[Swivel luxácia Chopartovho kĺbu.].

The authors report a special type of Chopart dislocation named as swivel dislocation or tarsal rotation. This injury is characterized by dislocation of the talonavicular joint but the calcaneocuboid joint remains intact. The foot creates a typical rotational movement but it does not evert or invert. The axis of rotation is interosseous talo-calcaneal ligament, which remains intact. In the reported case the medial swivel dislocation is present in combination with fracture-dislocation of the ankle joint with diaphyseal fracture of fibula and segmental fracture of tibia in the same extremity in mu

Which is superior in the treatment of AO Type 42A tibial shaft fracture? A comparison of talon intramedullary nailing and conventional locked intramedullary nailing.

BACKGROUND: This study aimed to compare the clinical and radiological outcomes of conventional locked intramedullary nailing (IMN) and talon IMN in AO Type 42A tibial fractures. METHODS: A total of 93 patients with AO Type 42A fracture were retrospectively analyzed. The patients were divided into two groups: Those treated with conventional IMN (Group 1), and those treated with talon distal locked nailing (Group 2). The patients were statistically compared in terms of age, sex, mechanism of injury, follow-up time, time to union, smoking status, presence of open fracture, presence of concomitan

Dimensions of the Lateral Malleolar Fossa and Its Potential Violation With Lateral Distal Fibular Plate Fixation.

A previously undescribed pitfall of lateral distal fibular locking plates is the risk of violating the lateral malleolar fossa (MF). No previous study has described the dimensions of this fossa. All cases using a lateral distal fibular plate for a fibula fracture from December 2012 to December 2015 (n=365) at a single institution were reviewed. Screws that violated the medial fibular cortical density corresponding to the MF were identified as "at-risk" screws. Available preoperative computed tomography (CT) scans were reviewed (n=69) to measure MF dimensions. Of 365 patients, 115 (31.5%) patie

The current status of free vascularized bone grafts.

In the last decade, free vascularized bone transfer has become an established technique, with success rates above 90 per cent reported in most series. These operations have special application for those situations in which the bone defect is large, the recipient bed is poorly vascularized, and there is an associated soft tissue deficiency. The donor sites now include the fibula, iliac crest, rib, metatarsal, radius, and scapula. Because this is a rapidly expanding field of surgical endeavor, much of the work is unpublished and information in some instances has been obtained necessarily from re

Tibialization of Fibula in an Implant Failure Non-Union Poliotic Limb: A Case Report.

INTRODUCTION: Non-union of long bone fractures, particularly of the tibia, presents considerable challenges due to poor vascularity, minimal soft-tissue coverage, and complications such as implant failure or infection from previous surgeries. Tibialization of the fibula remains a viable, though underreported, technique for addressing such complex cases. CASE REPORT: We report a case of a 26-year-old male with a history of poliomyelitis who presented with non-union of the left tibia following implant failure. Clinical evaluation revealed 4 cm limb shortening and varus deformity. Radiographs sh

Stress fractures caused by physical exercise.

A series of 142 stress fractures caused by sporting activities and physical exercise is presented. 121 fractures occurred in athletes and 21 in non-competitive sportsmen. Distance runners presented with 68 fractures, skiers 12, sprinters 10, orienteering runners 9, vaulters 3, and football-players 3 fractures. Athletes engaged in other events had fewer stress fractures. 76 fractures occurred in the tibia, 26 in the metatarsal bones, 20 in the fibula, 5 in the femoral neck, 4 in the femoral shaft, and 2 in the metacapal bones, lower pubic arch and sesamoid bones of the first MTP-joint. Interposition of posterior tibial tendon in tibiofibular syndesmosis in a bimalleolar ankle fracture: a case report.

Ankle fractures are relatively common orthopaedic injuries; however, irreducible ankle fractures with or without dislocations are a rare, unstable type of injury that require surgical treatment. The structures impeding the accurate reduction of ankle fractures may be soft tissues, such as the deltoid ligament, extensor retinaculum, tendons, or bony fragments between the fracture sites. A 47-year-old male patient with irreducible medial malleolus and distal fibula fracture was referred for treatment. Intraoperatively, it was discovered that failed reduction was due to the interposition of the p

Femoral neck fracture as the sentinel sign of child abuse in an infant: a case report.

UNLABELLED: Child abuse is a significant problem encountered by orthopedic surgeons in the USA. Fractures are the second most common presentation of physical abuse. In this case report, we present a 5-month-old male infant who initially presented with acute hip pain secondary to a femoral neck fracture as a result of abuse. The patient was taken to the operating room for open reduction and pinning of the femoral neck fracture. Further investigation found evidence of fractures of the bilateral femur and fibula at various stages of healing. To our knowledge, a femoral neck fracture in a nonambul

3D-Printed Brace Utilization for an Isolated Fibular Head Fracture: A Case Report.

Isolated fibular head fractures are rare and often result from direct trauma to the lateral knee or varus stress. These fractures, accounting for a small percentage of fibular fractures, can be challenging to diagnose on initial imaging. We present a case of a 65-year-old man who sustained a blow to the right fibular head from a cow's kick. Initial X-rays were inconclusive, but an MRI revealed an impacted fibular head fracture. The patient was managed conservatively with a 3D-printed brace, a walking boot, and crutches for mobility support. The brace, made of carbon fiber nylon with a thicknes

Ipsilateral island fibula transfer for segmental tibial defects: antegrade and retrograde fashion.

