Ewing Sarcoma of Larynx: A Rare Case in a 5-Year-Old Boy.

Ewing Sarcoma of the head and neck region is an extremely rare entity. Treatment usually involves surgery, chemotherapy and radiotherapy in varying sequences. We present the third case to date of Ewing sarcoma of the larynx in a paediatric population. A 5-year-old boy presented to emergency room with acute respiratory distress. Computerized tomography scan showed a mass in the supraglottis; he was intubated using videolaryngoscope and tracheostomy was avoided, mass was removed by cold dissection. Final histopathologic examination revealed Ewing sarcoma. Further workup showed no systemic metast

An incidental finding of an asymptomatic intraneural glomus tumor: A case report and review of the literature.

Glomus tumors are rare, soft-tissue neoplasms arising from the thermoregulatory neuromyoarterial glomus bodies. They are commonly observed in the extremities and typically present with symptoms of cold hypersensitivity, pain and localized tenderness. Intraneural glomus tumors (INGTs) are even rarer. Here we review the literature on INGT and present an unusual case of an asymptomatic INGT, found incidentally within the excision specimen of a spiradenocarcinoma that arose near the natal cleft. Interestingly, this had not been identified on magnetic resonance imaging (MRI) used to investigate the

Relapsing polychondritis-associated Takayasu's arteritis.

We report a 40-year-old woman with saddle shaped-nose and bilateral deformed "cauliflower" ears who was admitted to our clinic because of arthralgia, fatigue, weakness and coldness in both arms, recurrent swelling and pain of the nose and both auricles. On physical examination, bruits were heard over both carotid arteries and blood pressure was not measurable in both upper extremities. Conventional angiography revealed total occlusion of left common carotid, right internal carotid, right subclavian, right renal and bilateral vertebral arteries. Relapsing polychondritis and associated Takayasu'

A case report of perioperative managements for a patient with gastric cancer and cold agglutinin syndrome.

RATIONALE: Gastric cancer patient with cold agglutinin syndrome (CAS) is an extremely rare entity. This kind of patients is very sensitive to the environment, and they always need scrupulous perioperative treatment, however the experience of perioperative treatment for these patients has been seldom reported. PATIENT CONCERNS: The patient was a 54-year-old male. He suffered diarrhea for 3 months, and later the gastroscopy found a tumor located in the gastric antrum and the biopsy was performed. The pathological result reported that it was poorly differentiated gastric adenocarcinoma. The pati

Effects of beraprost sodium (Dorner) in patients with diabetes mellitus complicated by chronic arterial obstruction.

The effects of the prostaglandin I2 derivative beraprost sodium (Dorner) on ankle pressure index (AP; ankle joint-to-upper extremity systolic pressure ratio), subjective symptoms, and intermittent claudication were investigated in diabetic patients with arteriosclerosis obliterans (ASO). Forty patients (25 men and 15 women), mean age 63.9 years, were enrolled in this study. ASO was grade I in 30 patients, grade II in seven, grade III in one, and grade IV in two according to the Fontaine classification. They were administered six tablets (20 microg/tablet) of beraprost sodium daily for 6 months

Short-term sleep deprivation and human thermoregulatory function during thermal challenges.

NEW FINDINGS: What is the topic of this review? It is generally accepted that sleep deprivation constitutes a predisposing factor to the development of thermal injury. This review summarizes the available human-based evidence on the impact of sleep loss on autonomic and behavioural thermoeffectors during acute exposure to low and high ambient temperatures. What advances does it highlight? Limited to moderate evidence suggests that sleep deprivation per se impairs thermoregulatory defence mechanisms during exposure to thermal extremes. Future research is required to establish whether inadequate

Peripheral neuropathy in scleroderma.

Because patients with scleroderma report neuropathic symptoms including numbness, paresthesias, and dysesthesias, we assessed peripheral nerve function in such patients. Fourteen scleroderma patients underwent complete neurologic examination, nerve conduction studies (NCS) and quantitative sensory testing (QST). Neurologic examination revealed reduced vibration (7) or pinprick (4) sensation in the upper or lower extremities, focal atrophy or proximal weakness (2), and decreased deep tendon reflexes (2). NCS showed reduced sensory nerve action potentials (1) and carpal tunnel syndrome (1). QST

Effect of the interaction between outdoor air pollution and extreme temperature on daily mortality in Shanghai, China.

