Fivefold Fracture in a Perilunate Fracture Dislocation Involving Scaphoid, Capitate, Hamate, Triquetrum and Lunate: A Case Report.

This case report presents a very rare fracture combination in a perilunate dislocation including the scaphoid, capitate, hamate and triquetrum and the cornu anterior of the lunate, with an intact scapholunate ligament in a left wrist. An open reduction and internal fixation of the scaphoid, capitate, hamate and triquetrum was performed.

Comparison of Fracture Location and Extent of Comminution in Isolated Scaphoid Fractures Versus Transscaphoid Fracture-Dislocations.

Background: Isolated scaphoid fractures (ISFs) are common, whereas transscaphoid fracture-dislocations (TSFDs) are not. Scaphoid fracture location and the extent of comminution are factors that affect treatment and outcome. The purpose of this study is to compare the radiographic characteristics of ISFs with TSFDs associated with greater arc injury. Methods: This study is a retrospective review of all ISFs and TSFDs that presented to our institution during a 5-year period. Fracture location (along the long axis of the scaphoid) was calculated by dividing the distance from the proximal pole to

Rare Monteggia Type IV Fracture With Posterior Radial Head Dislocation and Concomitant Glenoid Fracture: A Case Report.

Monteggia fractures are uncommon ulnar fractures with associated radial head dislocation, which are classified into four Bado types. Type IV lesions, involving both forearm fractures with anterior radial head dislocation, are particularly rare. This case describes an exceptionally rare variant in a 23-year-old male patient following a motor vehicle accident: a Bado Type IV Monteggia fracture with posterior radial head dislocation, accompanied by an ipsilateral Ideberg type III glenoid fracture, which was initially missed. Despite prior open reduction and internal fixation (ORIF) of the forearm

Compound fracture of humeral shaft associated with two-part fracture dislocation of ipsilateral shoulder: a rare combination.

Humerus fracture is very rarely associated with ipsilateral shoulder dislocation. Here, we have reported probably the first case of compound fracture of humerus associated with a two-part fracture dislocation of ipsilateral shoulder. A 30-year-old female presented to the emergency department after a road traffic accident. The vehicle in which she was travelling skidded off the road and she got trapped. She had degloving injury over her left arm with a fracture of left humerus and anterior dislocation of ipsilateral shoulder and greater tuberosity fracture. She was managed initially with wound …

Traumatic posterior fracture-dislocation of the hip with fracture of the femoral head or neck, or both.

To determine the prognosis and best treatment for patients who have a posterior dislocation of the hip associated with a fracture of the femoral head or neck (Grade IV, according to the classification of Stewart and Milford), we surveyed the records of 201 patients who had been treated for 203 posterior dislocations from 1958 to 1985 and selected the cases of 19 patients (19 posterior fracture-dislocations of the hip) for further review. Each of the injuries had resulted from a motor-vehicle accident. Thirteen patients had had a posterior dislocation with an associated fracture of the femoral

A Rare Case of Multiple Carpometacarpal Joint Fracture-Dislocations with Fractures of the Hamate and Capitate.

INTRODUCTION: Carpometacarpal (CMC) joint dislocations and fracture-dislocations are infrequent and often challenging to diagnose. Among these, dorsal fracture-dislocations involving the capitate and third metacarpal, as well as the hamate with the fourth and fifth metacarpals, represent an exceptionally rare subset. These injuries are typically the result of high-energy mechanisms, such as axial loading transmitted through a clenched fist striking a solid surface. Their subtle clinical signs and often inconspicuous radiographic findings, particularly in the context of polytrauma, contribute t

Ulnar-sided carpometacarpal fractures and fractures-dislocations. A systematic review and publication guidelines.

BACKGROUND: Intra-articular fractures and fracture-dislocations of the ulnar-sided carpometacarpal joints (US-CMC) are common and under-reported. Their diagnosis is often delayed or incomplete, and there is no consensus as to the best treatment. The aim of this systematic literature review was to focus on the quality of existing data and to describe in detail the available evidence on the diagnosis and treatment of these lesions. Based on our findings, we will propose guidelines for improving the design of future studies on these lesions. METHODS: This systematic literature review looked at a

[Treatment of pediatric radial neck fractures: consideration of patient age and fracture dislocation].

