Surgical fixation of pathologic fractures: an evaluation of evolving treatment methods.

The surgical techniques for the fixation of pathologic fractures continue to evolve. The present study examines clinical outcomes and complications associated with the use of newer generation interlocked intramedullary nails compared with open reduction and internal fixation with plates and screws for pathologic fractures in long bones. Fifty operative procedures performed on 37 patients by five community orthopaedic surgeons were retrospectively reviewed. The operative procedures were separated into two groups based upon the method of fracture fixation; the groups were compared for difference

Transarticular stabilization for malunited fracture of the distal tibia in diabetics with loss of protective sensation.

Five diabetic patients who require insulin developed symptomatic malunited stress fracture of the distal tibia and fibula. All were treated with intramedullary fixation crossing the ankle joint. All progressed to stable union without evidence of infection. Two patients treated with antegrade intramedullary nailing required nail exchange for subsidence at the fracture site. One patient sustained a fracture at the proximal tip of a short retrograde nail, requiring replacement with a longer nail. Transarticular intramedullary fixation is an effective method of achieving stabilization for malunite

Technique for Intramedullary Stabilization of Ulnar Neck Fractures.

BACKGROUND: Distal ulna fractures at the ulnar neck can be seen in association with distal radius fractures, and multiple techniques have been described to address the ulnar neck component of these injuries. We have found that treatment of ulnar neck fractures can be challenging in terms of anatomy and fracture fixation. We present a new percutaneous fixation technique for ulnar neck fractures commonly seen with distal radius fractures. TECHNIQUE: Fixation of the ulnar neck fracture is performed after fixation of the distal radius fracture. Our technique uses anterograde intramedullary fixati

Use of light activated intramedullary device for revision of a proximal humerus fracture: a case study.

BACKGROUND: Post-operative non-compliance is a risk factor for fracture fixation failure and presents a challenge for revision surgery planning. We present a patient who underwent revision surgery for a proximal humerus fracture with lateral locked plating augmented with a UV light activated intramedullary implant. CASE: A 45-year-old woman with a history of alcoholism presented with a proximal humerus fracture. After undergoing open reduction internal fixation with a lateral locking plate, the patient suffered a fall secondary to delirium tremens. New radiographs demonstrated displacement of

Limits of internal fixation in long-bone fracture.

Alternatives to internal fixation of long-bone fracture comprise, depending on location, external fixation or joint replacement. Limitations comprise risk of infection and functional outcome quality, which vary according to technique. The present study examines these limitations, based on comparative or large-scale studies from which certain significant results emerge. Four main questions are dealt with: (1) the present role of locking plates; (2) conditions for intramedullary nailing in Gustilo grade IIIb open fracture; (3) the limitations of conversion from external fixation to intramedullar

Clinical Outcomes of Surgical Treatment for Paediatric Forearm Fractures: A Retrospective Analysis of 86 Fractures.

Background: Surgical treatment of paediatric forearm fractures is usually based on age and fracture type. Most patients undergo intramedullary and, occasionally, conventional pinning or plate fixation. However, the optimal fixation method remains controversial. We retrospectively examined the clinical and radiographic outcomes and complications associated with pinning (intramedullary and conventional) and plate fixation. Methods: Surgery was performed for 86 diaphyseal fractures of the forearm in 60 patients aged ≤ 15 years. Their mean age and follow-up duration were 8.7 years and 10.4 months,

Acute surgical management of Jones' fractures.

The purpose of this study was to evaluate the effectiveness of surgical management for acute Jones' fractures and Torg types I and II proximal diaphyseal stress fractures presenting acutely in both athletes and nonathletes. Twenty-two patients underwent intramedullary screw fixation between 1994 and 1999. Immediate intramedullary screw fixation of acute Jones' fractures and type I stress fractures resulted in a 100% union rate with an average time to union of 6.2 weeks. Fixation of type II stress fractures had a union rate of 100% with a mean time to union of 8.3 weeks. The overall complicatio

Finite element analysis of 3 internal fixations for distal type C3 femur fractures with medial wall bone defects.

OBJECTIVES: Managing 33-C3 femur fractures with medial wall bone defects poses a significant challenge for orthopedic surgeons. The gold standard treatment for arbeitsgemeinschaftfür osteosynthesefragen (AO)/orthopedic trauma association (OTA) 33-C3 distal femur fractures with medial wall bone defects remains elusive. This study employs finite element analysis to compare the stability and mechanical behavior of 3 internal fixation patterns (single lateral distal locking plate, retrograde intramedullary nail, and dual plates) for 33-C3 femur fractures with medial wall bone defects. The aim is t

Complication following intramedullary fixation with a Fixion nail in a patient with osteogenesis imperfecta: a case report.

