[Replantation or prosthesis: individual planning].

Rehabilitation of a replanted upper limb is difficult and has to start close to the operation. Physical and occupational therapy are working close together. The unimpaired opposite upper extremity must be trained as well. Replantation of upper limb causes not only a lot of reoperations with extended morbidity, but also problems in social, family and psychic. After eventual amputation of the upper limb prosthetic compensation is necessary as soon as possible. There is a gap open between the technical and financial possibilities. To avoid spine disorders a shoulder cap is necessary if prosthetic

A family with multiple instances of definite, probable and possible early-onset Alzheimer's disease.

A family with a multigenerational history of proven or suspected early-onset Alzheimer's disease (AD) consistent with autosomal-dominant inheritance is described. To date, the pedigree comprises five generations in which there are 13 known affected individuals. The mean age of onset of cognitive deficits in those for whom data are available (n = 11) is 47.6 (s.d. 3.0) years and the mean age of death (n = 10) is 58.8 (s.d. 4.0) years. The variability in the extent and quality of available data illustrates the diagnostic difficulties encountered in ascertaining such an extended pedigree, and the

Nucleotide sequence of the androgen-dependent arginine esterase mRNA of canine prostate.

The nucleotide sequence of canine prostate arginine esterase mRNA was determined using a 400 bp cDNA clone and primer-extended cDNA transcripts for the 5'-coding and noncoding regions. The mRNA contains 864 nucleotides encoding a protein of 236 amino acids preceded by 24 amino acids which constitutes both the signal and the zymogen peptides. The sequence indicates the presence of one potential glycosylation site. A high degree of homology was found between the canine enzyme and other members of the kallikrein family including human prostate specific antigen. The protein appears to be specified

The role of family burden on informant discrepancies between parents and youths with protracted recovery from mild traumatic brain injury.

Previous literature shows that family burden can lead to symptom-report discrepancies between parents and children. The present study sought to extend this research by investigating the influence of family burden factors, including socioeconomic status (SES) and family stress on informant discrepancies between parents and youths with mild traumatic brain injury (mTBI). Participants were clinically referred youths with mTBI ages 8-17, consecutively seen in a hospital-based neuropsychology concussion clinic (N = 81; females = 54.3%). Parents and children completed the Behavioral Assessment for C

Recruiting participants for palliative care research: A reflective discussion paper.

BackgroundThis discussion paper provides a reflection on ethical aspects of participant recruitment experiences during one research (PhD) project in the Republic of Ireland (ROI). Using Gibbs' framework for reflection, this paper examines these experiences. The research (PhD) project that informed this reflection aimed to understand the experiences of family caregivers when caring for family members at the end of their life, through recorded interviews in the home, within the context of palliative care provision. Ethical approval had been granted for the main study; however, this paper does no

Family communication with teens at clinical high-risk for psychosis or bipolar disorder.

Previous research has found that family problem-solving interactions are more constructive and less contentious when there is a family member with bipolar disorder compared with schizophrenia. The present study extended this research by examining whether family problem-solving interactions differ between clinical high-risk (CHR) stages of each illness. Trained coders applied a behavioral coding system (O'Brien et al., 2014) to problem-solving interactions of parents and their adolescent child, conducted just prior to beginning a randomized trial of family-focused therapy. The CHR for psychosis

Inheritance Patterns of Infantile Hemangioma.

BACKGROUND AND OBJECTIVE: Infantile hemangioma (IH) includes, among its other risk factors, familial clustering, but a definitive understanding of IH's inheritance model and genetic basis is lacking. Our objective was to collect IH pedigrees in Finland, to study the inheritance patterns of IH within these families, and to analyze the characteristics of familial IHs. METHODS: We identified 185 patients with IH who visited our vascular anomaly clinic between 2004 and 2007. Based on hospital records and a questionnaire sent to these patients and their families, IH characteristics and family hist

Deletion of exons 1-3 of the MEN1 gene in a large Italian family causes the loss of menin expression.

Multiple endocrine neoplasia type 1 (MEN1) syndrome is an autosomal dominant disease, characterized by parathyroid adenomas, endocrine gastroenteropancreatic tumors and pituitary adenomas, due to inactivating mutations of the MEN1 gene (chromosome 11q13). MEN1 mutations are mainly represented by nonsense, deletions/insertions, splice site or missense mutations that can be detected by direct sequencing of genomic DNA. However, MEN1 patients with large heterozygous deletions may escape classical genetic screening and may be misidentified as phenocopies, thereby hindering proper clinical surveill

Progressive myoclonic epilepsy as an adult-onset manifestation of Leigh syndrome due to m.14487T>C.

