Treatment of open tibial diaphyseal fractures by external fixation combined with limited internal fixation versus simple external fixation: a retrospective cohort study.

BACKGROUND: The treatment of open tibial shaft fractures is challenging. External fixation (EF) is comparatively safe in treating these open injuries, meanwhile it has the advantages of easy application, minimal additional disruption, and convenient subsequent soft tissue repair. Nevertheless, its application is accompanied by a series of problems in alignment and bone healing. Therefore, limited internal fixation (LIF), such as cortical screws, has been used based on the external fixator for better therapeutic effect. The aim of this study is to compare the outcomes of EF combined with LIF an

Is arthroscopically-assisted reduction and internal fixation (ARIF) superior to percutaneous fixation in talar neck fractures? A proportional meta-analysis.

INTRODUCTION: Arthroscopically assisted Reduction and Internal Fixation (ARIF) to fix talar neck fractures is progressively gaining popularity. Our aim was to determine if ARIF in this setting might increase the healing rate and reduce the number of complications as compared to percutaneous fixation (PF) alone. METHODS: As per PRISMA guidelines, multiple databases (Scopus, Pubmed, Web of Science and Cochrane) were used to retrieve studies reporting on patients diagnosed with a fracture of the talar neck undergone minimally invasive surgery using screws as exclusive fixation method. Data were

Predictors of intermaxillary fixation (IMF) after open reduction and internal fixation (ORIF) in maxillofacial fractures: A retrospective cohort study.

INTRODUCTION: Open reduction and internal fixation (ORIF) has several advantages over closed treatment modalities of maxillofacial fracture by offering an early return to function and better patient acceptance. The goal is to achieve good occlusion without placing the patient in maxillomandibular fixation (MMF). Nevertheless, there are certain situations when MMF becomes imperative in the postoperative period, even after ORIF. This study aims to identify the factors responsible for MMF following open reduction and internal fixation of maxillofacial fractures. MATERIALS AND METHOD: Maxillofaci

Cost Comparison of Intramedullary Screw Fixation Versus Open Reduction and Internal Fixation With Plate and Screw Technique for Metacarpal Fractures.

BACKGROUND: Intramedullary screw (IMS) fixation for metacarpal fractures is a relatively new fixation technique in comparison to plate and screw constructs. Our hypothesis evaluated whether IMS fixation for metacarpal fractures results in lower overall health care-associated costs in comparison to open reduction and internal fixation (ORIF). METHODS: A retrospective review of patients undergoing IMS fixation for metacarpal fractures at a single center during 2018 to 2022 was conducted. Health care-associated costs included primary operative costs (surgical time and implant(s)) and postoperati

Comparing Fixation Techniques in Metacarpal Fractures: Intramedullary Screw Versus Open Reduction Internal Fixation With Plate and Screw Construct.

PURPOSE: The purpose of this study is to compare intramedullary headless screw (IMN) versus plate and screw fixation (open reduction internal fixation [ORIF]) in metacarpal (MC) fractures, specifically looking at implant cost, operating room time, outcomes, and complications. We hypothesize that IMN will provide at least equivalent outcomes and complication rates compared with ORIF while simultaneously decreasing operating room time and lowering overall costs. METHODS: Patients with MC fractures treated surgically at a single, level-one, trauma center between January 2018 and December 2022 we

Study on the relationship between the timing of conversion from external fixation to internal fixation and infection in the treatment of open fractures of extremities.

OBJECTIVE: To investigate the relationship between the infection rate and the timing of replacement of temporary external fixators with internal fixation, and the timing of immediate or delayed internal fixation after removal of temporary external fixation in the staging treatment modality of open fractures of extremities. METHODS: A retrospective analysis was performed on 122 cases of open fractures of extremities. External fixators were applied at the early stage and replaced with internal fixation when the condition of soft tissues improved and inflammatory indexes dropped to the normal ra

Clinical and Radiological Outcome of Complex Proximal Tibia Fractures Managed with Hybrid/Ilizarov Fixator with or without Minimal Internal Fixation.

BACKGROUND: Complex high grade proximal tibia fractures with associated extensive soft tissue injury pose a management challenge. The timing of surgery and fracture fixation options depend upon the extent of soft tissue damage. Post-operative complications such as wound breakdown, infection and infected non-union are common in such cases managed early with open reduction and internal fixation. Such fractures can be treated with primary closed reduction and Ilizarov/hybrid fixator application. MATERIALS AND METHODS: It is a retrospective cross sectional study conducted at two tertiary care mul

Effect of Early Weightbearing Following Open Reduction and Internal Fixation of Unstable Ankle Fractures on Wound Complications or Failures of Fixation.

BACKGROUND: There is a growing trend toward early weightbearing as tolerated (WBAT) after open reduction and internal fixation (ORIF) of ankle fractures. To date, studies have excluded fractures with associated syndesmotic injuries from their cohorts. METHODS: In this retrospective cohort study, a chart review was performed at a single level 1 trauma center, identifying all unstable ankle fractures that underwent operative fixation between July 2016 and July 2017. After exclusion criteria, 63 patients were identified and 31 were included in the final analysis, with 14 undergoing syndesmotic f

External fixation versus open reduction and internal fixation for tibial pilon fractures: A meta-analysis based on observational studies.

PURPOSE: Tibial pilon fractures remain challenging for an orthopaedic surgeon to repair. External fixation (ExFix) and open reduction and internal fixation (ORIF) are two widely used methods for repairing tibial pilon fractures. However, conclusions of comparative studies regarding which method is superior are controversial. Our aim is to compare ORIF and ExFix and clarify which method is better in terms of reduction and union results and major complications. METHODS: A computerized research of MEDLINE, EMBASE, Springer, and Cochrane Library (before December 2014) for studies of any design co

Management of grade III open dislocated ankle fractures: combined internal fixation with bioabsorbable screws/rods and external fixation.

BACKGROUND: Isolated dislocation of the ankle with grade III open fracture has been scarcely reported. These ankle injuries usually involved mortise fractures, complete dislocation of the tibial astragaloid joint, capsuloligamentous structure disruption, and severe soft-tissue damage. There is no well-recognized regimen that would result in desirable outcomes. METHODS: Sixteen patients with grade III open dislocated ankle fractures were treated immediately with bioabsorbable implants and an external fixator between January 2003 and June 2007. According to the classification system of Gustilo

Open reduction and internal fixation versus hybrid fixation for bicondylar/severe tibial plateau fractures: a systematic review of the literature.

INTRODUCTION: Bicondylar/Schatzker 6 type tibial plateau fractures are a significant challenge to the trauma and orthopaedic surgeon. These injuries tend to be complex, high energy and are activated with significant morbidity. Two surgical methods are commonly in use: (1) hybrid external fixation or (2) internal fixation. We performed a systematic analysis of papers comparing these two techniques. METHODS: The Medline database was used and the MeSH terms associated with bicondylar/severe tibial plateau fractures were inserted. RESULTS: Forty-nine studies were retrieved but only five papers p

Internal fixation of dorsally displaced fractures of the distal part of the radius. A biomechanical analysis of volar plate fracture stability.

BACKGROUND: Volar plate fixation with use of either a locking plate or a neutralization plate has become increasingly popular among surgeons for the treatment of dorsally comminuted extra-articular distal radial fractures. The purpose of the present study was to compare the relative stability of five distal radial plates (four volar and one dorsal), all of which are commonly used for the treatment of dorsally comminuted extra-articular distal radial fractures, under loading conditions simulating the physiologic forces that are experienced during early active rehabilitation. METHODS: With use
