Posterior epistaxis is usually treated by repeated nasal packing and in failed situations by ligation of feeding arteries with considerable morbidity and mortality. The most logical approach should be location of the bleeding site and arrest of haemorrhage by local treatment. A patient with Klippel-Feil syndrome who underwent abdominal surgery is presented and the anomaly reviewed. The anatomical abnormality and potentially unstable neck provide a potentially difficult tracheal intubation which was undertaken using an awake fibreoptic technique. The Mallory-Weiss syndrome was observed in 4.5% of 4154 patients with gastrointestinal bleedings. In 113 patients with arrest of the bleeding the conservative drug therapy was performed, in patients with continuing bleedings (76 patients)-endoscopic diathermic coagulation was fulfilled with complete and perfect hemostasis without complications and lethal outcomes.

A series of 13 perforations of the esophagus in 12 patients is presented. All the perforations were caused by instrumentation. Minimal delay in treatment is advocated. Most importance is attached to gastric decompression, adequate drainage of the mediastinum with parenteral hyperalimentation and, if possible, suture of the lesion.

New fiberoptic scopes, originally designed for biliary tract operations, were used in 23 cases for removal of residual calculi at pyelolithotomy and for examination of the renal pelvis and ureter. The external diameters of the scopes are 6.6 mm. (CHF-X1) and 4.5 mm. (CHF-X2). Water irrigation and forceps manipulation can be performed simultaneously with the CHF-X1 scope but only irrigation can be done with the CHF-X2 scope. The scopes were reliable and allowed easy endoscopic forceps manipulation. Improvements for an ideal fibroptic nephroscope are discussed.

Transbronchial lung biopsy using the fibreoptic bronchoscope was carried out in 85 patients. There were no serious complications; two patients had a 10% pneumothorax and 17 had slight haemoptysis lasting less than 24 hours. The problems of interpreting small biopsy specimens are considered. Satisfactory specimens were obtained without fluoroscopic guidance, particularly in diffuse and lobar lesions. Polypectomy was performed with a special device (a loop and scissors) inserted via the biopsy canal of a fibroscope. During polypectomy diathermic currents from a commerical electric knife are used. An experience with 67 polypectomies has shown that this operation requirs no special pre- and postoperative measures. Failures of polypectomy were as follows: inadequate polypectomy and development of pseudorecurrences, "a loss" of a polyp during its removal, an impossibility ot loop a polyp.
