Evaluation of a computer-assisted learning programme on the oro-facial signs of child physical abuse (non-accidental injury) by general dental practitioners.

A computer-assisted learning programme with tutorials and self assessment multiple choice questions has been developed. One hundred and two general dental practitioners were asked to evaluate the programme. Over 80% of respondents felt that the programme was easy to use, contained an appropriate level of supporting information, and had improved their knowledge of non-accidental injury. All users of the programme felt that it was a better way of learning than video, audio tapes, and journals or books on the subject. It is concluded that CAL programmes are worthwhile for providing continuing pro

Comparison between VII-to-VII and XII-to-VII coaptation techniques for early facial nerve reanimation after surgical intra-cranial injuries: a systematic review and pooled analysis of the functional outcomes.

The surgical injury of the intracranial portion of the facial nerve (FN) is a severe complication of many skull base procedures, and it represents a relevant issue in terms of patients' discomfort, social interactions, risk for depression, and social costs. The aim of this study was to investigate the surgical and functional outcomes of the most common facial nerve rehabilitation techniques. The present study is a systematic review of the pertinent literature, according to the PRISMA guidelines. Two different online medical databases (PubMed, Scopus) were screened for studies reporting the fun

Facial emotion recognition deficits following moderate-severe Traumatic Brain Injury (TBI): re-examining the valence effect and the role of emotion intensity.

UNLABELLED: Many individuals who sustain moderate-severe traumatic brain injuries (TBI) are poor at recognizing emotional expressions, with a greater impairment in recognizing negative (e.g., fear, disgust, sadness, and anger) than positive emotions (e.g., happiness and surprise). It has been questioned whether this "valence effect" might be an artifact of the wide use of static facial emotion stimuli (usually full-blown expressions) which differ in difficulty rather than a real consequence of brain impairment. This study aimed to investigate the valence effect in TBI, while examining emotion

Local delivery of recombinant human bone morphogenetic protein-2 increases axonal regeneration and the expression of tau protein after facial nerve injury.

This study explored the function and possible mechanism of bone morphogenetic protein-2 (BMP-2) in the healing of injured peripheral nerves in vivo. Rabbit facial nerves were injured by clamping and then treated with recombinant human BMP-2 (rhBMP-2) or phosphate-buffered saline (control) by injecting once during surgery and twice a day post-injury for 7 days. Facial nerve fragments within 5 mm of the clamping point were examined at different times post-surgery. Axon structures visualized by Bielschowsky staining were similar in experimental and control nerves 2 and 6 weeks post-injury. At 4 w

An analysis of time-dependent changes of neurotrophic factors (BDNF, CNTF) in traumatic facial nerve injury of a nerve-cut and nerve-crush model in rats.

HYPOTHESIS: To investigate neurotrophic changes in the injured facial nerve in rats. BACKGROUND: Time-dependent follow-up of brain-derived neurotrophic factor (BDNF) and ciliary neurotrophic factor (CNTF) concentrations in a standardized nerve-cut and nerve-crush trauma model could present valuable information about estimation of the extent of the injury. METHODS: Ninety Wistar rats were grouped into permanent (transection: nerve-cut) and transient (compression: nerve-crush) nerve injury and analyzed for neurotrophic changes at Week 1 and after 1, 3, and 6 months. Ten undissected rats served

Dual-Modality Bioactive Repair of Facial Nerve Injury Using PEG-Mediated Axonal Fusion and Extracellular Vesicles-Enriched Human GMSC Secretome.

Facial nerve injuries may cause severe functional deficits with limited recovery after standard neurorrhaphy. Polyethylene glycol (PEG)-mediated axonal fusion enables immediate structural reconnection and prevents Wallerian degeneration, while gingiva-derived mesenchymal stem cell (GMSC) secretome provides a potent, acellular trophic stimulus that supports neuronal survival and remyelination. We report a dual-modality bioactive repair interface integrating PEG fusion with human GMSC secretome to accelerate neural repair. Fluorescently labeled secretome components were rapidly internalized by n

Pediatric Maxillofacial Fractures: A 10-Year Retrospective Analysis of 549 Cases With Emphasis on Facial Injury Severity Score Validation and COVID-19 Impact.

