Generalized joint hypermobility (GJH) often leads clinicians to suspect a diagnosis of Ehlers Danlos Syndrome (EDS), but it can be difficult to objectively assess. Video-based goniometry has been proposed to objectively estimate joint range of motion in hyperextended joints. As part of an exam of joint hypermobility at a specialized EDS clinic, a mobile phone was used to record short videos of 97 adults (89 female, 35.0 ± 9.9 years old) undergoing assessment of the elbows, knees, shoulders, ankles, and fifth fingers. Five body keypoint pose-estimation libraries (AlphaPose, Detectron, MediaPipe

The objectives of this study were to investigate middle finger movements and dynamics of ball movements around the instant of ball release during baseball pitching. Baseball pitching from an indoor mound among 14 semi-professional pitchers was captured using a motion capture system with 16 high-speed cameras (1,000 Hz). Kinematics of middle finger joints, ball rotation, and force applied to the ball were calculated. The proximal and distal interphalangeal joints continued to extend until the instant of ball release, then abruptly flexed. The abrupt flexion lasted for only several milliseconds,…

The proximal interphalangeal (PIP) joint is the articulating joint between the proximal and middle phalanges of the fingers. A dorsal fracture-dislocation of the PIP joint of the fingers with volar plate injuries is an uncommon injury. Few cases have been published in the literature. In this article, we report the case of a subluxation fracture of the PIP joint in a 27-year-old male patient, without pathological history, a manual worker, right-handed, diagnosed 28 days after the injury. The treatment was surgical with open reduction and fixation of the fragment of the base of P2 with osteosutu

PURPOSE: To determine the efficacy of pins and rubber band traction for treatment of comminuted intra-articular fractures in the hand.
METHODS: We performed a retrospective study from 1994 to 2013 to evaluate 33 patients in whom pins and rubber band traction was employed. We clinically evaluated the active range of motion of the affected fingers after surgery. Eleven of the 33 fractures were at the proximal interphalangeal joint, 10 at the distal interphalangeal joint, 5 at the thumb interphalangeal joint, and 2 at the metacarpophalangeal joint of the thumb. The remaining 5 patients had compl

BACKGROUND: Tophaceous gout masses are characterized by the accumulation of monosodium urate crystals in peripheral joints and soft tissues. The most commonly involved areas are the metatarsophalangeal and knee joints. Finger/hand localization is uncommon. If not correctly treated, a finger tophaceous mass can grow and, in rare cases, reach an abnormally large size, termed "giant."
AIM: The aim of our study is to present a rare case of a large tophaceous mass of the hand, localized in the fourth finger, and to highlight the role of surgical excision combined with a multidisciplinary team appr

Anatomically accurate models of a human finger can be useful in simulating various disorders. In order to have potential clinical value, such models need to include a large number of tissue types, identified by an experienced professional, and should be versatile enough to be readily tailored to specific pathologies. Magnetic resonance images were acquired at ultrahigh magnetic field (7 T) with a radio-frequency coil specially designed for finger imaging. Segmentation was carried out under the supervision of an experienced radiologist to accurately capture various tissue types (TTs). The final

Flexor tendon entrapment after a pediatric forearm fracture is a rarely reported complication that is often diagnosed late. Flexor tendon entrapment is more frequently reported after distal forearm fractures, and possible etiologies include fibrosis secondary to hemorrhage at the fracture site and simple entrapment of the muscle belly. This article describes a case of ring finger flexor digitorum profundus entrapment in a 12-year-old boy with a closed both-bone forearm fracture that was treated with closed reduction and intramedullary nail fixation. Preoperatively, the patient had full flexion

Ball sports are the most frequent cause of sport injuries and 60% of all hand injuries involve the fingers. The most common injury is closed rupture of the extensor tendon of the distal interphalangeal joint (mallet finger). Rupture of the deep flexor tendon (jersey finger) occurs particularly in contact ball sports, such as rugby. Injuries of the proximal interphalangeal and metacarpophalangeal joints are of high functional relevance. These injuries frequently represent complex lesions which are demanding both in diagnostics and therapy. Errors in diagnosis or insufficient treatment can lead

