etiology of Meniere's disease (MD) and endolymphatic hydrops believed to underlie its symptoms remain unknown. One reason may be the exceptional complexity of the human inner ear, its vulnerability, and surrounding hard bone. The vestibular organ contains an endolymphatic duct system (EDS) bridging the different fluid reservoirs. It may be essential for monitoring hydraulic equilibrium, and a dysregulation may result in distension of the fluid spaces or endolymphatic hydrops. Material and Methods: We studied the EDS using high-resolution synchrotron phase contrast non-invasive …