BACKGROUND: Pediatric maxillofacial fractures (PMFs) represent a distinct clinical entity due to their unique features. However, objective tools that inform surgical decision-making and stratify risk in PMF remain limited. In particular, the clinical utility of the Facial Injury Severity Score (FISS) in predicting management strategies and outcomes, as well as its behavior during extraordinary periods such as the COVID-19 pandemic, remains unclear. PURPOSE: The purpose of this study was to measure the association between FISS and surgical intervention in PMF, including shifts during the COVID

The Effect of Body Expressions on the Learning Process and Facial Recognition among Healthy Participants and Individuals with Traumatic Brain Injury: Examination Using Eye Movements.

OBJECTIVE: Context-dependent effect (CDE) is a process by which reinstating at test the original learning context enhances the recall ability of the material being studied. Although recognition by people with traumatic brain injury (TBI) is poorer than that of healthy controls, both groups show CDE equally. In the current study, we seek to test the effect of body emotional expressions as contextual information, on facial recognition, and eye movements. METHOD: Twenty-four healthy individuals and 27 patients with moderate-to-severe TBI participated in the study. Participants were exposed to ph

CXCL12 has therapeutic value in facial nerve injury and promotes Schwann cells autophagy and migration via PI3K-AKT-mTOR signal pathway.

Facial nerve injury is a clinically common disease accompanied by demyelination of damaged nerves. The remyelination of damaged nerves and the unsatisfactory function recovery are problems that have been plaguing people for a long time. The role that CXCL12 plays after facial nerve injury remains unknown. Our experiments found that the expression of CXCL12 was up-regulated in the early stage of facial nerve injury and decreased after two weeks. Further research found that CXCL12 had no effect on Schwann cells proliferation, apoptosis and cell cycle, while significantly promoted Schwann cells m

Using facial expressions as CSs and fearsome and disgusting pictures as UCSs: affective responding and evaluative learning of fear and disgust in blood-injection-injury phobia.

Two experiments examine use of an evaluative conditioning (EC) paradigm in the acquisition of fear and disgust responding to neutral facial expressions. In Experiment 1, 60 participants were randomly assigned to one of three evaluative learning conditions in which neutral facial expressions were paired with fearsome, disgusting, or neutral pictures. No statistically significant differences were detected between the three conditions. However, significant differences emerged within subjects as post-exposure of fear and disgust ratings were higher among expressions that were paired with pictorial

Appropriate dosage, timing, and site of intramuscular injections of brain-derived neurotrophic factor (BDNF) promote motor recovery after facial nerve injury in rats.

INTRODUCTION/AIMS: Daily intramuscular injections of fibroblast growth factor 2 (FGF2) but not of brain-derived neurotrophic factor (BDNF) significantly improve whisking behavior and mono-innervation of the rat levator labii superioris (LLS) muscle 56 days after buccal nerve transection and suture (buccal-buccal anastomosis, BBA). We explored the dose-response of BDNF, FGF2, and insulin growth factor 2 (IGF2) on the same parameters, asking whether higher doses of BDNF would promote recovery. METHODS: After BBA, growth factors were injected (30 μL volume) daily into the LLS muscle over 14, 28,

Have one's view of the important overshadowed by the trivial: chronic progressive external ophthalmoplegia combined with unilateral facial nerve injury: a case report and literature review.

Chronic progressive external ophthalmoplegia (CPEO) is a mitochondrial encephalomyopathy that is characterized by progressive ptosis and impaired ocular motility. Owing to its nonspecific clinical manifestations, CPEO is often misdiagnosed as other conditions. Herein, we present the case of a 34-year-old woman who primarily presented with incomplete left eyelid closure and limited bilateral eye movements. During the 6-year disease course, she was diagnosed with myasthenia gravis and cranial polyneuritis. Finally, skeletal muscle tissue biopsy confirmed the diagnosis. Biopsy revealed pathologic

Interleukin-6 (IL6) and cellular response to facial nerve injury: effects on lymphocyte recruitment, early microglial activation and axonal outgrowth in IL6-deficient mice.

Nerve injury triggers numerous changes in the injured neurons and surrounding non-neuronal cells. Of particular interest are molecular signals that play a role in the overall orchestration of this multifaceted cellular response. Here we investigated the function of interleukin-6 (IL6), a multifunctional neurotrophin and cytokine rapidly expressed in the injured nervous system, using the facial axotomy model in IL6-deficient mice and wild-type controls. Transgenic deletion of IL6 caused a massive decrease in the recruitment of CD3-positive T-lymphocytes and early microglial activation during th

A randomised controlled phase II trial to examine the feasibility of using hyper-oxygenated fatty acids (HOFA) to prevent facial pressure injuries from medical devices among adults admitted to intensive care-A research protocol.

