The Jarisch-Herxheimer reaction complicating syphilotherapy in pregnancy.

Thirty-three gravidas with syphilis were monitored with hourly vital signs and examinations for 24 hours after treatment with benzathine penicillin G. Fifteen (45%) of the subjects had a Jarisch-Herxheimer reaction, including all three, 12 of 20 (60%), and none of ten of those with primary, secondary, and latent syphilis, respectively. The most common symptoms were fever (73%), uterine contractions (67%), and decreased fetal movement (67%). The signs or symptoms began 2-8 hours after treatment; fevers peaked at 6-12 hours post-therapy and the events usually abated by 16-24 hours after treatmen…

Functional assessment of atrial wall excursion and foramen ovale flap tracings in 3rd trimester as predictor of short-term hemodynamic stability in congenital heart defects fetuses.

OBJECTIVES: Right atrium and left atrium of the fetal heart play a fundamental role in fetal heart circulatory physiology. METHODS: Excursion of fetal atria walls and tracings of foramen ovale (FO) flap movement were analyzed by M-mode echocardiography by new FO index to determine fetal and neonatal outcome in the first week and month of life in various congenital heart defects. The study group was divided into 3 subgroups: group A: neonates stable after birth (all on I.V. prostaglandin infusion) for at least 7 days, group B: neonates unstable after birth (despite I.V. prostaglandin infusion)

Two-dimensional fast imaging employing steady-state acquisition (FIESTA) cine acquisition of fetal non-central nervous system abnormalities.

PURPOSE: To evaluate the value of two-dimensional fast imaging employing steady-state acquisition (2D FIESTA) cine MR with parallel imaging … ract obstruction. CONCLUSION: With sub-half-second temporal resolution of the 2D FIESTA sequences, fetal movement is no longer problematic. In addition to the anatomical information also provided by conventional SSFSE sequences, 2D FIESTA demonstrates information on motility and peristalsis of hollow organs and helps the diagnosis of fetal visceral anomalies.

Severity of Placental Abruption in Restrained Pregnant Vehicle Drivers: Correct Seat Belt Use Confirmed by Finite Element Model Analysis.

Despite wearing a seat belt, pregnant drivers often suffer from negative fetal outcomes in the event of motor accidents. In order to maintain the safety of pregnant drivers and their fetuses, we assessed the severity of placental abruption caused by motor vehicle collisions using computer simulations. We employed a validated pregnant finite element model to determine the area of placental abruption. We investigated frontal vehicle collisions with a speed of 40 km/h or less involving restrained pregnant drivers with a gestational age of 30 weeks. For a crash speed of 40 km/h, the placental abru

Amniotic fluid infection and its principal sequel bacterial infection of the newborn are major problems in obstetric pathology. The authors … a, premature delivery, urinary tract infections and fetal distress; an apparent lessening in active fetal movements is also well correlated to the risk of infection. The bacteriological criteria are: the presence of quantities of altered polymorphonuclear cells in the amniotic fluid--this seems to be more important than others (sensitivity = 70%, specificity = 8 …

A Peculiar Case of Fetal Distress Leading to the Diagnosis of Diabetic Ketoacidosis in Pregnancy.

Diabetes ketoacidosis (DKA) in pregnancy is associated with significant maternal and neonatal morbidity. It is rare for women without a prior history of diabetes mellitus (DM) to develop DKA. This case report describes an atypical presentation of DKA in a 38-year-old primigravida, with no history of DM, presenting with "unexplained" fetal distress. She presented at 25 weeks to our labor ward triage with an unrelated complaint of prolapsed piles. There were no complaints of reduced fetal movement, abdominal or contraction pains or per vaginal bleeding. Ultrasonography showed an appropriately gr…

[Three-dimensional power Doppler ultrasonography (3D-PDU) in fetal diagnosis].

AIM: 3D power Doppler ultrasonography (3D-PDU) is a new method which allows the spatial presentation of fetal vessels in utero. In the prese … onal demonstration of fetal vessels is possible with the system used. The feasibility is limited by fetal movements and unfavourable fetal positioning. The possible benefit of the method is to diagnose complex fetal vascular malformations in the future.

Discovery of pathogenic variants in EFEMP2 and RAG1 and undetectable fetal phenotype: A challenge of prenatal exome sequencing.

BACKGROUND: Exome sequencing in prenatal context confronts with pathogenic variants associated with phenotypes that are not detectable prenatally. MATERIALS AND METHODS: A consanguineous couple was referred at 24 weeks of gestation for prenatal genetic investigations after ultrasonography findings including decreased fetal movements, hypoplastic male external genitalia, retrognathia, prefrontal edema, anomalies of the great vessels with pulmonary atresia and dilated tortuous aorta. RESULT: Prenatal trio exome sequencing identified two homozygous likely pathogenic variants, i.e. a missense in…

Fetal heart rate patterns in postasphyxiated fetal lambs with brain damage.

OBJECTIVE: We previously showed that in asphyxiated fetal lambs the duration of hypotension correlated well with the severity of histologic damage to the brain, whereas the duration of bradycardia did not. This study compares fetal heart rate patterns with the degree of histologic damage to the brain. STUDY DESIGN: Twelve chronically instrumented near-term fetal lambs were subjected to asphyxia by umbilical cord occlusion until fetal arterial pH was <6. 9 and base excess was <-20 mEq/L. An additional 4 fetuses served as sham-asphyxia controls. Fetal heart rate (from electrocardiogram), arteri

Pre-eclampsia Complicated by HELLP Syndrome in a Primigravida: A Case Report.

