[USEFULNESS OF FECAL INCONTINENCE CATHETER (Flexi-Seal®) FOR POSTOPERATIVE MANAGEMENT OF EVACUATION IN FOURNIER'S GANGRENE: TWO CASE REPORTS].

In patients with Fournier's gangrene, massive debridement is necessary. The debrided region is usually near the anus, which means that management of evacuation becomes difficult and a colostomy is often created for diversion of stool. Our two patients were a 72-year-old man with scrotal swelling and disturbance of consciousness and a 65-year-old man with scrotal swelling and fever. In both patients, CT scanning revealed air in the scrotal sac. We diagnosed Fournier's gangrene and operated on both patients immediately. After debridement, we used a fecal incontinence catheter, a Flexi-Seal® to k…

Fecal incontinence after moderately hypofractionated volumetric modulated arc therapy for prostate cancer: incidence and impact on quality of life.

PURPOSE: Fecal incontinence (FI) is a late toxicity consequence in prostate cancer patients treated with current radiotherapy schedules that remains poorly described and understood. The main objective of this work is to describe the incidence of appearance of FI in these patients, and to analyze whether this late radiotoxicity event affects the quality of life of survivors. METHODS: Prospective follow-up study was performed on a cohort of 91 patients with non-metastatic prostate cancer undergoing moderately hypofractionated volumetric modulated arc therapy (VMAT at 2.5-3 Gy/session) with imag…

Integrated low-intensity biofeedback therapy in fecal incontinence: evidence that "good" in-home anal sphincter exercise practice makes perfect.

BACKGROUND: Biofeedback therapy (BFT) is an established treatment for fecal incontinence (FI), with access often being restricted to tertiary centers due to resources and the perceived requirement for high-intensity regimes. However, the optimal regime remains unknown. We evaluated outcomes from our low-intensity integrated BFT program in a secondary care center. METHODS: Outcomes of our BFT service for FI were evaluated retrospectively. Response was defined by ≥50% improvement in FI frequency from baseline or complete continence. Responders were compared to non-responders for factors includi…

Fecal incontinence, sexual complaints, and anorectal function after third-degree obstetric anal sphincter injury (OASI): 5-year follow-up.

INTRODUCTION AND HYPOTHESIS: Our aim was to evaluate alterations in anorectal function after anal sphincteroplasty for third-degree obstetric anal sphincter injury (OASI) in relation to clinical outcome. METHODS: In this retrospective, descriptive, cross-sectional study conducted between 1998 and 2008, women with persisting fecal incontinence (FI) after 3a OASI and all women with grade 3b or 3c OASI were sent for anorectal function evaluation (AFE) consisting of anal manometry and endosonography 3 months after sphincteroplasty. In 2011, questionnaires regarding FI (Vaizey/Wexner), urinary inc…

Minimally invasive resection rectopexy as a treatment method for obstructive defecation (ODS): functional outcome in ODS; constipation and fecal incontinence.

BACKGROUND: Minimally invasive resection rectopexy is an effective treatment for obstructive defecation syndrome (ODS) in carefully selected patients, offering favorable functional outcomes with potentially reduced surgical trauma. As minimally invasive techniques continue to advance in colorectal surgery, integrating resection rectopexy with Natural Orifice Specimen Extraction (NOSE) or minilaparotomy retrieval may further enhance postoperative bowel function by minimizing constipation and fecal incontinence. We hypothesize that surgical outcomes are influenced by the severity of disease. ME…

Changes in Psychological States During Instrumented Biofeedback for Dyssynergic Defaecation and Fecal Incontinence Are Associated With Changes in Patient-Reported Outcomes.

BACKGROUND: Chronic constipation and fecal incontinence (FI) are severe and chronic conditions associated with reduced quality of life and distress for those affected. Instrumented anorectal biofeedback has demonstrated efficacy in both conditions and is often described as a learning process, suggesting efficacy is mediated via central cognitive and motivational pathways, but empirical evidence is lacking. The aim of this exploratory study was to seek evidence of change in psychological processes as central mediators of biofeedback treatment efficacy. METHODS: Patients (n = 135) were prospect…

Outcomes and efficacy of magnetic resonance imaging-compatible sacral nerve stimulator for management of fecal incontinence: A multi-institutional study.

BACKGROUND: Fecal incontinence (FI) is an involuntary passage of fecal matter which can have a significant impact on a patient's quality of life. Many modalities of treatment exist for FI. Sacral nerve stimulation is a well-established treatment for FI. Given the increased need of magnetic resonance imaging (MRI) for diagnostics, the InterStim which was previously used in sacral nerve stimulation was limited by MRI incompatibility. Medtronic MRI-compatible InterStim was approved by the United States Food and Drug Administration in August 2020 and has been widely used. AIM: To evaluate the eff…

Simple Anal Reinforcement with Anal Encirclement Using an Artificial Ligament in Patients with fecal Incontinence: A Single-center Observational Study.

OBJECTIVES: Surgical repair of anal sphincter defects in patients with fecal incontinence (FI) has been associated with excellent or good short-term results; however, its benefits have been shown to deteriorate over long-term follow-up. When sphincteroplasty fails or is not feasible, the subsequent surgical options are limited. This study aimed to evaluate the efficacy of anal encirclement using the Leeds-Keio ligament in patients with FI. METHODS: The inclusion criteria for the procedure were failure of or unsuitability for sphincteroplasty and the presence of a patulous anus (diameter, ≥35 …

Critical appraisal of international guidelines for the management of fecal incontinence in adults: is it possible to define what to do in different clinical scenarios?

Fecal incontinence (FI) is a complex often multifactorial functional disorder which is associated with a significant impact on patients' quality of life. There is a broad spectrum of symptoms, and degrees of severity and diverse patient backgrounds. Several treatment algorithms from different professional societies and experts are available in the literature. However, no consensus has been reached on several aspects of FI management. We performed a critical review of the most recently published guidelines on FI, emphasising the lack of consensus, highlighting specific topics mentioned in each …

The diagnostic value of a digital rectal examination compared with high-resolution anorectal manometry in patients with chronic constipation and fecal incontinence.

OBJECTIVES: Digital rectal examination (DRE) is a simple clinical method to diagnose anorectal disorders. High-resolution antorectal manometry (HRAM) based on a spatiotemporal plot is expected to promote improved diagnostic accuracy. However, there are no reports comparing the effectiveness of DRE and HRAM. The aim of our study was therefore to evaluate the diagnostic value of DRE compared with HRAM. METHODS: A total of 309 consecutive patients with chronic constipation (n=268) or fecal incontinence (n=41) who underwent a standardized DRE, HRAM, and balloon expulsion test were enrolled in thi…
