Systemic inflammation has been suggested to have a pivotal role in atherothrombosis, but the factors that trigger systemic inflammation have not been fully elucidated. Pattern recognition receptors (PRRs) on host cells detect pathogens to activate innate immunity which, in turn, initiates inflammatory and adaptive immune responses. Successful activation of PRRs is, therefore, critical to controlling infections and driving pathogen-specific adaptive immunity, but overactivity of PRRs causes systemic inflammation, which is detrimental to the host. Here we review the PRR literature as it relates to horses and speculate on the role PRRs may play in sepsis and endotoxaemia.

Endotoxemia and bacterial infection are frequent in patients with cirrhosis. They alter systemic and splanchnic hemodynamics, worsen coagulation disorders, impair liver function and thus may induce variceal bleeding. In variceal bleeding, bacterial infection favours failure to control bleeding, early rebleeding, and death. In patients with cirrhosis and variceal bleeding, antibiotic-prophylaxis decreases bacterial infection and the incidence of early rebleeding, and, more important, significantly decreases the death rate in these patients.

The origins of sepsis-induced hyperlactatemia are still imperfectly understood and probably multifactorial, resulting both from an increased production by various tissues through aerobic and anaerobic glycolysis, and from a decreased lactate clearance. In the previous issue of Critical Care, Michaeli and colleagues showed that lactate elevation during mild endotoxemia is due to an increased aerobic production that does not take place in the muscle; other tissues/cells may thus be important contributors.

Possible role of gram-negative bacteria in the genesis of hepatic damage during chronic viral hepatitis B and C was studied. Histological activity, hepatocyte proliferative activity, and expression of alpha-smooth muscle actin were compared with the severity of endotoxemia in patients with chronic hepatitis B and C. The study revealed a correlation between the degree of intralobular necroses, hepatocyte proliferation, and myofibroblast transdifferentiation of Ito cells with endotoxin concentration in the blood.
