Differential diagnosis of virus-like particles in the human inner ear.

The entire endolymphatic duct and sac as well as the vestibular epithelia were obtained from four patients with Meniere's disease during translabyrinthine (TL) eighth nerve section and from 12 patients undergoing TL resection of acoustic schwannomas. After these specimens were processed for routine transmission electron microscopy (TEM), they were studied for morphologic evidence of viral infection. Although no virus particles were identified, numerous regularly occurring cell components and artifacts were found to morphologically mimic viruses. An atlas of these structures is presented.

Computed tomographic findings in Ménière's disease.

Symptoms in Ménière's disease are explained by hydrops of the endolymphatic system with recurrent ruptures of the membranous labyrinth. The primary cause of the increased endolymphatic volume appears to be an imbalance between secretion and resorption of endolymph which may be due to an obstruction of the endolymphatic duct and sac, located in the vestibular aqueduct (VA). Non-visualization or narrowing of the latter have been demonstrated by conventional tomography. Also, sclerosis and hypoplasia of the retrolabyrinthine portions of the temporal bone have been documented. By high resolution c

Clinical electrocochleography in the diagnosis and management of Meneère's disorder.

Patients with a fluctuating hearing loss, episodic vertigo and tinnitus with no known aetiological cause were investigated as cases of Meniè … with high scores, were statistically more likely to show an abnormal SP/AP waveform. The results of endolymphatic sac surgery were also assessed over 1 year in 28 cases, and it was found that those patients whose ECochG had revealed an abnormal SP/AP waveform prior to surgery benefitted in a statistically significant manner more from the operation.

Endolymphatic fistulization.

Three previous papers have detailed my experience with various methods of surgical treatment of Meniere's disease: 1) surgery for all patients; 2) surgery for only those patients who demonstrated patency of the vestibular aqueduct as seen on tomographic examination; and 3) prognostic tests to determine which patients were most suitable for endolymphatic sac surgery. This paper details the results of endolymphatic shunt surgery performed on 43 patients over a 3-year period, restudying my prior conclusions. All patients were operated using a new method of capillary endolymph dispersement regardl…

Endolymphatic mastoid shunt for treatment of Meniere's disease: a five year study.

The endolymphatic mastoid shunt seems to be an effective treatment for those patients whose Meniere's disease is refractory to medical and/or supportive treatment. This series is composed of 48 patients who were followed for up to 5 years postoperatively, with 1 year being the minimum follow-up period; 81% of these patients obtained a satisfactory relief of their vertigo. The morbidity of the procedure is low with no severe or total hearing loss reported in this series. Surgery should not be recommended until the symptoms have caused severe incapacitation; however, this delay must be tempered

Usefulness of the radiological planning for hearing preservation surgery in vestibular schwannoma.

PURPOSE: During vestibular schwannoma surgery there is a risk of endolymphatic duct and sac injury, which may cause a loss or a deterioration of hearing. The goal of the study was to evaluate the empirical utility of presurgical planning using CT with the bone window for the hearing preservation surgery. METHODS: The study was performed on 14 human temporal bones. CT scans with the bone window were obtained in the standard position. Safe drilling line was evaluated and after that drilling distances were analysed: the lateral drilling distance, total length of internal acoustic meatus, drilled

Reduction of inner ear inflammation by treatment with anti-ICAM-1 antibody.

The in vivo effect of systemic administration of monoclonal antibody (mAb) against ICAM-1 (alphaICAM-1) in experimental immune-mediated labyrinthitis was evaluated. The alphaICAM-1-treated rats showed reduced inflammatory cell infiltration in the scala tympani and the perisaccular tissue of the endolymphatic sac. However, with this experimental paradigm, labyrinthitis could not be completely abolished. These findings suggest that ICAM-1-dependent pathways play an important role in the series of immunologic events occurring in the inner ear, and that the use of ICAM-1 antagonist may be a possib…

Pathology of Ménière's disease as it relates to the sac and tack procedures.

The rationale for endolymphatic sac and tack operations is to prevent the further accumulation of endolymph, the former by draining endolymph into extralabyrinthine tissues and the latter by shunting endolymph into the perilymphatic space. While the basic concepts are reasonable enough, the probability of achieving these objectives seems remote. There appears to be adequate clinical evidence, however, to show that these procedures are of therapeutic value in selected cases. It is tempting to speculate that these surgical insults to the labyrinth, with the associated inflammatory and biochemica…

Diagnostic and therapeutic strategies for Meniere's disease of the Japan Society for Equilibrium Research.

OBJECTIVE: We provided diagnostic and therapeutic strategies for Meniere's disease in accordance with Japanese Clinical Practice Guideline of Meniere's disease and delayed endolymphatic hydrops 2nd ed. Tokyo: Kanehara Shuppan; 2020 edited by the Japan Society for Equilibrium Research. METHODS: The Committee for Clinical Practice Guidelines was entrusted with a review of the scientific literature on the above topic. Clinical Questions (CQs) concerning the treatment for Meniere's disease were produced, and the literature according to each of them including CQ was searched. The recommendations a

[Effects of inner ear immune responses on auditory function in guinea pigs].

Following direct challenge with KLH antigen primary or secondary (after systemic immunization) to the ES (endolymphatic sac) in guinea pigs, ECoG (electrocochleograms) were recorded from the round window induced by click and tone pips. The recordings were carried out on the 2nd, 7th and 21st days after local antigen challenge. There were no abnormal findings in the ECoG of the primary antigen challenged animals. On the other hand, prolongation of latencies, decrease in amplitudes of APs (compound action potentials) and increases in SP/AP ratios were observed in the 2nd day recordings of the se…

Meniere's disease.

Meniere's disease is a chronic illness that affects a substantial number of patients every year worldwide. The disease is characterised by intermittent episodes of vertigo lasting from minutes to hours, with fluctuating sensorineural hearing loss, tinnitus, and aural pressure. Although there is currently no cure, more than 85% of patients with Meniere's disease are helped by either changes in lifestyle and medical treatment, or minimally invasive surgical procedures such as intratympanic steroid therapy, intratympanic gentamicin therapy, and endolymphatic sac surgery. Vestibular neurectomy has…

Endolymphatic hydrops: can it be controlled?

While endolymphatic shunt procedures are conceptually logical, there are characteristics of underlying pathological conditions and host response that severely limit the probabilities of achieving functional shunts.

Revision of the endolymphatic subarachnoid shunt for Meniere's disease. Review of 59 cases.

Members of the Otologic Medical Group, Inc, Los Angeles, diagnose Meniere's disease in patients with fluctuating hearing loss, fluctuating tinnitus, episodic vertigo, and fluctuating pressure symptoms in the ear. Treatment is vasodilation. If medical treatment fails, an endolymphatic subarachnoid shunt operation is performed. If the shunt relieves symptoms for three or more months but symptoms then recur, revision of the shunt is recommended. If the revision surgery also fails, a destructive procedure is suggested.