BACKGROUND: m.14487T>C, a missense mutation (p.M63V) affecting the ND6 subunit of complex I of the mitochondrial respiratory chain, has been reported in isolated childhood cases with Leigh syndrome (LS) and progressive dystonia. Adult-onset phenotypes have not been reported. OBJECTIVES: To determine the clinical-neurological spectrum and associated mutation loads in an extended m.14487T>C family. METHODS: A genotype-phenotype correlation study of a Belgian five-generation family with 12 affected family members segregating m.14487T>C was carried out. Clinical and mutation load data were avail

Multiple endocrine neoplasia type 1 in Brazil: MEN1 founding mutation, clinical features, and bone mineral density profile.

OBJECTIVE: Only few large families with multiple endocrine neoplasia type 1 (MEN1) have been documented. Here, we aimed to investigate the clinical features of a seven-generation Brazilian pedigree, which included 715 at-risk family members. DESIGN: Genealogical and geographic analysis was used to identify the MEN1 pedigree. Clinical and genetic approach was applied to characterize the phenotypic and genotypic features of the family members. RESULTS: Our genetic data indicated that a founding mutation in the MEN1 gene has occurred in this extended Brazilian family. Fifty family members were

Severe Childhood Autism: The Family Lived Experience.

This research examined the experiences of families living with a child with severe autism. There is limited literature on the experiences of families when a child has severe autism as distinct from milder autism and includes the voices of multiple family members. Van Manen's phenomenological approach was used for data collection and analysis. This approach allowed for the use of innovative data sources, including unstructured individual and family interviews, observations, and family lifelines (a pictorial, temporal picture with comments of the families lives). This study included 29 interview

Silent escalation: The journey of Shigella from gut to blood.

Shigella are Gram-negative non motile facultative anaerobic bacteria belonging to family Enterobacterales. They are a leading cause of diarrhoea and dysentery and constitute a significant public health problem in both the developing and developed nations. In Asia alone, 125 million infections and 14,000 deaths are estimated to be due to shigellosis annually. Bacteremia caused by Shigella spp is rare. We hereby present a case of bloodstream infection caused by fluoroquinolone resistant, extended spectrum beta lactamase producing Shigella flexneri strain in a five-year-old malnourished child tha

Losses experienced by children alienated from a parent.

Parental alienation occurs when a child aligns with one parent and unjustifiably rejects the other as a result of parental alienating behaviors. This article provides an overview of current research and theory regarding the losses alienated children endure. Parental alienating behaviors alter the child's beliefs, perceptions, and memories of the alienated parent, triggering a cascade of profound losses for the child. These losses include loss of individual self, childhood experiences, extended family, community, and activities and relationships essential for healthy development. Consequently, …

Robinow syndrome: phenotypic variability in a family with a novel intragenic ROR2 mutation.

Robinow syndrome comprises dysmorphic facial features, short stature, brachymesomelia, segmental spine defects, and genital hypoplasia. The range of severity in this disorder is broad. We report on the clinical and molecular findings of two sib pairs from the same extended family with Robinow syndrome due to a novel intragenic ROR2 deletion involving exons 6 and 7 that could not be detected by sequencing. The affected individuals exhibited variability with respect to the cleft lip, cleft palate, and cardiac findings and for the presence in one of the patients of syringomyelia, which has not be…

Aetiology of severe iron overload in a family with hereditary haemolytic anaemia.

Severe iron overload is a reported complication of certain erythroid disorders which are characterized by increased erythropoietic activity. Proposed mechanisms include enhancement of iron absorption secondary to increased erythroid activity and coexistent heterozygosity or homozygosity for haemochromatosis. We performed PCR-based analysis for the haemochromatosis-related HFE C282Y mutation in an extended family with inherited haemolytic anaemia in which several members exhibited iron overload. The results demonstrated iron overload was associated with homozygosity but not heterozygosity for t…

Mystery(n) Phenotypic Presentation in Europeans: Report of Three Further Novel Missense RNF213 Variants Leading to Severe Syndromic Forms of Moyamoya Angiopathy and Literature Review.

Moyamoya angiopathy (MMA) is a rare cerebral vasculopathy in some cases occurring in children. Incidence is higher in East Asia, where the heterozygous p.Arg4810Lys variant in RNF213 (Mysterin) represents the major susceptibility factor. Rare variants in RNF213 have also been found in European MMA patients with incomplete penetrance and are today a recognized susceptibility factor for other cardiovascular disorders, from extracerebral artery stenosis to hypertension. By whole exome sequencing, we identified three rare and previously unreported missense variants of RNF213 in three children with
