[Spontaneous external biliary fistula].

Spontaneous biliary cutaneous fistula is a rare complication of inflammatory perforation of the gallbladder. We describe a 70-year-old woman who developed a fistula between the gallbladder and the skin following drainage of an abdominal wall abscess. In such cases primary drainage of the abscess alone is advisable. Only after the active infection has subsided should definitive cholecystectomy and excision of the fistulous tract be carried out.

[Cholesteatoma and labyrinthine fistula].

43 cases of large labyrinthic fistulas caused by a cholesteatoma of the middle ear were studied (33 External Semi-circular Canal fistulas, 4 External Semi-circular Canal amputations with opening of the vestibule, 3 of the promontory, 2 of the oval window, 1 of the Semi-circular Canal). The audiometric, preoperative, radiological data was analyzed. The radiological exam is often disappointing for the average-sized fistulas. Preoperative labyrinthization is moderate for External Semi-circular Canal fistulas and even for certain vestibular amputations; it is often incomplete in the other location…

Treatment of labyrinthine fistula.

This cholesteatoma series comprises 84 ears, 81 of which had a labyrinthine fistula and 3 a horizontal semicircular canal opening that arose as a surgical complication. In 49 ears (58.3%), the operation was a primary one; in 35 ears (41.7%), it was a revision. Of all ears, 21 (25%) were deaf preoperatively. The fistula was located in the horizontal canal in 76 ears (90.4%). The matrix was removed in all these ears, and the fistula was covered with fibrin glue and fascia or periosteum. Hearing was preserved in all 57 ears in which matrix removal was carried out as the planned last stage. These …

[Treatment of rectal fistula].

New methods of operations for high extrasphincter fistulas of the rectum have been developed consisting in dissection of the sphincter passage from the side of perineum to its inner opening and closure of the opening from the side of the intestinal lumen with a flap of wide femoral fascia or bioelastic material with the help of medical glue MK-7.

Tubercular fistula in ano.

Of 122 patients with fistulae in ano operated upon in a 5-year period, 19 (15.6 per cent) were found to have tubercular fistulae on histopathological examination. There was no characteristic clinical picture and concomitant pulmonary tuberculosis was present in only three patients. Histological examination of the excised fistula is mandatory for the diagnosis of anal tuberculosis. There were no cases of Crohn's disease of the anal region.

Primary iliac-appendiceal fistula.

Primary aorto-appendiceal fistula has been reported only once previously. A further case is presented in which bleeding, per rectum, occurred in association with rupture of an aorto-iliac aneurysm adherent to an acutely inflamed appendix. Postoperatively the patient had several complications including permanent asymmetrical L1 paraplegia and delayed presentation of an intramedullary abscess of the femur which grew Bacteroides fragilis organisms. These complications highlight the high morbidity associated with aortocolic fistulae.

Trends in tracheoesophageal fistula.

The changing trends in the management of patients with esophageal atresia, with or without tracheoesophageal fistula during 1974 to 1983, and its effect on survival and morbidity rates are analyzed herein. Of 139 patients in this series, 44 per cent were in Waterston group C. The over-all mortality rate was 25.5 per cent but this decreased consistently after 1980 to 11.0 per cent in 1983. The incidence of infants in the series who weighed less than 1.8 kilograms at birth increased from 7 per cent before 1979 to 18 per cent after 1979 and the survival rate increased from zero to 50 per cent. Al…

Reno-colic-cutaneous fistula.

A case of reno-colic-cutaneous fistula is presented. Of several diagnostic methods, fistulography combined with computerized tomography was the most useful to locate the precise anatomical site of involvement. Total nephrectomy, resection of the descending colon and excision of the retroperitoneal abscess were successfully performed.

[Postarteriographic vertebral arteriovenous fistula].

The vertebral artetio-venous fistulas are seldom reported as a complication of the carotid angiography. As emphasized in this case report it's difficult to identify the place of the communication between the vein and the artery. It explains that two operations were necessary before the total exclusion of the fistula. The study of the literature recalls the main clinical features in that sort of fistula, constituted by the subjective and objective perception of a hum. The more often related complications consist in vertebro-basilar insufficiency, symptoms of root or spinal-cord squeezing, ische…

[Congenital renal arteriovenous fistula].

On the occasion of a recent observation in our Centre of a case of congenital renal arteriovenous fistula, we make a review of the literature and give a detailed description of the clinical case presented. We stress the difficulty in differentiating between congenital and acquired fistulae and explain the course of therapy currently used.

Experience with colovesical fistula.

Experience with 37 patients with adult colovesical fistula over the past 19 years is reviewed. Specific guidelines for treatment of adult colovesical fistula are influenced by the location and cause of the fistula, the patient's general condition, the presence of a pelvic abscess and the presence of colonic obstruction. When criteria are met, a one-stage procedure is safe. The two-stage approach should enjoy wider application, with the three-stage approach reserved for patients who are unprepared or who have a large pelvic abscess. In patients with colovesical fistula due to cancer, the extent…

[Enterovesical fistula (author's transl)].

The authors get a starting point from describing a case with undecided etio-pathogenesis, to focalize the entero-vesical fistulas that represent the most frequent variety of entero-urinary fistulas. Particularly they stop a little on manifold aspects of etio-pathogenetic and classificative order, refer anamnestic, clinical, instrumental and radiological data which allow to get an undoubted diagnosis, emphasize the high rates of complete recoveries in relation to the timely application of adequate surgery.
