Blinded by the depths: A case of acute loss of vision in a scuba diver.

Decompression illness (DCI) is a well-known medical emergency among scuba divers, aviators, astronauts, and compressed-air workers. It includes two diseases: decompression sickness (DCS) and arterial gas embolism (AGE). Vision changes are one of the symptoms that can happen in patients with DCI, but acute complete loss of vision is extremely rare. We report a 28-year-old male patient, a novice scuba diver who presented to our hospital with complete loss of vision while he was ascending from 18 m depth. The diagnosis of AGE was made depending on the history, clinical examination, and radiologic

Frontal Sinus Fractures.

Management of frontal sinus fractures is controversial with no universally accepted treatment protocol. Goals of management are to correct aesthetic deformity, preserve sinus function when it is deemed salvageable, prevent sequela related to the injury, and minimize complications associated with intervention. Studies suggest that frontal sinus injuries, including disruption of the nasofrontal outflow tract (NFOT), can be managed nonoperatively in many cases. Advances in the utilization of endoscopic techniques have led to an evolution in management that reduces the need for open procedures, wh

Investigation of the Unstable Total Hip Arthroplasty.

The investigation of recurrent instability after total hip arthroplasty requires a thorough history and physical examination focused on characterizing the dislocation events and the patient factors that contribute to instability. Radiographic studies include plain radiographs, in particular a cross-table lateral, and computed tomography to accurately assess the implant orientation relative to the patient's native anatomy. More advanced imaging such as magnetic resonance imaging may be useful to assess abductor tendon integrity. A classification system, based on etiology of recurrent instabilit

Syncope: Approaches to Diagnosis and Management.

Syncope is defined as a transient loss of consciousness due to cerebral hypoperfusion followed by spontaneous recovery. Common causes of syncope include vasovagal syncope, situational syncope, orthostatic hypotension, carotid sinus hypersensitivity, left- and right-sided obstructive cardiac lesions, and cardiac arrhythmias. History and physical examination often provide valuable clues about the underlying etiology of syncope. Admission decisions in the emergency department can be guided by various risk prediction scores. Evaluation of a patient with syncope involves a large battery of diagnost

One swallow does not a summer make: safety and renal denervation.

Renal denervation is increasingly being adopted as a treatment option in patients with resistant hypertension. The long-term safety of this procedure is unknown. Though the procedure interrupts the sympathetic nerves at the renal level, it also has effects on other organ beds, notably the heart and vasculature. These effects have been purported to be clinically beneficial and thus formed a rationale for examining the role of renal denervation in other conditions, including heart failure, arrhythmia, obstructive sleep apnoea and the metabolic syndrome. There is a theoretical concern that attenu

Toxic Anterior Segment Syndrome following Phacoemulsification Secondary to Overdose of Intracameral Gentamicin.

Objective. To report a case of toxic anterior segment syndrome (TASS) that was caused by inadvertent anterior chamber and cornea stromal injection with high dose gentamicin following cataract surgery. Methods. Case report. Results. We report a 72-year-old female patient who developed TASS that was caused by high dose gentamicin (20 mg/0.5 mL), which was inadvertently used during the formation of the anterior chamber and hydration of the corneal incision. Unlike previous cases, hyphema and hemorrhagic fibrinous reaction were seen in the anterior chamber. Despite treatment, bullous keratopathy d

Amperometric determination of cyanides at the low ppb level by automated preconcentration based on gas diffusion coupled to sequential injection analysis.

A simple, sensitive method for determining free cyanides is described. The assay is based on automated gas diffusion of the analyte using sequential injection analysis (SIA) coupled to amperometric detection on a silver working electrode. The effects of varying several parameters affecting the analytical procedure (including the flow rates of the donor and acceptor streams, the concentrations of the reagents and the sample volumes) were studied. The validity and quality of the method were also assessed, by examining its linearity, limits of detection and quantitation, precision, selectivity to

Contact forces in the subacromial space: effects of scapular orientation.

The purpose of this study was to examine the effects of scapular orientation on clearance in the subacromial space. Eight glenohumeral joints from fresh-frozen human cadavers were secured to an Instron mechanical testing machine via a custom-made translation table. Forces were applied to simulate rotator cuff and deltoid contraction. Superior translation of the glenohumeral joint was simulated, and the distance before the development of significant subacromial contact force was measured. Specimens were tested at varying orientations of scapular posterior tilting, upward rotation, and external

Radial nerve injury after general anaesthesia in the lateral decubitus position.

A 43-year-old female patient underwent pyelolithotomy in the left lateral decubitus position. Her upper right arm was placed on a padded armboard. Surgery lasted for 240 min. Postoperatively, she complained of numbness of the dorsal part of her right hand and wrist drop. Neurological examination revealed hypoaesthesia of the dermatome of the right forearm and hand innervated by the radial nerve. Electromyography revealed advanced axonal degeneration of the radial nerve below the level of the elbow. Treatment with diclofenac, vitamin B and physiotherapy was started. Her symptoms improved gradua

The protective effect of social engagement on 1-year mortality in a long-stay nursing home population.

We examined the effect of social engagement (SE) on mortality in 30,070 long-stay nursing home residents who were > or =65 years of age and did not have a serious communication problem. Information on SE and resident characteristics were obtained from the Minimum Data Set, and death information was obtained from the National Death Index. Life table analyses show that greater levels of SE are associated with longer survival (P=.0001). Adjusted proportional hazards regression results show that for each increase in the SE scale, residents are 0.94 (range 0.92-0.95) times as likely to die during t

[The spiral CT of cervical lymph node enlargements. The initial clinical results].

