BACKGROUND: The microsurgical anterior approach to the cervical spine is commonplace. Fewer surgeons perform posterior cervical microsurgical procedures on a routine basis for lack of indication, more bleeding, persistent postoperative neck pain, and risk of progressive misalignment. In comparison, the endoscopic technique is preferentially performed through the posterior approach. Many spine surgeons and even surgeons versed in lumbar endoscopy are often reluctant to consider endoscopic procedures in the cervical spine. We report the results of a surgeon survey to find out why.

OBJECT: In this study the authors report on the clinical outcomes, safety, and efficacy of lateral retroperitoneal transpsoas minimally invasive surgery-lumbar interbody fusion (MIS-LIF) at the L4-5 disc space in patients with spondylolisthesis. This approach has become an increasingly popular means of fusion. Its most frequent complication is lumbar plexus injury. Reported complication rates at the L4-5 disc space vary widely in the literature, bringing into question the safety of MIS-LIF for the L4-5 region, especially in patients with spondylolisthesis.
METHODS: The authors retrospectively

OBJECTIVE: Headache is a common complaint associated with cervical spondylotic myelopathy (CSM), presenting in more than one-third of patients. Surgical treatment of CSM has been linked to improved headache symptoms. The etiology of headache associated with subaxial cervical spine disorders is not well understood, nor is the mechanism of surgery in relieving symptom intensity. The authors hypothesized that surgical treatment of CSM has a positive impact on patients with headache symptoms, and that anterior cervical discectomy and fusion (ACDF) provides simpler access to foraminal decompression

PURPOSE OF THE STUDY: The aim of the study is to present our surgical method of treating degenerative spondylolisthesis, which includes radical bilateral laminectomy to relieve compression on the spinal cord, transpedicular fixation of the segment and arthrodesis by bilateral intra-articular fusion.
MATERIAL AND METHODS: This surgery was indicated in patients with grade I or grade II of degenerative sponylolisthesis with a 4-mm or more slippage. Our prospectively studied group consisted of 46 patients (17 men, 29 women; average age, 64.2 years; range, 39-84 years). Before surgery and at 1 yea

PURPOSE OF THE STUDY: The aim of this retrospective study was to evaluate the results of intra-operative myelography as the method used to assess the reduction of bone fragments from the posterior margin of the vertebral body.
MATERIAL AND METHODS: Forty patients with 42 comminuted fractures of the thoracolumbar spine were included in the study. The pre-operative spinal stenosis caused by bone fragments from the posterior margin of the vertebral body, as detected by CT scanning, ranged from 25 % to 85 %. Neurological deficit was due to injury in 19 patients and in one it developed post-operat

BACKGROUND CONTEXT: Lumbar discectomy is a common procedure following which emergency department (ED) visits may occur. Although many quality improvement initiatives target reemissions, ED visits may be more common, be a marker of quality of care, affect patient satisfaction, and contribute to health-care resource utilization and costs.
PURPOSE: To analyze the timing and risk factors predicting ED utilization following lumbar discectomy and thereby facilitate better-targeted risk reduction.
STUDY DESIGN/SETTING: Retrospective database review of the 2010 to April 30th, 2021, M157Ortho PearlDi

Introduction Vertebroplasty and kyphoplasty are widely performed for osteoporotic vertebral compression fractures (VCFs). The objective of this study was to compare long-term clinical outcomes between patients undergoing vertebroplasty and kyphoplasty for osteoporotic VCFs. Methods We used the TriNetX US Collaborative Research Platform to identify adults aged ≥50 years with osteoporotic VCFs treated with vertebroplasty or kyphoplasty between 2000 and 2025. Patients with malignancy were excluded. Cohorts were propensity-matched 1:1 (7,528 per cohort) on demographic, comorbidity, and bone health

UNLABELLED: Posterior vertebral column resection (pVCR) is a powerful tool for correcting rigid spinal deformity; however, it is a technically demanding procedure and may be associated with a substantial rate of complications1. pVCR is often reserved for appropriately selected patients with severe focal deformity, in whom soft-tissue releases and posterior column osteotomies alone are unlikely to achieve satisfactory correction. Surgeon experience has also been shown to be correlated with outcomes2, placing further emphasis on appropriate training and practice before performing pVCR.

BACKGROUND: Minimally invasive surgical transforaminal lumbar interbody fusion (MIS-TLIF) is an increasingly common procedure for the treatment of lumbar degenerative pathologies. The MIS-TLIF technique often results in less soft-tissue injury compared with the open TLIF technique, reducing postoperative pain and recovery time1-3. However, the narrow surgical aperture of this minimally invasive technique has increased the difficulty of interbody cage placement. Expandable cages were designed to improve ease of insertion, improve visualization around the cage on insertion, reduce neurological r

BACKGROUND: The objective of this study was to analyze readmission rates after outpatient transforaminal endoscopic decompression surgery for lumbar foraminal and lateral recess stenosis done in an ambulatory surgery center. Endoscopic lumbar spinal surgery is gaining popularity for the treatment of lumbar disc herniations. Recent advances in surgical techniques allow for percutaneous endoscopically assisted bony decompression for neurogenic claudication symptoms due to spinal stenosis. The surgery can be done under local anesthesia and sedation. Patients may be discharged home within hours fr

BACKGROUND: Percutaneous transforaminal endoscopic discectomy (PTED) is a minimally invasive technique for the treatment of symptomatic lumbar disc herniation (LDH) that is growing in popularity. The procedure involves the insertion of a transforaminal spinal endoscope for direct access and removal of intra and extra-foraminal disc fragments1.
DESCRIPTION: The patient is preferably placed in a prone position. A spinal needle is advanced under fluoroscopic guidance into the foramen to the medial border of the inferior pedicle. A guidewire is introduced through the needle cannula, and sequentia

Purpose Pseudarthrosis within the spine tumor population is increased from perioperative radiation and complex stabilization for invasive and recurrent pathology. We report the radiographic and clinical rates of pseudarthrosis following multiple courses of instrumented fusion and perioperative stereotactic radiosurgery (SRS). Methods We performed a single institution review of 418 patients treated with non-isocentric SRS for spine between October 2002 and January 2013, identifying those with spinal instrumentation and greater than six months of follow-up. Surgical history, radiation planning,

BACKGROUND: Vitamin D deficiency has been linked to poor outcomes after spinal deformity correction, but it is unclear whether osteoporosis is the underlying factor. This study analyzed a national administrative database to clarify this association.
METHODS: The 2010 to Q1 2022 PearlDiver M161 database was queried for adult spinal deformity surgeries. Patients were stratified by not having versus having vitamin D deficiency diagnosis and matched 4:1 based on age, sex, and Charlson Comorbidity Index (CCI), and multivariate analysis (controlling for age, sex, and CCI) for 90-day postoperative a

Minimally invasive neurosurgical techniques offer reduced surgical trauma, lower complication rates, and faster recovery compared to traditional open procedures. Approaches such as keyhole craniotomy, endoscopic skull base surgery, endovascular neurosurgery, and percutaneous spinal techniques have advanced the management of cranial and spinal pathologies. These methods aim to minimize tissue disruption, blood loss, and hospital stay while maintaining surgical efficacy. This narrative review explores their evolution, applications, and advantages in modern neurosurgery.