BACKGROUND: Both outdoor air pollution and extreme temperature have been associated with daily mortality; however, the effect of their interaction is not known. METHODS: This time-series analysis examined the effect of the interaction between outdoor air pollutants and extreme temperature on daily mortality in Shanghai, China. A generalized additive model (GAM) with penalized splines was used to analyze mortality, air pollution, temperature, and covariate data. The effects of air pollutants were stratified by temperature stratum to examine the interaction effect of air pollutants and extreme

Altered Functional Connectivity and Sensory Processing in Blepharospasm and Hemifacial Spasm: Coexistence and Difference.

Background: Blepharospasm (BSP) and hemifacial spasm (HFS) are both facial hyperkinesia however BSP is thought to be caused by maladaptation in multiple brain regions in contrast to the peripherally induced cause in HFS. Plausible coexisting pathophysiologies between these two distinct diseases have been proposed. Objectives: In this study, we compared brain resting state functional connectivity (rsFC) and quantitative thermal test (QTT) results between patients with BSP, HFS and heathy controls (HCs). Methods: This study enrolled 12 patients with BSP, 11 patients with HFS, and 15 HCs. All sub

Raynaud syndrome.

Raynaud syndrome (RS) was first described by the French physician Maurice Raynaud in 1862 with the characteristic tricolor change featuring pallor (ischemic phase), cyanosis (deoxygenation phase), and erythema (reperfusion phase) induced by cold or stress. Although the underlying pathophysiological mechanism is unclear, alterations in activity of the peripheral adrenoceptor have been implicated, specifically an enhanced smooth muscle contraction due to overexpression or hyperactivity of postsynaptic alpha 2 receptors. There are 2 ways that RS can appear clinically; isolated, formerly referred

The physical assessment of the patient with arterial disease.

Utilizing techniques of inspection, palpation, and auscultation the complete arterial system of a patient should be examined when arterial insufficiency is suspected. Examination of pulses of the upper part of the body is important not only in localizing disease but also in indicating diffuse disease. Although described separatley in this article, the examination of the arterial system can be integrated in a complete physical examination when the examiner is assessing the extremities, the abdomen, and the cardiovascular system.

A case of solitary angiokeratoma circumscriptum of the tongue.

Angiokeratoma circumscriptum is an uncommon vascular entity. The lesion is present at birth and usually involves the lower extremities. A 9-year-old girl presented with multiple pink-colored, small-raised lesions in the dorsal part of her tongue, which had been present since birth. There was no history of trauma, cold injury, or similar lesions elsewhere in her body. She complained of pain and rare bleeding from the lesions after eating hard foods. Physical examination showed multiple, grouped, erythematous, shiny papules mostly in the dorsal aspect of the tongue. There were no other lesions i

Physiological responses of insular wild black rat (Rattus rattus) to natural infection by the digenean trematode Fasciola hepatica.

Wild black rat Rattus rattus is regularly infected by the liver fluke Fasciola hepatica on Corsica. This report constitutes the only example of a murid rodent that plays an important epidemiological role for the Fasciolosis. We investigated the influence of such unusual parasite infection on black rat physiology by measuring its oxygen consumption at different ambient temperatures. Black rat energy requirements are influenced by body mass, temperature of the experiment and parasite infestation. The influence of the presence of F. hepatica was more pronounced for cold temperatures. The mean inc

Coexistence of erythromelalgia and Raynaud's phenomenon.

Erythromelalgia is characterized by spontaneous recurrent episodes of redness, heat, and pain of the extremities that can be triggered or worsened by heat. Raynaud's phenomenon occurs in response to cold exposure and presents as pallor of the fingers or toes, often followed by cyanosis and rubor. Although the 2 conditions may appear to be opposites in symptomatology and clinical presentation, there are very rare reports of their coexistence. A case of coexistent erythromelalgia and Raynaud's phenomenon is presented. The pathophysiology is reviewed to elucidate a common mechanism underlying som

[A case report: abdominal aorta-femoral artery bypass grafting in patient with essential thrombocythemia].

A 58-year-old man was admitted to our hospital suffering from intermittent claudication and angiogram revealed complete obstruction of the left common iliac artery. The essential thrombocythemia was diagnosed from platelet count over 1,000,000/mm3 as well as bone marrow finding. After the platelet count was reduced into normal range by administration of ranimustine (MCNC), abdominal aorta-femoral artery bypass grafting using 8mm-diameter Gelseal graft was successfully performed without any complications such as thrombus formation or bleeding. Coldness and cyanotic discoloration of the lower ex
