Monochorionic monozygotic twin pregnancy complicated with twin-twin transfusion syndrome presenting with an obvious lambda sign in the first trimester.

In this report, we present a case of twin-twin transfusion syndrome in the presence of the thick dividing wall of each sac in dichorionic twins. The dichorionic diagnosis was based on the presence of the lambda sign at the first-trimester ultrasound evaluation. In addition to fetoscopic and pathological investigation, DNA typing confirmed that the twin set was monochorionic and monozygotic. This case illustrates that although extremely uncommon, the presence of lambda sign does not exclude monochorionic pregnancy.

[Congenital cystic adenomatoid malformations of the lung: diagnosis, treatment, pathophysiological hypothesis].

Congenital cystic adenomatoid malformations (CCAM) of the lung are the most frequent congenital lung malformations. Their diagnosis is based on histological features. CCAM consist of bronchopulmonary cystic lesions which are classified according to the presence and cysts size. Type I CCAM are composed of large cysts (>2 cm) lined by a columnar pseudostratified epithelium. Type II CCAM contain multiple small cystic lesions (<1 cm) lined by a flattened cuboidal epithelium. Type III CCAM are more solid and contain immature structures resembling the pseudoglandular stage of lung development. Ultra

[When congenital lung malformations have to be operated?].

Both technical and anatomical features and the need for a long follow-up, usually over several years, explain the specificity of lung surgery in children. Apart from the oncological resections, the main indications for this surgery are congenital lung anomalies (cystic adenomatoid malformation, bronchopulmonary sequestration, lobar emphysema), symptomatic postinfectious lesions (bronchiectasis, atelectasis, abscess) and pneumothorax. Advances in miniaturization of the surgical equipment are used to carry the majority of these interventions by thoracoscopy, except when performed at the neonatal

Alternative technique for Nd: YAG laser coagulation in twin-to-twin transfusion syndrome with anterior placenta.

Nd: YAG laser coagulation is used to treat severe twin-to-twin transfusion syndrome (TTS). Success of the technique depends on visualization of the placenta, the fetal membranes and the targeted vessels, as well as obtaining an optimal inclination angle for laser coagulation. In the rare case of an extensive anterior placenta, it may be difficult to achieve these conditions using the percutaneous approach. Here, we propose an alternative to the percutaneous procedure. Modifications involve an open access and the use of a flexible cannula and bent scope. An extraplacental area, usually at the f

[Gestation of 42 weeks or longer. Management protocol].

We have evaluated, in a prospective approach to the perinatal results of 98 patients with gestation of 42 weeks and more. The patients with an uncertain gestation age were excluded. The vigilance of the fetus was realized by electronic monitorization, sonographic examination and amnioscopy 55.1% of patients begin labor spontaneously 44.9% were interrupted. The principals causes of interruption were pelvic score of bishop > 7 points and condition that suggest fetal injury. In 31.6% of patients a cesarean section was made. Neonatal morbidity was a 19.4% and we don't have mortality.

Relationship of fetal hemoglobin and oxygen content to lactate concentration in Rh isoimmunized pregnancies.

Fetal blood samples were obtained fetoscopically from 32 Rh isoimmunized pregnancies at 18-32 weeks' gestation, and the hemoglobin concentration, plasma lactate concentration, and oxygen content were measured. When the hemoglobin concentration was more than 8 g/dL, the umbilical arterial and venous lactate concentrations were equal. Abnormal elevations of lactate were found in the umbilical artery at hemoglobin concentrations below 8 g/dL (oxygen content 2 mmol/L) and in the umbilical vein at hemoglobin concentrations below 4 g/dL (oxygen content 2 mmol/L); the arterial lactate values were hig

Impact of fetal intervention on postnatal management of congenital diaphragmatic hernia.

UNLABELLED: We report our experience in the postnatal management of congenital diaphragmatic hernia (CDH) after fetal endoluminal balloon tracheal occlusion (FETO). MATERIAL AND METHODS: Out of 19 CDH fetuses diagnosed since December 2005, 13 had a lung-to-head ratio (LHR) < 1.1 (0.86 +/- 0.11) and underwent FETO at a mean gestational age (GA) of 27.9 weeks. The balloon was removed in 3 cases through puncture in EXIT (ex utero intrapartum treatment); there were 3 trans-utero punctures, 3 fetoscopies and 3 balloons deflated spontaneously. One patient died from bleeding during the FETO procedur

Incidence of histologic chorioamnionitis following fetal surgery and its association with maternal and neonatal outcomes.

OBJECTIVE: Chorioamnionitis is a recognized complication of intra-amniotic interventions. The implications of histologic chorioamnionitis (HCA) in fetal surgery is poorly understood. We aimed to evaluate the incidence of acute HCA following fetoscopic repair of fetal myelomeningocele (fMMC) and its associations with baseline clinical variables as well as maternal, perinatal, and neonatal outcomes. METHODS: A retrospective review at our single fetal therapy center was performed of patients who underwent fMMC repair between April 2018 and October 2023. Acute HCA was based on final placental pat

The Impact of Maternal Body Mass Index on Fetoscopic Laser Surgery for Twin-Twin Transfusion Syndrome.

INTRODUCTION: Both a low and an increased body mass index (BMI) are risk factors for surgical complications. It is less clear whether they also affect the outcomes of fetoscopic procedures. In this manuscript, we aimed to assess the effect of maternal BMI on operative and pregnancy outcomes following fetoscopic laser ablation of placental anastomoses for twin-twin transfusion syndrome (TTTS). METHODS: We retrospectively reviewed all patients with twin pregnancies complicated by TTTS treated with fetoscopic laser surgery at the Ontario Fetal Centre, Toronto, over a 24-year period. Mini-laparotomy to avoid vascular injury during fetoscopic laser ablation in a case of twin-twin transfusion syndrome with anterior wall placenta: Report of a case complicated with renal dysplasia and cord-around-trunk in donor cotwin and review of the literature.

OBJECTIVE: Twin-twin transfusion syndrome (TTTS) is a special complication of monochorionic (MC) twins, with unbalanced flows through vascular anastomoses within the placenta, causing hemodynamic derangement that can adversely affect perinatal outcome. The initial clinical manifestations of TTTS may be hypovolemic and oliguric of the donor cotwin (the "struck" twin), and hypervolemia and polyureic of the recipient cotwin under prenatal ultrasound. However, the mortality rate can reach 90 % if left untreated. Fetoscopic laser ablation (FLA) is considered the golden treatment standard of TTTS. H

Needle Fetal Thoracoscopy: A Technique to Assist with Ultrasound-Guided Placement of Challenging Thoracoamniotic Shunts.

INTRODUCTION: Fetal thoracoamniotic shunts are common lifesaving interventions but frequently require replacement. Needle fetal thoracoscopy is a technique that uses standard thoracoamniotic shunt introducer sheaths to permit direct visualization and even instrument manipulation during shunt deployment to facilitate optimal positioning and primary shunt function in the most challenging cases. CASE PRESENTATION: In this study, 5 patients who underwent needle fetal thoracoscopy-assisted thoracoamniotic shunt placement were reviewed.
