[Skier's thumb--osseous injury and rupture of the ulnar collateral ligament].

Injuries to the ulnar collateral ligament of the thumb are common in down hill-skiing, and are thus called "skier's thumb". In complete disruption of the ligament, surgical treatment is mandatory to restore stability. The management gets more difficult if a bony lesion is present: does a non-displaced fragment represent a non-displaced bony avulsion of the ulnar collateral ligament, thus allowing conservative treatment? We reviewed sixty-three consecutive patients with an acute skier's thumb injury in order to determine the anatomical nature of injuries in thumbs that were treated surgically e

An unreported cause of rupture of the extensor pollicis longus tendon.

Avulsion fractures of the index metacarpal at the insertion of extensor carpi radialis longus are rare. We report such a fracture and the resulting complication of division of the extensor pollicus longus tendon, by the avulsed bony fragment. Careful clinical assessment and appropriate radiological examination is needed to diagnose this rare fracture and internal fixation is recommended.

Delayed proximal hamstring tendon repair after ischial tuberosity apophyseal fracture in a professional volleyball athlete: a case report.

BACKGROUND: Ischial tuberosity apophyseal fractures are avulsion fractures of the anatomic footprint of the proximal hamstring tendons. Generally, these injuries are rare and frequently occur in skeletally immature, active patients due to incomplete ossification. Depending on the fragment displacement, non-operative or operative treatment approaches are used. CASE PRESENTATION: We report a case of a 29-year-old professional volleyball athlete who has suffered from a nonunion avulsion fracture for 14 years. Isolated suture anchor fixation was performed after open excision of a large bony fragm

[Extra-articular fractures near the knee and concomitant knee damage].

While the prognosis of intra-articular fractures of the knee joint is determined by the reconstruction of the articular surfaces, ligamentous damage and resulting joint instability is a main problem with extra-articular fractures neighbouring the knee. In a follow-up of 43 cases of A 1-3 fractures according to the AO/ASIF classification, 37.2% of the patients revealed concomitant ligamentous damage. Especially supracondylar femoral fractures and the avulsion fractures of the posterior cruciate and lateral collateral ligament are at higher risk.--Ligamentous ruptures accompanying avulsion fract

Avulsion injuries: an update on radiologic findings.

Avulsion injuries result from the application of a tensile force to a musculoskeletal unit or ligament. Although injuries tend to occur more commonly in skeletally immature populations due to the weakness of their apophysis, adults may also be subject to avulsion fractures, particularly those with osteoporotic bones. The most common sites of avulsion injuries in adolescents and children are apophyses of the pelvis and knee. In adults, avulsion injuries commonly occur within the tendon due to underlying degeneration or tendinosis. However, any location can be involved in avulsion injuries. Radi

[Fractures of the scaphoid tubercle].

BACKGROUND: Avulsion fractures of the scaphoid tubercle (Herbert type A1) are rare. Recommendations in the literature focus on a four to six week plaster immobilization including the thumb. Surgery is recommended in case of additional injuries to the carpus. MATERIAL AND METHODS: In a retrospective investigation including a 36 month period we reviewed acute fractures of the scaphoid and avulsion fractures of the scaphoid tubercle. RESULTS: We found 81 acute scaphoid fractures, four of them (4.9 %) were isolated tuberculum avulsions. Three cases were extra-, one case an intraarticular fractur

Partial sleeve fractures of the tibia in children: an unusual fracture pattern.

Sleeve avulsion fractures of the patella are a well-recognized fracture pattern in children. Less appreciated is a similar fracture pattern involving the anterior metaphyseal area of the tibia. These metaphyseal sleeve fractures represent a fracture pattern characterized by the avulsion of a large area of periosteal attachment of the patellar tendon associated with small subchondral fragments of bone, encountered in skeletally immature children secondary to contraction of the quadriceps associated with forced knee flexion. Although an avulsion fracture of the tibial tubercle is the usual resul

Analysis of Extreme Beak Calcaneal Fracture (Type 2-Lee) Fixed with Cannulated Cancellous Screws: An Original Research Article.

INTRODUCTION: Extreme beak calcaneal fractures (Type 2 Lee's tuberosity avulsion fractures) are rare injuries, accounting for only 1.3-3% of all calcaneal fractures. These injuries are considered as surgical emergency as they can lead to significant functional impairment and soft-tissue compromise if not promptly managed. This study evaluates the efficacy of cannulated cancellous (CC) screw fixation for these fractures. MATERIALS AND METHODS: A prospective study of 14 patients with extreme beak calcaneal fracture (Type 2 Lee's - tuberosity avulsion fractures) treated with CC screw fixation wa

Radiography may not be accurate in assessing acute ankle sprains in children.

BACKGROUND: Acute ankle sprains are among the most common injuries in children and are often associated with chondral avulsion fractures and ligament injuries. However, radiography may not be sufficiently accurate for assessing cartilage and ligament injuries in children. The primary purpose of this study was to evaluate the necessity of radiography in the diagnosis of acute ankle sprains in children. The secondary purpose was to assess whether ultrasonography can effectively improve the diagnostic accuracy of acute ankle sprains in children. METHODS: We collected medical data from 78 childre

Thumb Metacarpal Periosteal Sleeve Avulsion: A Case Report.