One in three patients admitted to intensive care will sustain a pressure injury (PI) from a medical device. These injuries are painful and when on the face, head or neck they can result in permanent disfigurement. Preliminary evidence of the efficacy of hyper-oxygenated fatty acids (HOFAs) to prevent facial pressure injuries from medical devices is promising; however, the feasibility of incorporating HOFAs into current standard care to prevent PI from a medical device of the face, head and neck has not been extensively explored. It is intended that the findings from this phase II feasibility s

Alcohol-Only and Drug/Medication-Only Product-Associated Facial Injury Encounters Across COVID-19 Periods: A Public CPSC NEISS Study, 2019-2023.

Previous analyses of substance-involved craniofacial trauma have often combined alcohol and drug/medication involvement. We conducted a cross-sectional analysis comparing alcohol-only and drug/medication-only facial injury encounters in the public Consumer Product Safety Commission National Electronic Injury Surveillance System (NEISS), 2019-2023. Primary facial injuries (Body_Part = 76) were classified using the released Alcohol_Involved and Drug_Involved fields. Survey-weighted estimates and Taylor-linearized 95% confidence intervals incorporated the released PSU, stratum, and weight variabl…

Does a Modified Endaural Incision Reduce Facial Nerve Injury and Improve Cosmesis When Compared to the Modified Pre-Auricular Incision for Management of Temporomandibular Joint Ankylosis?

BACKGROUND: Facial nerve injury (FNI) and unesthetic scar are distressing complications of temporomandibular joint (TMJ) surgery. Insufficient evidence on whether a modified endaural incision can reduce FNI and serve as an aesthetic alternative is a concern. PURPOSE: The purpose of this study was to compare the postoperative FNI and surgical scar cosmesis using modified endaural incision (Inviscision approach [IA]) and modified preauricular incision (Alkayat-Bramley approach [ABA]) in TMJ ankylosis. STUDY DESIGN, SETTING, SAMPLE: The authors implemented a single-centre, retrospective, cohort

Primary Palate Reconstruction Combined With Cranio-Maxillo-Facial Fixation After Self-Inflicted Gunshot Wound Injury: Critical Incision Planning for Regional Flap Reconstruction.

Self-inflicted gunshot wounds (GSW) to the palate result in complex bony and soft tissue trauma to the mid and upper face. Patients who survive these injuries are faced with significant speech and feeding difficulties. Upper and midface fractures open reduction and internal fixation (ORIF) is required for many of these patients, and consideration to incision planning is critical in order to preserve a primary option for oroantral fistula repair. The temporoparietal fascia (TPF) flap is an excellent option for primary palate repair as it is often exposed in the operative field during facial fra

Effect of Ultrasound Therapy on Expression of Vascular Endothelial Growth Factor, Vascular Endothelial Growth Factor Receptors, CD31 and Functional Recovery After Facial Nerve Injury.

Functional recovery is provided by some neurotrophic factors released from the near vicinity of the injury site. Ultrasound treatment is known to increase neurotrophic factor expression. This study was aimed at determining the effect of ultrasound treatment on the expression of vascular endothelial growth factor (VEGF), its receptors and new vessel formation after facial nerve injury. Sixty-four Wistar rats were divided into four groups: control (group 1), sham (group 2), facial-facial coaptation (group 3), and facial-facial coaptation and ultrasound treatment (group 4). Recovery of whisking function promoted by manual stimulation of the vibrissal muscles after facial nerve injury requires insulin-like growth factor 1 (IGF-1).

Recently, we showed that manual stimulation (MS) of denervated vibrissal muscles enhanced functional recovery following facial nerve cut and suture (FFA) by reducing poly-innervation at the neuro-muscular junctions (NMJ). Although the cellular correlates of poly-innervation are established, with terminal Schwann cells (TSC) processes attracting axon sprouts to "bridge" adjacent NMJ, molecular correlates are poorly understood. Since quantitative RT-PCR revealed a rapid increase of IGF-1 mRNA in denervated muscles, we examined the effect of daily MS for 2 months after FFA in IGF-1(+/-) heterozyg