Five patients with segmental tibial defects whose contralateral fibula could not be used as a donor were treated with ipsilateral island fibular transfer in an antegrade or retrograde fashion. Antegrade-flow pedicled flaps based on the peroneal vessel as in the conventional free flap were used for the proximal or middle one-third tibial defects, whereas retrograde-flow pedicled flaps based on the communicating branch between the peroneal and posterior tibial vessels were used for the middle or distal one-third of the tibia. All patients had one of the following problems: a previously failed fr

Fatal Cerebral Fat Embolism After Pelvic and Multiple Long Bone Fractures without Associated Lung Injury: A Case Report.

CASE: A 29-year-old motor vehicle collision passenger presented with fractures of the pelvic ring, bilateral femurs, and right tibia/fibula. All fractures were stabilized with external fixation and fixed definitively 48 hours later. Postoperatively, the patient suffered rapid clinical decline. Emergent head computed tomography (CT) demonstrated tonsillar herniation with loss of gray-white matter differentiation. Although respiratory status remained uncompromised throughout presentation, pathology revealed extensive cerebral fat embolism. CONCLUSION: Fat embolism can traverse the lungs without

Coeliac disease presenting with bilateral fibular stress fractures.

We present the case of an apparently healthy 22-year-old female who presented with atraumatic stress fractures of both fibulae. Further investigation demonstrated that she had osteopoenia secondary to occult coeliac disease. The fractures were successfully treated non-operatively. The awareness of the prevalence of occult coeliac disease is increasing due to simpler diagnostic tests, as is its significance as a cause of secondary osteoporosis. Knowing the relationship between these two conditions will help orthopaedic surgeons treating such patients to refer them promptly to appropriate specia

Fat embolism syndrome.

The fat embolism syndrome is clinically evident in approximately 0.5-2.0% of patients with long bone fractures. The clinical signs and symptoms are evident in 60% of patients within 24 hours and 85% of patients within 48 hours after trauma. A patient is reported who complained of dyspnea and hemoptysis approximately 72 hours after sustaining a fracture to the distal tibia and fibula. Radionuclide ventilation/perfusion imaging was obtained to rule out pulmonary thromboemboli. Perfusion imaging demonstrated the characteristic diffuse, subsegmental ("mottled") appearance of fatty emboli to the lu

Hypercalcemia and Multiple Pathological Fractures in Chronic Lymphocytic Leukaemia: A Case Report.

Hypercalcemia is common in some lymphoproliferative disorders such as myeloma or T- cell leukaemialymphoma, but is rarely described in B-cell chronic lymphocytic leukaemia (CLL). A CLL patient who have been presented with multiple pathological fractures and widespread osteolytic lesions is reported. He was a 74 year old male with fractures of his bilateral humerus and radii and multiple osteolytic lesions of skull, fibula, femur and costals. On his admission to the hospital for the fractures he has been diagnosed as CLL. Hypercalcemia has also been documented. All the disorders that could be t

Open versus minimally invasive fixation of a simulated syndesmotic injury in a cadaver model.

BACKGROUND: Malreduction of unstable syndesmotic ankle fractures is common. This study compared the reduction quality of an anterolateral open technique (OT) versus a conventional minimally invasive technique (MIT). METHODS: Fourteen fresh-frozen lower torso specimens with 28 matched lower extremities underwent computed tomography (CT) to measure syndesmosis position before dissection. Reduction was performed using direct visualization and fluoroscopy for the OT group (right-sided specimens) and fluoroscopy only for the MIT group (left-sided specimens). Fixation was achieved with 2 cortical s

Fibular-Sided Avulsion of Posterior Inferior Tibiofibular Ligament-Patel-Sharma Fragment: A Case Report.

CASE: A 30-year-old man presented with left proximal tibia fracture (open Grade 3A) and a transverse lateral malleolus fracture (Weber B) following direct impact injury. Computed tomography revealed a rare posteromedial tubercle fibula fracture at the posterior inferior tibiofibular ligament (PITFL) insertion, with syndesmosis disruption. Initial damage control included an external fixator. Definitive surgery, performed 10 days later, used Herbert screws and a tubular plate, along with syndesmotic screws. CONCLUSION: This case highlights a unique fracture pattern: PITFL avulsion from the fibu

Posterior Tibial Tendon Rupture Associated With Anterolateral Distal Tibial and Medial Malleolar Fracture and a Novel Pattern of Tibiofibular Syndesmotic Injury: A Case Report and Review of the Literature.

A posterior tibial tendon (PTT) rupture associated with ankle fractures is a very rare condition. Ankle pronation and external rotation (PER) movement are the typical traumatic mechanism. This injury is frequently overlooked preoperatively. Early diagnosis and treatment are very important to prevent the serious consequences related to functional PTT insufficiency on biomechanics of the foot. Few cases have been described in the literature that highlight the relationship between PTT rupture and PER type ankle fracture with a medial malleolar fracture. We present a case of a complete PTT rupture

Modern anatomical locking plates are associated with increased postoperative wound complications and unplanned surgical revisions compared to standard tubular plates in the management of unstable ankle fractures: a comparative cohort study in 595 patients.

BACKGROUND: The clinical benefit of locking plates in distal fibula fractures has not yet been proven. In addition, the risk of wound complications appears to be higher than with conventional tubular plates. We hypothesize that the benefits of locking plates in terms of biomechanical properties are outweighed by a higher risk of wound complications. METHODS: We conducted a retrospective review of fibula fractures treated by osteosynthesis with either a conventional one-third tubular plate or an anatomically shaped locking plate from January 1, 2015 to December 31, 2021. We recorded baseline d…