Periosteal sleeve fractures, or avulsions of cartilage and/or periosteum with or without an osseous fragment in skeletally immature individuals, are notoriously easy to miss and a high index of suspicion is necessary for accurate diagnosis and treatment. While periosteal sleeve avulsion fractures are classically reported in the patella, they have also been reported in the shoulder, clavicle, and elsewhere in the knee. However, no published reports exist for a periosteal sleeve avulsion fracture in the hand. This case details the first reported instance of such an injury involving a thumb metac

Posterior Approach for Single-stage Fixation of Medial Hoffa with Posterior Cruciate Ligament Avulsion: A Rare Case Report.

INTRODUCTION: Medial Hoffa fractures are rare intra-articular injuries of the femoral condyle, and their association with posterior cruciate ligament (PCL) avulsion fractures presents a highly uncommon and complex clinical scenario. This case report describes the successful surgical management of such a combination using a posterior open approach. CASE REPORT: A 48-year-old male sustained a medial Hoffa fracture with a PCL avulsion following a road traffic accident. Surgical intervention involved open reduction and internal fixation (ORIF) through a posterior approach, allowing direct access

Sequential avulsions of the tibial tubercle in an adolescent basketball player.

Tibial tubercle avulsion is an uncommon fracture in physically active adolescents. Sequential avulsion of tibial tubercles is extremely rare. We reported a healthy, active 15-year-old boy who suffered from left tibial tubercle avulsion fracture during a basketball game. He received open reduction and internal fixation with two smooth Kirschner wires and a cannulated screw, with every effort to reduce the plate injury. Long-leg splint was used for protection followed by programmed rehabilitation. He recovered uneventfully and returned to his previous level of activity soon. Another avulsion fra

Misdiagnosed cartilaginous PCL avulsion in young children.

Posterior Cruciate Ligaments injuries are rare in children and usually due to bony avulsion fractures or midsubstance tears. This study focused on cartilaginous avulsions initially misdiagnosed despite of MRI assessment. Two 6-year-old boys had cartilaginous avulsion fracture injury at the femoral attachment of the PCL. One had associated medial meniscal lesion and was reinserted. The other conducted to non-union. MRI second lecture reveals an original description with nail-biting sign on cartilage surface of anterior notch, and a close PCL angle without anterior tibial translation. No bone br

Ulnar styloid fixation in the treatment of posttraumatic instability of the radioulnar joint: a biomechanical study with clinical correlation.

Biomechanical displacement testing was done on nine fresh human upper extremities to define the stabilizing influence of the triangular fibrocartilage on the radioulnar joint and the efficacy of triangular fibrocartilage-ulnar styloid avulsion fracture repair in restoring lost stability. Test data confirmed that the triangular fibrocartilage is a major stabilizer of the radioulnar joint and internal fixation of triangular fibrocartilage-ulnar styloid avulsion fractures can restore preavulsion stability in all positions of forearm rotation. On the basis of this data and a successful clinical ex

Outcome of the functional treatment of first-time ankle inversion injury.

BACKGROUND: Avulsion fractures of the lateral malleoli in ankle inversion injuries are often undetected on routine radiographs. Undetected avulsion fractures have been managed as ankle sprain, which may affect the outcome of the treatment of the ankle sprain. The purposes of this study are to compare the outcomes of functional treatment between the first-time severe ligament injury and avulsion fracture of the lateral ankle, and to investigate how the anterior talofibular ligament (ATFL) view or the calcaneofibular ligament (CFL) view affects the diagnosis of the avulsion fracture and outcome

Isolated rupture of the subscapularis tendon in children: a case report and literature review.

We report the case of an 11-year-old, male, competitive hockey player with a complete rupture of the subscapularis insertion but no avulsion fracture of the lesser tuberosity. Ultrasound and MRI were performed before surgical repair with suture anchors. The boy regained a normal range of movement after 4 weeks of immobilization and 2 months of rehabilitation. This unique case (with the absence of lesser tuberosity avulsion) is compared with the 16 other cases of paediatric subscapularis avulsion fracture reported in the literature.

Plantar flexion injuries of the ankle. An experimental study.

A biomechanical study was designed to test the hypothesis that isolated plantar flexion of the ankle can produce avulsion fractures of the medial and lateral malleoli without fracture of the posterior articular surface of the tibia. Cadaver specimens were subjected to a standard plantar flexion force in the Burstein torsion testing machine. They showed the following typical patterns of injury in 26 of 32 ankles: (1) avulsion of the anterior ankle capsule; (2) a short oblique distal fibular fracture or fibular avulsion of the anterior talofibular ligament; and (3) an oblique medial malleolar fr

Fractures of the intercondylar eminence in children and adolescents.

The treatment of choice of completely dislocated fractures of the intercondylar eminence is controversial. Recently, Zifko and Gaudernak [14] introduced a new classification in which they distinguish between two different types of intercondylar fractures: Type A: isolated avulsion of the anterior cruciate ligament Type B: fractures including the intercondylar eminence In order to assess whether this new classification could lead to a better selection of patients requiring open reduction, 19 children were reviewed 2-16 years after they had sustained a fracture of the intercondylar eminence.