The treatment of chronic mallet finger deformities in children can be challenging. Previously proposed for the treatment of chronic mallet fingers in adults, tenodermodesis is a reconstructive procedure that may also be applied to the pediatric population. The technique of tenodermodesis for chronic mallet finger injuries in children is presented here, featuring careful repair of the extensor mechanism and temporary transarticular Kirschner wire fixation of the distal interphalangeal joint. Preliminary clinical results with this procedure have been encouraging, with high patient/parent satisfa

Nineteen patients with a dorsal fracture-dislocation of the proximal interphalangeal joint of a finger were treated with either closed reduction and transarticular Kirschner wire fixation (eight cases) or open reduction and internal fixation, using either one or two lag screws (six cases) or a cerclage wire (five cases). At a mean follow-up of 7 (range 6-9) years, most patients reported satisfactory finger function, even though some of the injuries healed with proximal interphalangeal joint incongruency (seven cases) or subluxation (four cases). Those treated by open reduction complained of mo

Accurate estimation of finger joint stiffness is important in assessing the hand condition of stroke patients and developing effective rehabilitation plans. Recent technological advances have enabled the efficient performance of hand therapy and assessment by estimating joint stiffness using soft actuators. While joint modular soft actuators have enabled cost-effective and personalized stiffness estimation, existing approaches face limitations. A corrective approach based on an analytical model suffers from actuator-finger and inter-actuator interactions, particularly in multi-joint systems. I…

In 2 patients, a man aged 30 and a woman aged 31, a hyperextension trauma of the proximal interphalangeal joint of the right index finger and the left middle finger respectively was diagnosed. The man underwent surgery for volar plate rupture in the acute phase. The woman presented 7 weeks after the trauma with a chronic flexion contracture; she had an avulsion fracture of the second phalanx and was treated conservatively with splints and exercise therapy. The hyperextension trauma of the proximal interphalangeal joint of a finger is one of the most frequently occurring hand traumas. In the mo

Dynamic splinting is a well-accepted modality in gaining joint motion in the injured hand. Presented is a splinting design system referred to as "low profile" dynamic splinting whereby high outriggers are avoided. A review of the literature reveals that this technique is based on the original design approach used by Dr. Sterling Bunnell. Described are the basic principles of the low profile design system, with illustrations of the system in specific splints and specific construction details. This splinting system is indicated for a stiff hand that has sustained direct trauma. Hands with a musc

This observational study aimed at investigating pain in stroke patients with upper limb spastic dystonia. Forty-one consecutive patients were enrolled. A 0-10 numeric rating scale was used to evaluate pain at rest and during muscle tone assessment. Patients were asked to indicate the most painful joint at passive mobilization (shoulder, elbow, wrist-fingers). The DN4 questionnaire was administered to disclose neuropathic pain. All patients were assessed just before and 1 month after incobotulinumtoxin-A treatment. Pain was present in 22 patients, worsened or triggered by passive muscle stretch

OBJECTIVES: To evaluate patients with mutilans-like hand deformities in chronic inflammatory joint diseases and to determine radiographic scoring systems for arthritis mutilans (AM).
METHODS: A total of 52 patients with severe hand deformities were collected during 1997. A Larsen hand score of 0-110 was formed to describe destruction of the hand joints. Secondly, each ray of the hand was assessed individually by summing the Larsen grade of the wrist and the grades of the MCP and PIP joints. When the sum of these grades was > or = 13, the finger was considered to be mutilated. A mutilans hand

Background: Although extension block pinning for mallet fracture is popular, it occasionally results in poor outcome. We reviewed previous cases to elucidate the factors associated with poor outcome. Methods: From 2012 to 2017, 50 mallet fingers in 50 patients were consecutively repaired by extension block pinning using modified Ishiguro method. Inserted Kirschner-wires (K-wires) were removed at 6 weeks, followed by night splinting in extension. For outcome evaluation, distal interphalangeal (DIP) joint motion was measured and classified as either good or poor. Poor outcome was defined as eith