Spiral CT was performed before treatment in 35 patients with suspected cervical lymph node enlargement. By coronary and sagittal reconstruction it is possible to utilise the M/Q quotient which has become accepted as the result of sonographic and MRI examinations. It is now possible to obtain high diagnostic accuracy (97%) for distinguishing between reactive change from metastatic infiltration of lymph nodes (> 8 mm) by using CT. Using spiral CT with 5 mm table movement, three patients diagnosed as stage N1 by axial CT were shown to be N2b. This represents a highly sensitive and highly specific

Factors influencing the age at natural menopause.

To examine sociodemographic, menstrual, reproductive, and other factors which may influence the age at natural menopause, the authors analyzed data from a large series of women participating in a nationwide breast cancer screening program conducted between 1973 and 1980. Standard life table techniques permitted assessment of factors suspected of varying the time to menstrual cessation among 983 premenopausal women, 1091 surgically menopausal women, and 1423 naturally menopausal women. The median age at natural menopause was 51.1 years. Multivariate analysis indicated that parity, irregularity

Case 346: γ-Aminobutyric Acid A Receptor Encephalitis.

A 66-year-old man with a history of hypertension presented with left-sided jaw jerking, repetitive movements, aphasia, and dysarthria. He was afebrile and in no acute distress, with mild hypertension (153/81 mm Hg). Routine laboratory studies were unremarkable. Brain MRI showed multiple nonenhancing supratentorial cortical and subcortical hyperintensities on T2-weighted fluid-attenuated inversion recovery (FLAIR) images, which were nonspecific for infectious, inflammatory, vascular, or neoplastic etiologies. CT of the chest, abdomen, and pelvis and MRI of the cervical and thoracic spine were u

[Screening for glaucoma by blue light-pattern ERG and OCT--are the methods comparable?].

BACKGROUND: Up to now, several examinations are necessary to identify early glaucoma. It is also possible to analyse the peripapillary retinal nerve fibre layer thickness by means of OCT. The loss of retinal ganglion cells can be measured by pattern ERG, therefore this method could be used for the detection of early glaucoma. This study compares the results of the blue light-pattern ERG with the Stratus OCT for glaucoma early recognition. PATIENTS AND METHODS: We studied 30 healthy test persons, 20 patients with glaucoma and 20 patients with suspected glaucoma. Each group was examined using e

Nucleoside transport in normal and neoplastic cells.

The permeation of nucleosides across the plasma membrane of mammalian cells is complex and mediated by at least five distinct transporters that differ in their sensitivity to inhibitors and in their specificity for nucleosides. The basic properties and permeant specificity of these transporters are summarized in Table 3. It appears that there may be differences in the distribution of these transporters in tumors and normal tissues that might be exploited for chemotherapeutic purposes. The human tumor cell lines examined express predominantly the NBMPR-sensitive equilibrative transporter es whi

Beyond deficits: which executive functions link to which NSSI behaviors? A network analysis.

BACKGROUND: Executive function deficits have been demonstrated to be closely associated with non-suicidal self-injury (NSSI) among adolescents. However, no studies to date have explored how the dimensions of executive function relate to specific NSSI behaviors in a joint framework. This study aimed to examine the relationships between the two constructs at a fine-grained level and identify the central nodes and bridge nodes of their relationships using network analysis. METHODS: In this cross-sectional study, 1,300 Chinese adolescents completed the Adolescent Executive Function Scale to asses

Too young to die: social inequalities and infant mortality in marginalized Roma communities in Slovakia.

BACKGROUND: In Slovakia, several tens of thousands of persons live in extremely poor living conditions in segregated Roma settlements. We can hardly find populations with such a short life expectancy and a high risk of death in infancy anywhere in Europe. The inadequate infrastructure, catastrophic housing conditions, deteriorated environmental quality, high unemployment and dependence on social transfers in combination with social and geographical segregation or negative behavioral aspects markedly affect their health status and mortality rates. Despite this, only little attention has hithert

Hoist failure during recovery transfer in an anaesthetised horse: a near miss analysed through a Safety-II lens.

A 660 kg, 11-year-old Irish Sport Horse gelding underwent general anaesthesia for an out-of-hours emergency exploratory laparotomy. After approximately 5 hours of anaesthesia, transfer to the recovery box began, using a ceiling-mounted hoist. As the horse crossed the theatre-recovery interface, the hoist failed due to mechanical disengagement. This resulted in loss of electrical power, leaving the anaesthetised horse suspended approximately 150 cm above the floor for approximately 10 minutes. The horse could not be advanced, reversed, or lowered using standard controls. Activity was paused, an

Feasibility and reproducibility of handheld and table-mounted optical coherence tomography in children with craniosynostosis.

BACKGROUND: Optical coherence tomography (OCT) can be a valuable tool for non-invasively monitoring the optic nerve status in children with craniosynostosis. However, it is currently unknown whether optic nerve parameters derived from handheld OCT are comparable to those derived from table-mounted OCT, which is more widely used. This study aims to assess the feasibility and reproducibility of handheld and table-mounted OCT in craniosynostosis. METHODS: This was a cross-sectional study conducted at Great Ormond Street Hospital (GOSH), London. Twenty children aged 4-18 years with a clinical/gen