Fractures of the radial neck represent approximately 5-10% of all injuries of the elbow in children. Depending on the degree of radial head displacement, either an operative intervention or conservative treatment is recommended. The medical records of 168 children treated between 1999 and 2008 were analyzed and fractures were classified according to the Judet classification system. The fractures were treated according to an algorithm depending on age and fracture dislocation and 103 type I, 21 type II, 30 type III and 14 type VI fractures were found. Conservative treatment was possible in 124 …

Functional reconstruction of the TM joint in cases of severely displaced fractures and fracture dislocation.

In a clinical and axiographic study the outcome of patients with severely displaced fractures and fracture dislocations of the mandibular condyle was evaluated. Two operation methods were compared one via an intraoral approach without joint revision and another via a preauricular approach with open reduction of the joint. In the group with joint revision, resorbable material was used for osteosynthesis. Twenty-eight patients (32 joints) treated without revision of the joint and 26 patients (29 joints) with open reduction of the joint were evaluated. The mean observation time following surgery …

Analysis of 75 operated thoracolumbar fractures and fracture dislocations with and without neurological deficit.

Seventy-five surgically treated patients with thoracolumbar fractures and fracture dislocations, operated on between 1978 and 1982 at the Orthopedic Department of the University of Basel, were analyzed. The follow-up ranged from 18 months to 6 years. There were 45 men and 21 women, and 60% of the patients were not more than 30 years old. Additional injuries were common: 30% of the patients had craniocerebral injuries and 20% were polytraumatized. Ninety-six percent of all patients reached a hospital within 6 h, but only 23% initially presented at a center for spinal surgery. Sixteen patients h…

A plea for judgment in management of thoracolumbar fractures and fracture-dislocations. A reassessment of surgical indications.

Until basic knowledge of fracture anatomy and stability includes experiments with comminuted anterior column injuries, posterior column injuries, and these injuries in combination with ligament and capsular injuries, and until the long-term implications of the long fusion in an otherwise healthy patient are fully known, clinicians making decisions about individual patients must assemble all the variables that bear on the patient's ability to heal and become fully functional again before making a decision about surgical stabilization of the thoracolumbar fracture. An assessment of plain roentge

Establishing a Consensus Definition of a Knee Fracture-Dislocation (Schenck Knee Dislocation V) Using a Global Modified Delphi Method.

BACKGROUND: Knee fracture-dislocations are complex injuries; however, there is no universally accepted definition of what constitutes a fracture-dislocation within the Schenck Knee Dislocation (KD) V subcategory. The purpose of this study was to establish a more precise definition for fracture patterns included within the Schenck KD V subcategory. METHODS: A series of clinical scenarios encompassing various fracture patterns in association with a bicruciate knee ligament injury was created by a working group of 8 surgeons. Utilizing a modified Delphi technique, 46 surgeons from 18 countries a

Radial head fractures: loss of cortical contact is associated with concomitant fracture or dislocation.

HYPOTHESIS: Among radial head fractures displaced greater than 2 mm (Broberg and Morrey modified Mason type 2), separation (complete loss of cortical contact) of at least 1 radial head fracture fragment is associated with a complex injury pattern, meaning that there are other concomitant elbow fractures or ligament injuries. MATERIALS AND METHODS: We identified 291 consecutive skeletally mature patients with 296 radial head fractures treated during a 6-year period. Of these, 121 consecutive fractures of part of the radial head displaced greater than 2 mm (type 2) were classified according to

Combined shear fractures of the trochlea and capitellum associated with anterior fracture-dislocation of the elbow.

This article presents two unusual cases of combined shear fractures of the trochlea and capitellum associated with anterior fracture-dislocation of the elbow. The mechanism of injury was thought to be a fall on the dorsum of a partially flexed forearm, in which the trochlea and capitellum are displaced by the fractured olecranon and the radial head.