Osteogenesis imperfecta (O.I.) is a genetic disorder with increased bone fragility and low bone mass. We report the history of a 17-year-old male patient with O.I. who presented a fracture of his left femoral shaft. He had osteogenesis imperfecta type I A according to Silence. He had presented two years previously an ipsilateral cervical fracture of the femur which had healed. Intramedullary fixation with a Fixion intramedullary nail was elected. While the Fixion nail was being inflated to 70 bars with saline, a longitudinal fracture occurred in the femoral shaft. A conventional intramedullary

Nail or plate fixation for A3 trochanteric hip fractures: A systematic review of randomised controlled trials.

Continuing controversy exists for the choice of implant for treating A3 trochanteric hip fractures so we undertook a systematic review of randomised controlled trials from the year 2000 onwards that have compared an intramedullary nail with an extramedullary fixation implant for the treatment of these fractures. Data on the occurrence of any fracture healing complications was extracted and the results combined to calculate Peto odd ratio. Nine studies involving 370 fractures were identified. Three studies involving 105 fractures compared an intramedullary nail with a static fixation (condylar,

Characterization of novel intramedullary nailing method for treating femoral shaft fracture through finite element analysis.

Femoral shaft fracture is one of the most common types of fracture encountered in the clinic. For certain complex femoral shaft fractures, the traditional intramedullary nail may not provide sufficient stability. Therefore, novel intramedullary nail systems are required. The femur 3D model and the internal fixation model were designed using Mimics 17.0 (Materialise), Geomagic Studio 2012 (Raindrop) and Solidwork 2016 (Dassault) software. The validity of the models was verified through comparison with previous data in silico. To further simulate the comminuted femoral shaft fracture, the novel

Application of a humeral intramedullary nail for subtrochanteric fracture in a patient with poliomyelitis sequelae: a case report.

OBJECTIVE: To investigate the clinical feasibility and efficacy of using a humeral intramedullary nail for the treatment of subtrochanteric fracture in a patient with ipsilateral poliomyelitis sequelae. METHODS: A case of a 54-year-old female patient with a left subtrochanteric fracture caused by a traffic accident was reported. The patient had a 50-year history of ipsilateral poliomyelitis, leading to developmental deformity and severe stenosis of the femoral medullary canal in the affected limb, which could not accommodate a conventional femoral intramedullary nail. Therefore, we innovative

Percutaneous leverage technique for reduction of radial neck fractures in children: technical tips.

PURPOSE: Paediatric radial neck fractures are challenging to treat. Multiple strategies exist for reduction and fixation; there is no clear consensus on the best surgical technique to achieve reduction. The percutaneous leverage technique is a method for reduction of radial neck fractures that has previously been described by Wallace, though there is a lack of published literature on this technique. We present a technical note and a modest case series on our modification to the percutaneous leverage technique accompanied by intramedullary fixation. METHODS: We describe a retrospective series

Intramedullary nailing in pertrochanteric fractures of the proximal femur.

INTRODUCTION: Pertrochanteric fractures of the proximal femur should be treated surgically, unless the medical condition of the patient does not allow it. Currently, there are two ways to fix these fractures; either with a sliding hip screw or with an intramedullary nail. However, there is much debate over which implant is the best for pertrochanteric fracture fixation. The sliding hip screw has been used over time with good clinical results. While it was true that with first generation intramedullary nails the risk of complications was higher, there is evidence supporting the superiority of i

Treatment of reverse oblique and transverse intertrochanteric fractures with use of an intramedullary nail or a 95 degrees screw-plate: a prospective, randomized study.

BACKGROUND: Intertrochanteric fractures are composed of different anatomic patterns that vary in their degree of stability following open reduction and internal fixation. A particularly unstable group is classified as AO/OTA 31-A3, with the fracture pattern described as reverse oblique or transverse. The purpose of this study was to compare the results of intramedullary fixation with those of plate fixation for these intertrochanteric fractures in elderly patients. METHODS: Thirty-nine elderly patients with AO/OTA 31-A3 intertrochanteric fractures of the femur were randomized into two treatme

Periprosthetic fracture fixation in osteoporotic bone.

Fixation techniques of periprosthetic fractures are far from ideal although the number of this entity is rising. The presence of an intramedullary implant generates its own fracture characteristics since stiffness is altered along the bone shaft and certain implant combinations affect load resistance of the bone. Influencing factors are cement fixation of the implant, intramedullary locking and extramedullary or intramedullary localization of the implant and the cortical thickness of the surrounding bone. Cerclage wires are ideally suited to fix radially displaced fragments around an intramedu