Pre-eclampsia is a hypertensive disorder of pregnancy that can rapidly progress to HELLP syndrome, increasing maternal and fetal risks. We report a case of a 33-year-old primigravida at 28+1 weeks gestation who presented with severe hypertension, proteinuria, and reduced fetal movements. Despite initial management, she developed progressive thrombocytopenia, worsening liver function, and oligohydramnios, necessitating close monitoring. Biochemical markers indicated worsening disease, prompting an emergency cesarean section at 28+6 weeks, delivering a growth-restricted neonate. Postoperatively,…

Umbilical artery Thrombosis: A case report of prenatal diagnosis and systematic review of the literature.

Umbilical Artery Thrombosis (UAT) is an extremely rare complication of pregnancy strongly associated with severe fetal distress and death. The pathogenesis is still unclear but it is often associated with anatomical cord abnormalities that leads to blood stasis and thrombosis formation. Other possible risk factors are maternal thrombophilia, autoimmune disease, gestational diabetes, hypertension disorders of pregnancy and Rh-alloimmunization. The most common clinical symptom is the reduction of fetal movements. The diagnosis is histopathological, but it can be suspected by clinical and prenata…

Lethal Congenital Contracture Syndrome 11: A Case Report and Literature Review.

Lethal congenital contracture syndrome 11 (LCCS11) is caused by homozygous or compound heterozygous variants in the GLDN gene on chromosome 15q21. GLDN encodes gliomedin, a protein required for the formation of the nodes of Ranvier and development of the human peripheral nervous system. We report a fetus with ultrasound alterations detected at 28 weeks of gestation. The fetus exhibited hydrops, short long bones, fixed limb joints, absent fetal movements, and polyhydramnios. The pregnancy was terminated and postmortem studies confirmed the prenatal findings: distal arthrogryposis, fetal growth …

Intrauterine death at term in a cocaine user detained under the Mental Health Act.

We report the case of a 31-year-old woman who presented at 23 weeks gestation following a mixed overdose of medication. She was detained under the Mental Health Act 1983/2007 for ongoing suicidal ideation and heavy cocaine use. She had a normal nuchal scan, and unremarkable anomaly and growth scans at 19, 29, 32 and 34 weeks. At 40 weeks, an ultrasound scan confirmed fetal demise. A macerated male stillborn was delivered by forceps. Women with mental health and substance misuse problems may have reduced awareness of fetal movements, and they may be poor historians or frequent attenders for non…

[The biological action of physical factors in the critical periods of embryogenesis].

The critical period in a chicken embryonic development (the 10-13-th days of incubation) is revealed under total electromagnetic radiation. This external factor is a physiologically active irritant which can influence functional state of the brain. The increased absorption of electromagnetic energy takes place in this incubation period. Its dynamics within 20 days of embryonic development has phasic, up and down character. Electromagnetic exposure (4 hours a day daily) in the above mentioned period evokes a delay in embryo adaptive motor behavior (biofeedback learning). Morphological investiga

Antenatal Diagnosis of a True Knot of the Umbilical Cord: A Rare Case With a Successful Surgical Outcome.

True knots of the umbilical cord (TKUCs) are rare and can compromise fetal well-being. Their diagnosis is typically made at delivery, with antenatal identification remaining rare. This case report describes an antenatal diagnosis of a TKUC at 35 weeks' gestation in a 28-year-old third gravida with two previous miscarriages. The patient presented with reduced amniotic fluid and absent fetal movements. Ultrasound imaging raised suspicion of a TKUC, prompting an emergency cesarean section. A healthy neonate was delivered. This case highlights the potential role of color Doppler ultrasound and mat…

Fetal heart rate in the second stage of labor: recording, reading, interpreting and acting.

Although central to the cardinal movements that lead to the delivery of the fetus, the second stage of labor is often neglected from a cultural and clinical viewpoint. In this phase, acquisition is more difficult, and reading and interpretation may not be the same as in the active first phase. Namely, the most relevant findings are the occurrence and depth of bradycardia and its duration. This allows the evaluation of fetal heart rate (FHR) even without cardiotocographic recording. Instead, FHR can be accurately detected using Doppler auscultation, and it is reasonable to speak of "FHR during

A case of spinal cord injury that occurred in utero.

Spinal cord injury is a rare occurrence that is often difficult to diagnose. This report describes a 2-year-old male with cervicothoracic spinal cord injury. The injury is thought to have occurred in the fetal period because the patient's thorax was markedly hypoplastic at birth and his mother had noticed a decrease in fetal movements for several weeks before the birth. The deep tendon reflex of the upper and lower extremities was absent at birth, as observed in other cases of spinal cord injury. It was noteworthy that deep tendon reflex of the upper and lower extremities remained decreased at…

Blows to the maternal abdomen causing fetal demise: report of three cases and a review of the literature.

Traumatic abruption results in 6% of third-trimester gravidas who are hit or kicked in the abdomen during assaults. Three cases are presented in which blows inflicted to the abdomen of pregnant women in their third trimester resulted in the death of the fetus due to abruptio placentae. Two cases were domestic altercations while one was a third-party criminal assault. In all cases the gravida herself escaped significant intra-abdominal injury, and external abdominal findings were minimal. The clinical signs were a history of loss of fetal movements shortly after the assault and loss of fetal he…
