CASE: An 18-year-old man with severe hemophilia A (without inhibitors) developed severe left knee deformity after an untreated injury, resulting in wheelchair dependence. He underwent knee arthrodesis under factor VIII cover but developed a postoperative hematoma and inhibitor formation. Following evacuation and treatment with activated prothrombin complex concentrate and recombinant factor VII, solid fusion and pain relief were achieved within 6 months. Surgery is a leading cause of major hemorrhage as well as of thrombosis unless patients are administered appropriate antithrombotic prophylaxis after their thrombo-hemorrhagic risk has been stratified. Therefore, thorough preoperative evaluation is essential to minimize surgical complications. In cases of incoercible bleeding, drugs such as desmopressin, synthetic antifibrinolytics or recombinant factor VII can be administered. To prevent postoperative thrombosis, low molecular weight heparins or pentasaccharide have been shown to significantly reduce the incidence of thromboembolism.

A variety of haemostatic agents can be helpful for patients with von Willebrand disease in case of bleeding. There are orally administered substances, such as tranexamic acid, oestrogens and prednisone; agents for topical or local use; and medicines for injection or infusion, including desmopressin, blood plasma products and recombinant activated factor VII. The choice depends on the type of haemorrhage, the severity of the disease, presence of inhibitors, availability and cost. Some of these aspects are reviewed here.
